[Classification of secretory otitis based on cytological analysis of middle ear transudate].

On the basis of the number of cell elements in the cytologic smear of the transudate from the middle ear, secretory otitis media was divided into four groups providing indication for therapeutic procedures. The first group is characterized by a mass of mucus and very scarce cell elements, suggesting a relatively short duration of illness and the healing by conservative therapy and adenoidectomy. The second group contained somewhat more cell elements in the cytologic smear, suggesting a longer duration of illness and requiring the use of a ventilation tubule in the therapeutic procedure. The third group showed a considerable cell infiltration and the appearance of individual collagenic fibres scattered in disorder in the smear. The transudate is dense, sticky, and dark. Its aspiration from the cavum is not easy. The finding suggests a long duration of illness, requiring a more active approach of the otosurgeon. The fourth group is characterized by a very scarce but dense aspirate from the middle ear. The cytologic smear shows an intense cell infiltration and a mass of collagenic fibres, which speaks for a very long duration of illness and for the necessity, in view of the formation of connective tissue in the middle ear, of an explorative tympanotomy coupled with adhesiolysis and the use of the ventilatory tubule.

[Tinnitus: an etiologic, diagnostic and therapeutic problem].

Tinnitus as an etiological, diagnostic and therapeutic problem in everyday otorhinolaryngological practice has been described in this paper and the diseases and conditions leading to tinnitus discussed. A diagnostic procedure, by means of which attempts have been made to establish the cause of the noise has been presented. Contemporary methods of treating tinnitus have been reviewed on the basis of ample descriptions from the literature.

[Secretory otitis media in childhood: epidemiologic and etio-pathogenic aspects, diagnosis and therapy].

Secretory otitis media is still poorly understood. In spite of extensive investigations, there are nevertheless many unsolved problems, from etiology to therapy. This paper reviews the current knowledge on epidemiology, etiology, pathogenesis, diagnosis and therapy of secretory otitis media. The possibilities of both conservative and surgical treatments as well as still ununiform view-points on a surgical procedure are presented. In conclusion, on the basis of abundant literature and our experience, a treatment approach to secretory otitis media in children is proposed.

[Tuberculosis of the throat].

The study included 8 patients with tuberculosis of larynx, in whom the diagnosis was made by an otolaryngologist. The disease was commonly attended by hoarseness. The diagnosis of pulmonary tuberculosis was established by clinical observations in all the patients. But, based on the results of indirect microlaryngoscopy and pathohistologic examination, all the patients were found to have tuberculosis of larynx. They were admitted to special hospitals for adequate treatment of tuberculosis.

[Tuberculosis of the cervical lymph nodes].

Clinical observations and experiences in the treatment of 52 patients with tuberculous cervical lymphadenitis are presented during the period of 15 yrs. In all patients the diagnosis was confirmed by pathohistological analysis of extirpated lymph nodes. In 88% of patients the caseous necrosis form of tuberculosis was histologically verified while in 12% the productive one. In 87% of patients the tuberculous process was localised unilaterally and in 13% bilaterally. Deep lymph nodes were more frequently involved than those on the surface (58%:42%). Most of the patients were in the third, fourth and fifth decade of life. Female patients were more frequent than males (58%:42%). In all patients the chemotherapy with antituberculous drugs was administered. In 42% patients tonsilectomy was performed with or without adenoidectomy depending on age of the patient. Pathohistologically tuberculosis of the tonsillae was found only in 3 cases.

[Tuberculosis of the middle ear].

A female patient with tuberculosis of the middle ear diagnosed postsurgically by patohistologic granulations from cavum tympani is presented. Tuberculosis of the middle ear is a rare occurrence today due to the preventive measures like vaccination of the newborns and specific chemotherapy. However, the disease may represent a diagnostic problem. Together with a case report a review of the literature on tuberculosis of middle ear has been given.

[Epidemiologic analysis of lip cancer in a 15-year period in the region surrounding Varazdin].

The author presents the epidemiological analysis of lip cancer in the sub-region of Varazdin. The parameters such as sex, age, histological type, localization and occupation of all recorded patients with lip cancer were analyzed. Amongst the 103 recorded patients with lip cancer there were 81 (78.64%) men and 22 (21.36%) women. The upper lip involvement was detected in 18 (17.46%) and the lower lip in 85 cases (82.54%). There were 12 examples of basalioma (17.46%), 90 of planocellular carcinoma (87.37%) and a single example of melanoma (0.97%). The majority of patients were aged 60 [29 (28.15%)] and 70 [40 (38.83%)]. At recording time, the major group consisted of peasants and housewives [42 (40.78%)], followed by retired persons [37 (35.92%)] and workers exposed to the influence of sun rays [15 (14.56%)].

[Tuberculosis of the upper respiratory tract].

A patient with tuberculosis of the upper respiratory tract is presented. This disease is--because of the preventive measures, obligatory vaccination of newborns and specific chemotherapy--a very rare occurrence today. Yet the disease may represent the diagnostic problem. In differential diagnosis many other ulcerous and granulomatous diseases especially sarcoidosis and malignant disease should be taken into account. Together with the detailed clinical treatment for establishing the diagnosis the biopsy and pathohistological analysis should be performed.
