The value of ultrasound in the diagnosis of congenital hypertrophic pyloric stenosis.

The charts of 103 children with a clinically and surgically confirmed diagnosis of congenital hypertrophic pyloric stenosis were retrospectively reviewed. We found a significant correlation between sonographic and surgical measurements of the muscular thickness of the pylorus (r = .987, P < .001). In 73.7% (76 cases), the clinical picture of gastric outlet obstruction was present when the thickness of the enlarged pyloric muscle was 3.0 mm or more. In 26.3% (27 cases), the pyloric muscle was less than 3.0 mm wide. For 10 patients in whom the muscle width was less than 2.5 mm by sonography, a barium meal was necessary to confirm the diagnosis. The width of the pyloric muscle is the most important factor in the sonographic diagnosis of pyloric stenosis, and even a width of less than 3.0 mm may be associated with clinically significant obstruction.

[Retrorectal cystic hamartoma].

The pathogenesis of retrorectal cystic hamartomas in the presacrococcygeal space, while known to be related to embryological remnants of the caudal region, is controversial. These lesions are rarely found in children; in the early stages of their development they may be symptomless. Diagnosis is usually made when the tumor is large enough to cause pain or intestinal obstruction, or when it becomes infected. A 2-month-old girl with a retrorectal cystic hamartoma initially treated as a perianal abscess is presented.

[Adenocarcinoma of the appendix].

Adenocarcinoma of the vermiform appendix is rare. It usually produces signs of acute appendicitis and is discovered only on pathological examination. The treatment of choice is right hemicolectomy, usually performed as a secondary operation. 2 cases of appendiceal carcinoma, in a 59-year-old man and in a 70-year-old woman, are described.

Prompt diagnosis of 'acute groin' conditions in infants.

Urgent sonographic examination of the groin was performed in 58 patients under the age of two years. Accurate diagnosis was done in conditions such as: incarcerated inguinal hernia, twisted undescended testis, incarcerated ovary in inguinal hernia and others. Ultrasonography facilitated decision-making and differentiation between surgical and non-surgical conditions.

[Enteroperitoneal anastomosis for short bowel syndrome].

Creation of neomucosa is one of many experimental surgical techniques tried in order to improve the results of the complicated treatment of short bowel syndrome. The syndrome is associated with severe physical and mental retardation and not infrequently may be fatal. In this experimental study on pigs and rats we successfully enlarged the small bowel surface by growing new mucosa on the parietal peritoneum following enteroperitoneal anastomosis. The technique has 2 more important advantages: fluids and electrolytes are absorbed through the peritoneum and bowel transit time is slowed.

[Splenic subcapsular hematoma and long-term hemodialysis].

Uremic patients, or those on maintenance hemodialysis, are at increased risk for spontaneous bleeding. Manifestations include retroperitoneal, costal, hepatic, renal, pericardial, mediastinal and subdural hemorrhages. We describe 2 patients on chronic hemodialysis, a 45-year-old man and a 50-year-old woman. Each developed a subcapsular hematoma of the spleen, diagnosed by sonography and CT scan. They were conservatively managed by low-dose heparin, bed rest and careful monitoring. Frequently repeated ultrasound and CT scans demonstrated complete resolution of the hematomas after 2 and 4 months, respectively.

[Ultrasonic examination of the abdominal wall in surgery].

Ultrasonic (US) examination is a simple, effective and quick method of investigating the abdominal wall. We evaluated various cystic and solid lesions in 26 patients by US. In 24 the US diagnosis was correct and revealed pathologic conditions of the abdominal wall such as abscesses, cellulitis, metastatic tumors and hematomas. In 2 patients lesions of the abdominal wall were missed. All but 3 patients were treated surgically.

[Simultaneous aortic and renal revascularization in chronic renal artery occlusion].

The decision to revascularize an occluded main renal artery is often extremely difficult, especially when aortic and renal occlusive disease co-exist. The main objectives of surgery are preservation of renal parenchyma and possible amelioration of renovascular hypertension, provided the patient's condition permits. In a 52-year-old man with renovascular hypertension and intermittent claudication transfemoral aortography revealed severe atheromatosis of the aorta and iliac arteries. There was total occlusion of the left main renal artery, but the right renal artery was patent. Aorto-femoral bypass was performed using a bifurcated graft. The left kidney was revascularized using an 8-mm straight graft anastomosed end-to-end with the renal artery and the other end was inserted into the distal part of the bifurcated graft, constituting a triple bypass. The postoperative course was uneventful, blood pressure decreased and renal scan showed good left renal function. 6 months later aorto-bifemoral and renal bypass grafts were fully patent.

[Vascular injuries in children].

Vascular trauma in children differs in some respects from that in adults. In infants and children the total blood volume is smaller, and despite effective compensatory mechanisms, critical blood loss is also smaller. Gentle and meticulous technical skill is needed to correct injuries of minute vessels and allow adequate blood flow. The use of synthetic tubes and materials is not recommended, as they do not lengthen as the child grows. Ischemia is a major factor influencing limb growth and function. 3 different types of vascular injuries operated on in the past year are presented.

[Conservative treatment of splenic and liver injuries in children].

Operative and conservative management of 44 children with injuries of the abdomen are reviewed retrospectively. Conservative treatment is well accepted for splenic injury. 12/26 children were thus treated during the period 1984-1988, and in the same 5 years, 9/18 children with hepatic injury were also treated conservatively. Indications for operation in splenic and liver injuries are: hemodynamic instability, evidence of associated severe intra-abdominal injuries, and blood transfusion of over 40 ml/kg during the resuscitation period. There was no mortality nor morbidity related to nonoperative management.

[Chylous ascites imitating acute appendicitis].

Acute accumulation of chyle in the peritoneal cavity is rare. It is usually idiopathic and the diagnosis is made at laparotomy, when indicated because of signs of acute peritonitis. Drainage is the only treatment required and the prognosis is excellent. A 33 year-old man who was operated on for signs of acute peritonitis is described. There was more than a liter of chyle, but no underlying pathological condition was found. 3 months after discharge, physical and laboratory examinations were normal, with no evidence of chylomicron retention disease.

Successful parathyroid gland autotransplantation after total parathyroidectomy in dialysis patients.

Secondary hyperparathyroidism is one of the major complications encountered in patients on maintenance dialysis. Medical management is frequently unsuccessful, and there is often a need to perform a parathyroidectomy. Recurrent clinical secondary hyperparathyroidism may occur as a sequela of subtotal parathyroidectomy and necessitate a reexploration of the neck. Total parathyroidectomy followed by autotransplantation of parathyroid tissue into the patient's forearm bypasses the need for a second operation, and it provides a simple ambulatory procedure, under local anesthesia, for the removal of hyperplastic parathyroid tissue. The results in six patients indicate that this new procedure should be considered the treatment of choice for this condition.
