[Left temporal arachnoid cyst].

Ocular onset is seldom for an arachnoid cyst. We present the case of a young man whose only symptom when hospitalized, was the unilateral peripheral field contraction. It was afterwards specified the presence of the arachnoid cyst and it's pathogenic feature, the compression of the optic nerve.

[Penetrating ocular trauma--therapeutic approach].

The most important factor for functional and anatomic prognosis of an injured eye is the appropriate emergency assistance. This paper presents a case of penetrating ocular trauma with minor clinical sings which associates severe lesions of posterior pole discovered intraoperatively. Careful history and clinical examination allowed the best surgical management, with complete anatomical and functional recovery of the eye.

[Post-traumatic glaucoma].

This paper presents theoretical aspects concerning the physiopathological mechanisms involved in producing the glaucoma following trauma, especially the lens related glaucoma, correct identification of the ethiopathogeny of the intraocular pressure elevation in this situation is essential for the appropriate management. These aspects are illustrated by two cases of recent glaucoma following trauma lens related (lens subluxation) treated by medication and surgery. The surgical procedure consisted of intracapsular extraction of the lens, anterior vitrectomy, iridectomy and implantation of sulcus sutured posterior chamber lens. We present the technique, the difficulties met intra- and postoperative, evolution and prognosis.

[Premature elevated intraocular pressure after cataract surgery].

We evaluated the frequency of elevated intraocular pressure (IOP) the first 1-3 postoperative days for a group of patients undergoing standard or phacoemulsification extracapsular cataract extraction (ECCE). IOP was measured in the pre and postoperative period using non-contact tonometer. From the 356 operated patients, 21 patients (6%) had early elevated IOP. The frequency of elevated IOP was greater among the patients that received anterior chamber (AC) IOL (25%) as compared to that of patients that received posterior chamber (PC) IOL (5.5%). The viscoelastic substance was responsible for a great percentage of cases (51%). The results show that the majority of elevated IOP is transient, recovered spontaneously or by treatment with carbonic anhydrase inhibitors or beta-adrenergic blockers. Elevated postoperative IOP occurs more frequently in the presence of glaucoma, cataract associated with systemic diseases, in complicated and traumatic cataract.

Studies regarding the efficiency of Supro isolated soy protein in Olympic athletes.

66 Romanian olympic endurance athletes (30 kayak-canöe, 36 rowing; 45 males and 21 females) participated in our trial, for the purpose to make evident possible biological effects of Supro isolated soy protein supply (1.5 g/kg/day, 8 weeks). At random the athletes were divided into two groups: A group (n = 38) and B group (n = 28). Daily programme (4-6 hours daily, endurance and strength), food and medication were similar for all athletes. The athletes of the A group received daily 1.5 g/kg SuproR soy protein, for 8 weeks while the athletes of the B group did not received soy protein supply. Two months soy protein supply induced increases of body mass (about 3 kg, especially by lean body mass), strength indexes, serum proteins, haemoglobin and total calcium and significant decreases of urinary mucoproteins and fatigue after training sessions, especially when comparing to the athletes of the B group. No damages of liver, kidney and metabolism tests were noticed in all athletes; no side effects were noticed and the product was well tolerated by athletes. When applying the cross-over method for other 2 months we obtained the same results. Based on these data the authors support the idea of a beneficial effect of this soy protein supply on the biological preparation of endurance top athletes.

Studies regarding the effects of an antioxidant compound in top athletes.

The authors performed a controlled trial in 15 top junior athletes (road cyclists) in order to make evident some antioxidant effects of a new compound, containing selenium, vitamin E, glutathion and cysteine, prepared as tablets. Nonproteic -SH and lipid peroxides (MDA) in blood have been recorded initially on basal conditions and 3 weeks later--during the time the athletes received daily, before training session, 2 h cycling on road about 70km--2 tablets of the antioxidant product (respectively placebo). Then, after one week we applied the cross over method. 3 weeks of antioxidant treatment induced significant increase of the nonproteic -SH and significant decrease of the lipid peroxides (MDA), especially in comparison with placebo treatment. These results support the idea of some antioxidant effects of this compound which might be useful in the biological preparation of endurance athletes.

Studies regarding the antioxidant effects of selenium on top swimmers.

The authors performed another controlled trial in 33 top swimmers (16 girls and 17 boys) in order to make evident some acute and chronic effects (antioxidant) of selenium. Lipid peroxides (MDA-malondialdehyde), nonproteic--SH (essential glutathione) in the serum and blood lactate had been recorded initially on basal conditions and after 2 h endurance training (swimming) accompanied by a per oral administration of 150 micrograms selenium (respectively placebo); one week later we applied the cross-over method. In another trial we continued the treatment with 100 micrograms selenium daily for 14 days (n = 9), respectively placebo (n = 7) and then we applied the crossover method for another 14 days. The above biochemical parameters were recorded initially at rest, under basal conditions, after 14 days of treatment and again after 14 days of treatment, when crossing-over. No significant changes were noticed after a single dose + 2 h hard training, both under selenium and placebo treatment and also when the cross-over method was applied. Fourteen days of selenium treatment induced significant changes of lipid peroxides (especially when the subjects came after placebo) and nonproteic--SH, compared to placebo, changes which support the idea of some antioxidant effects of selenium which might be useful in endurance athletes.

Evaluation by immunofluorescence of the urinary bacteria, of the therapy effectiveness in urinary infections.

The therapy effectiveness in urinary infections (UI) was investigated in 27 patients by the test of immunofluorescence of the urinary bacteria (IFUB) in relationship with bacteriuria, various other biologic investigations and tests of the immune response. The study revealed a complex involvement of the immune mechanisms in the UI occurrence and persistence, as well as a parallelism between the mechanisms of cellular and humoral immunity in some of the patients. The IFUB test could reveal the eventual germ persistence at the renal parenchyma level even in the absence of significant bacteriuria, thus suggesting the necessity of a long term suppressive therapy in such cases.

Urinary infection in patients with chronic renal failure on hemodialysis.

A group of 37 hemodialyzed patients with chronic renal failure were investigated within a period of 6 months to 9 years. Of these patients five presented quantitatively significant bacteriuria noncorrelated with the causal nephropathy. Of the 9 patients with urologic past history two presented urinary infection. Urinary bacterial immunofluorescence was constantly positive in all the cases in which bacteriuria was significant and, at least in one determination when the germ count was below 10,000/ml. This finding was also supported by the increase of gammaglobulinemia and of the circulating immune complexes in all the cases investigated as well as of the B lymphocytes in one third of cases. Fifty percent of the patients presented T lymphocytopenia with excess of suppressor lymphocytes and deficit of the helper ones. It can, therefore be assumed that in patients with chronic renal failure nondialyzable antigens persist in the renal parenchyma where they stimulate anticorpogenesis but at a low level owing to some immune disturbances present in such patients.
