Use of continuous water quality records for hydrograph separation and to assess short-term variability and extremes in acidity and dissolved carbon dioxide for the River Dee, Scotland.

A combination of continuous (15-min) pH, conductivity and temperature measurements and fortnightly spot-sampled water quality data were used to examine temporal variability and extremes in river water quality in an upland Scottish river: the River Dee at Mar Lodge. An empirical relationship was established for Gran-alkalinity by multiple regression against flow and conductivity for the fortnightly data. Applying this relationship to the continuous data, an estimate of continuous Gran-alkalinity was calculated. The continuous Gran-alkalinity record was used as (1) a conservative tracer in a simple two-component mixing model to determine the relative proportions of near-surface runoff and deeper groundwater contributing to stream flow; (2) to deconvolute the contribution of weathering and sea-salt contributions to stream conductivity; and (3) to calculate the excess partial pressure of carbon dioxide in stream water. The episodic variations in pH, weathering and sea-salt conductivity and excess partial pressures of carbon dioxide (EpCO2) associated with high flow events in the River Dee suggest that hydrological pathways play an important role in determining stream chemistry. The results of the hydrograph separation indicate that groundwater provides an important contribution to stream flow, and that there are large and hydrologically active stores of groundwater within the upper River Dee catchment. Sea-salts have an important influence on stream conductivity, particularly with the onset of storm runoff following summer drought periods. This suggests that sea-salts are concentrated in the upper soil horizons by dry deposition and/or evapotranspiration. EpCO2 behaves non-conservatively and shows marked diurnal variability under low-flow conditions during summer, inducing diurnal pH variations, and indicating the importance of within-river biological processes. This study emphasises the very intermittent nature of water quality extremes with stream spates and the diurnal nature of biologically-induced responses. Fortnightly sampling programmes do not capture the range of high flow extremes, and with sampling undertaken during the working day, biological extremes occurring at night are also missed, introducing bias. This study shows the value of continuous measurements for infilling aspects of these intermittent extremes. However, it is also noted that the relatively simple patterns of response observed from the continuous measurements may well belie a much more varied response at the sub-catchment and hillslope scales, as local chemical and hydrological heterogeneities do occur. Thus, the two-component mixing model used is, in practice, based on catchment integrated values for a range of soil water and groundwater endmembers.

Human airway epithelial cell lines for in vitro drug transport and metabolism studies.

The pharmaceutical industry relies on appropriate in vitro models for the evaluation of drug absorption and metabolism. Despite increasing interest in drug delivery via the lung, there is currently no widely accepted cell culture model of the airway epithelium. This review considers the airway epithelium, the culture of airway epithelial cells and the need for cell lines which can model the airway epithelium. Three of the most promising human bronchial cell lines, 16HBE14o-, Calu-3 and BEAS-2B, are reviewed, with emphasis on their recent application for the study of drug transport, drug metabolism and gene delivery. Current limitations and future directions for the development of these cell lines as models of the airway epithelium are discussed.

Phaeochromocytoma in pregnancy.

The Royal Women's Hospital, Melbourne, is a tertiary referral hospital with over 7,000 deliveries per year. We report 3 cases of phaeochromocytoma in pregnancy at the hospital in the 20-year period since 1976. The first case was unsuspected and resulted in a maternal death. The subsequent 2 cases were diagnosed antenatally with successful outcomes for both mother and baby. The 3 cases demonstrate the importance of an early antenatal diagnosis and the special management problems of phaeochromocytoma in pregnancy. The diagnosis of phaeochromocytoma should be considered in any pregnant women with severe hypertension, particularly if presenting before 20 weeks' gestation, labile, or associated with sweating, palpitations or headaches.

In vivo properties of SB 200646A, a 5-HT2C/2B receptor antagonist.

1. SB 200646A, N-(1-methyl-5-indolyl)-N'-(3-pyridyl) urea hydrochloride, the first reported selective 5-HT2C/2B over 5-HT2A receptor antagonist, (pK1 rat 5-HT2C receptor 6.9, pA2 rat 5-HT2B receptor 7.5, pK1 rat 5-HT2A receptor 5.2) dose-dependently blocked a putative rat model of 5-HT2C receptor activation; 1-(3-chlorophenyl)piperazine (mCPP, 5 mg kg-1, i.p. 20 min pretest)-induced hypolocomotion (estimated ID50 19.2 mg kg-1, p.o.). 2. SB 200646A also blocked another putative in vivo model of 5-HT2C receptor function; mCPP (5 mg kg-1, i.p. 20 min pretest)-induced hypophagia in 23 h food-deprived rats (estimated ID50 18.3 mg kg-1, p.o.). 3. SB 200646A did not antagonize 1-(2,5-dimethoxy-4-iodophenyl)-2-aminopropane (DOI)-induced head shakes in rats at doses up to 200 mg kg-1, p.o., an effect thought to be mediated by 5-HT2A receptors for which SB 200646A has its next highest affinity (50 fold less) after the 5-HT2C and 5-HT2B sites. 4. SB 200646A (20, 40 mg kg-1, p.o., 1 h pretest) also reversed mCPP (0.5 mg kg-1, i.p., 30 min pretest)-induced anxiety in the social interaction test, under low light familiar conditions. 5. When given alone, under high light unfamiliar conditions, SB 200646A (2-40 mg kg-1, p.o.) increased active social interaction without affecting locomotor activity in the rat social interaction test. This is consistent with an anxiolytic action of SB 200646A. 6. These results indicate that SB 200646A has in vivo efficacy and that 5-HT2C or 5-HT2B receptors are indeed likely to mediate mCPP-induced hypolocomotion, hypophagia and anxiogenesis. They also suggest that 5-HT2C,2B receptor blockade induces anxiolysis.

Atypical structured glomerular deposits: an immunohistochemical study.

Atypical structured glomerular deposits were identified in sub-epithelial, sub-endothelial and mesangial areas in biopsy tissue from a female, aged 33, who presented with anaemia and was found to have proteinuria and microscopic haematuria. Histological examination showed that the deposits were periodic acid Schiff positive and silver negative whilst stains for amyloid were negative. Immunofluorescent staining for all immunoglobulins was negative, and only C3 showed moderate labelling. Conventional electron microscopy revealed that all deposits contained microtubular structures of variable length but with an average diameter of 25 nm and a periodicity of approximately 16 nm. The glomerular basement membrane was interrupted in many areas by deposits, and also contained 'myelin-like' structures. Free microtubular structures were also seen in the urinary space. Immunoelectron microscopy using protein-A-gold confirmed the immunofluorescent findings with immunoglobulins and fibrinogen, showed marked positive labelling of deposits with C1s and C3d and also intense labelling of coiled microtubular structures with C9. Other complement components C1q, C1r, C3c, C4 and C5 showed weak or negative results. Although these organised glomerular deposits contain complement components, their pathogenesis remains uncertain.

Urinary erythrocyte morphology in the diagnosis of glomerular hematuria.

Urine samples from 141 consecutive patients referred for investigation of microscopic hematuria were examined by phase-contrast microscopy to determine the probable site of bleeding into the urinary tract. Dysmorphic (i.e. morphologically variable) erythrocytes, suggestive of glomerular bleeding, were present in 86 of 87 patients with significant hematuria who were later shown to have glomerulonephritis. In contrast, all of 30 patients with non-glomerular lesions had isomorphic (i.e. morphologically uniform) erythrocytes in the urine. Ten patients yielded a mixed morphologic pattern suggestive of dual pathology which was confirmed in four patients. Urine erythrocyte counts from 376 healthy individuals showed that 95% yielded less than 8,000 cells/ml and in each case erythrocytes exhibited a dysmorphic pattern suggesting that erythrocytes enter the urine of healthy subjects via the glomerulus. Electron microscopy of urinary erythrocytes from 36 patients with glomerulonephritis demonstrated the range of dysmorphic changes characteristic of glomerular bleeding, presumably resulting from environmental changes suffered by the erythrocytes within renal tubules.

The emergence of gentamicin-resistant klebsiellae in a large general hospital.

Between March and December, 1975, an outbreak of infection with gentamicin-resistant Klebsiella pneumoniae (klebsiella (Gmr)) occurred at the Royal Melbourne Hospital. The klebsiellae were considered to be causing significant infection in 24 of 42 patients harbouring the microorganism. Commonest culture sources were urine and sputum, all but one of the significant urinary infections being associated with indwelling bladder catheters. Antibiotic therapy had been given to 40 patients before the isolation of klebsiella (Gmr), and of these 22 had received gentamicin. Cross infection was a major factor in the outbreak, and control measures (to limit this aspect and curtail the usage of gentamicin) have prevented wider spread within the hospital; at the time of writing the microorganism had been eradicated from all but one of the infected patients. It was shown that klebsiella (Gmr) carry an R-factor capable of transferring gentamicin resistance. Of 22 isolates tested quantitatively, 20 were resistant to 10 mug/ml of gentamicin.
