Carcinoma in a true diverticulum of the gallbladder.

Extensive search of the literature reveals the extreme rarity of true diverticulum of the gallbladder. Its frequency varies from 0.0008% of all resected gallbladders at the Mayo Clinic to 0.06% of a series of congenital anomalies of the gallbladder collected from the world literature. We are presenting a case of a true diverticulum of the gallbladder with sonographic demonstration and focal malignant alterations, neither one of which has been reported previously. The difference between true and false diverticula of the gallbladder is discussed.

Prophylaxis of bacterial endocarditis: a general practice audit.

A method of identifying patients at r,sk from bacterial endocarditis is described. All at-risk patients in one practice were identified, contacted and counselled. Twothirds of these at-risk patients had no contact with the appropriate hospital outpatient department. It is suggested that the general practitioner has the primary role in the prevention of this disease.

Calcification of the mitral annulus fibrosus with systemic embolization. A clinicopathologic study of 16 cases.

A review of 1343 autopsies on patients over the age of 40 years revealed 142 patients with calcification of the mitral annulus fibrosus, representing 10.6% of the studied population. Sixteen patients (11.3%) had systemic calcareous embolization. Eight patients showed clinical symptoms reflecting embolization, while the others did not. Three of the patients with clinical manifestations had been admitted to the hospital with a history of a fall and two of these, whose postmortem examination included the brain, showed focal meningitis due to embolization from an infected calcified mitral annulus fibrosus. Another three patients had been admitted to the hospital with a clinical diagnosis of cerebral vascular accident and were found to have focal ischemic necrosis or large cerebral infarcts. These were the result of either calcareous or bland embolization from a thrombus superimposed on an ulcerated calcified annulus. Eight of the 16 patients with calcareous embolization also had had sepsis, six attributable to infective endocarditis complicating the calcified mitral annulus fibrosus. This study suggests the relatively frequent occurrence of spontaneous calcareous embolization from an ulcerated calcified mitral annulus fibrosus in older adults.

Complete chemotherapeutic regression of hepatic metastases with resultant hepar lobatum.

An instance of hepar lobatum of unusual etiology is described. Because metastatic gastric adenocarcinoma in our patient involved multiple organs including the liver, chemotherapy was administered. There was total regression of the liver metastases at autopsy with cicatrization of the previous sites of neoplastic involvement. The scarring subdivided the liver into irregular areas resulting in the characteristic gross appearance of hepar lobatum. Evidence of syphilis or Hodgkin's disease was not found. Radioisotopic liver scans taken before and after the administration of chemotherapy are included to correlate the clinical findings with the pathological observations.

Importance of circulating and urinary tissue kallikrein in the control of acute natriuresis and diuresis evoked by water immersion in man.

In ten human subjects water immersion produced a two-fold diuresis and a three-fold increase in sodium secretion together with a marked fall in plasma renin activity and aldosterone. In contrast, circulating and urinary (active and total kallikrein) remained unchanged throughout the immersion period. Our results indicate that the acute diuresis and natriuresis evoked by water immersion in man is not regulated by circulating or urinary tissue kallikrein.
