Enhanced-latex-agglutination assay for C-reactive protein in serum, with use of a centrifugal analyzer.

This is an improved assay of C-reactive protein in serum, for use with the Baker "Encore" centrifugal analyzer. Features of this assay include: 250-specimen throughput per hour, within-batch CV 2.2%, between-batch CV 2.7%, no antigen-excess problems up to 1000 mg/L, negligible interference from rheumatoid factor, and good correlation (r = 0.99) with radial immunodiffusion. The method is inexpensive and automated, involving no predilution steps. It can be adapted for use in a wide range of systems and can be used for single urgent estimations.

Insulin dependence: problems with the classification of 100 consecutive patients.

We have evaluated the clinical and immunogenetic features of 100 consecutive patients presenting to an adult diabetic clinic who were judged clinically to need insulin therapy but were not sufficiently ill to be admitted to hospital. Over this same period 15 newly diagnosed patients (aged 13-70 years) were started on insulin as in-patients of whom ten were in ketoacidosis. The 100 out-patients, aged 11-75 years at the time of starting insulin, were followed for at least a year. Fifty-six had islet cell antibodies and/or were heterozygous for HLA DR3 and DR4 (Group A) whereas 44 had neither of these markers (Group B). Islet cell antibodies and/or DR3, DR4 heterozygosity were most common in the 70 patients diagnosed below the age of 40 years but were also found in older patients. Patients in Group A were significantly younger at diagnosis (29 vs. 43 years), had a shorter duration of symptoms (17 vs. 61 weeks), were more likely to have ketonuria, and had a lower random C-peptide level at diagnosis (0.2 vs. 0.31 nmol/l). The two groups could not be distinguished by weight, haemogloblin A1 or blood glucose at diagnosis or by diabetic control or insulin dose after one year. The National Diabetes Data Group (NDDG) definition of insulin dependence stresses the importance of HLA types and islet cell antibodies although we found their prevalence to be low in the 30 patients diagnosed over 40 years who clinically were indistinguishable from the younger patients.(ABSTRACT TRUNCATED AT 250 WORDS)

C reactive protein concentrations in neonates: determination by a latex enhanced immunoassay.

A latex enhanced immunoassay on a centrifugal fast analyser was used to compare serum C reactive protein concentrations in maternal and neonatal blood. In the neonate the C reactive protein concentration at birth was less than 1.0 mg/l; the concentration rose slightly during the first two weeks of life. There was no correlation between C reactive protein concentrations in maternal and neonatal sera. No significant difference was found between the C reactive protein concentrations in blood obtained by either heel prick or venepuncture.

Kinetics of the antigen:antibody reaction. Light scattering and particle size measurement using photon correlation spectroscopy.

Many techniques for the estimation of specific proteins measure light scattered by, or the turbidity of, suspensions of antigen:antibody aggregates formed in fluid-phase reactions. Although the shapes of reaction curves obtained are well documented, interpretation of the data in relation to the laws of light scattering and the mechanism of the antigen:antibody reaction are poorly described in the literature. Using photon correlation spectroscopy it has been shown that antigen:antibody complexes continue to grow in size even when no further increases in scatter intensity or turbidity are apparent.

Monoclonal antibodies to human IgG: reaction characteristics in the centrifugal analyzer.

We used monoclonal antibodies to human IgG in a kinetic turbidimetric method with a centrifugal analyzer. Results for total IgG with monoclonal and polyclonal reagents correlated well (r = 0.979). Within- and between-batch CVs compared favorably (3.1 and 4.8%, respectively, for polyclonal antisera and 3.5 and 4.0%, respectively, for monoclonal reagents). The monoclonal antibody combination used was shown to be of high titre, with an avidity toward the lower end of the range seen with conventional antisera to IgG. It displayed reaction characteristics similar to those observed with polyclonal antisera, although the time scale of reaction was extended and the effective concentration range was narrowed. Single monoclonal antibodies may not produce complexes of a size exhibiting measurable turbidity when reacted with soluble antigen. However, evidence is presented for the incorporation of such complexes into precipitates produced by other related antigen-antibody systems.

Quantitation of human total IgG, kappa IgG and lambda IgG in serum using monoclonal antibodies.

Monoclonal antibodies to human IgG have been applied, in parallel with polyclonal antisera, to the routine quantitation of human IgG. Two monoclonal antibodies directed against spatially distinct epitopes have been used in combination to form insoluble complexes exhibiting turbidity. Quantitation was performed using a centrifugal fast analyser and a correlation coefficient of 0.979 was obtained for the two estimations. The technique has been further developed to allow the separate quantitation of kappa IgG and lambda IgG in whole serum.

Light scattering and absorption--developments in immunology.

Fluid-phase immunoprecipitation analysis has undergone two distinct paths of development. One path utilizes existing absorptiometric equipment while the other relies on apparatus constructed specifically for the measurement of scattered light. The principles and instrumentation employed in light scattering and absorption techniques are reviewed with special reference to their applications in immunology. The main features of available analysers are summarised in the table. Specialised equipment is becoming increasingly sophisticated and expensive and it is important to assess critically the benefits, if any, that such apparatus may offer compared with more conventional devices. The latter are often capable of performing the same function with greater versatility. A good quality bench spectrophotometer is a more appropriate multi-purpose laboratory instrument than a specialised nephelometer with its more limited application. The equipment of choice for automated immunochemical protein quantitation, however, is the centrifugal analyser. The capital outlay is compensated for by high throughput, development potential and reduced consumption of reagents (i.e. antisera).

Screening methods for covert bacteriuria in schoolgirls.

Screening tests for bacteriuria based on two different principles were evaluated in1582 schoolgirls aged 5-11 years, and in 26 girls aged 3-16 years attending hospitalwith symptomatic urinary tract infection. Tests for hypoglucosuria, performed by a semi-automated fluorometric method and with Uriglox strips on early-morning urine samples voided after overnight fasting, gave unacceptably high false-negative rates (16.7% and 20.8% respectively). Oxoid and Uricult dipslides were immersed in fresh midstreamspecimens of urine obtained at school and read overnight incubation at 37 degrees C. Both gave comparable results, with low false-positive rates and no false-negative responses. The higher cost of screening by dipslides was halved by using the "dipstream" technique, which also gave no false-negative results. Its false-positive rate of 13.5% could be reduced to 1.8% by disregarding colony counts of 10-8 non-faecal organisms and over per litre, which appear unimportant in schoolchildren. Bacteriuria was found in 2.3% of the schoolgirls; 39% of them had symptons, compared with 7.2% of the healthy girls, and 25% showed vesicoureteric reflux, which in 17% was associated with renalscarring. Since the natural history of covert bacteriuria and its relationship withreflux and scarring remain undetermined further research is required. The dipstreamtechnique offers a simple, reliable, and comparatively cheap screening method which could also be applied in general practice.

Bisalbuminaemia Birmingham - a new variant in an Indian Family. Characterisation and comparative studies with albumin Kashmir.

Bisalbuminaemia has rarely been reported in races of the Indian subcontinet. The variant described in a native of the Punjab is shown to have a similar electrophoretic mobility to albumin Kashmir but different dye binding properties. The dye binding properties are also different from other albumin variants where such studies have been reported.
