[Medicine and health care].

Recently the importance of the infrastructure in which the medicine acts is rapidly developing. The health care system is a new science and practice due to the responsibility of the modern state about the health status of the citizens. At the same time the informatical epoch caused the rapid development of the diagnostically and therapeutically tools with the price explosion of the health provision. The slope of the health expenditure increase is steeper, than the slope of the GDP. The gap between the medical possibilities and economical constrains are deepening. This is the main indication of the wanted health care reforms. One of the possible solution is the partial introduction of the private health insurance. The financial constrains may inhibit the liberty of medical activity and to facilitate the dominance of the economic aspects in the health provision.

Use of Norcuron in the course of different surgical interactions.

Norcuron has been used for intubation and short-term relaxation in 21 cases and for maintaining neuromuscular blockade during major operations of 48 patients suffering from severe clinical conditions. The drug assured in all cases relaxation for the required period and longer operations could also be performed, if required, with the repetition of doses. According to the observations Norcuron proved to be a muscle relaxant of medium duration of action which may be combined with drugs commonly used for premedication, as well as with narcotics introducing and maintaining anaesthesia. The special advantage of the drug is that virtually it does not influence the circulatory parameters and its action may be reliably antagonized.

Towards a new health policy in Hungary.

The health situation of the Hungarian population in the postwar period with respect to the socio-economic changes has been evaluated and as a consequence a new health policy programme is outlined. It has been established that between 1945 and 1965 the development of the socio-economic and the public health situation, due to the introduction of a national health insurance system, the decreasing trends of the infectious diseases and malnutrition, and the raising of the cultural standards of the people has resulted in a rapid improvement of the health status at population level. Between 1965 and 1985 the rapid changes of the economic situation were followed by deep intra- and intergeneration family pattern behavioral and life-style changes. As a consequence of these a significant increase in the alcohol, tobacco and fat consumption, and the mortality rate of the 35-59-year-old male population was observed. The introduction of a national health promotion programme is necessary with a special emphasis on the healthy life-style at individual and at multi-sectoral level as well. The structure and function of the health service has to be changed according to the programme. The health promotion programme is based on a nationwide consensus.

Effect on renal function and renin secretion of noradrenaline infused into the renal artery.

Effect of noradrenaline on renal function and renin secretion was studied during infusion into the renal artery of anaesthetized dogs. Experiments were performed with or without alpha or beta receptor blockade. Noradrenaline infusion resulted in a significant elevation of renin secretion associated with marked vasoconstriction. Urine flow rate, the filtered and excreted amounts of sodium were diminished due to the decreased GFR. Alpha receptor blockade suppressed renin secretion in the presence of changes in renal haemodynamics. The simultaneous infusion of noradrenaline enhanced renin release without affecting renal haemodynamics or reducing Na-excretion. Following simultaneous inhibition of alpha and beta receptors renin secretion dropped markedly; there were no further changes in either renin secretion or renal haemodynamics upon the simultaneous administration of noradrenaline. Based on the present findings it is suggested that renin secretion is controlled by both alpha and beta receptors. Beta receptor simulation exerts a direct action, whereas alpha stimulation appears to be mediated in part by indirect mechanisms such as renal haemodynamics.

Effect of adenosine compounds (ATP, cAMP) on renin release in vitro.

Renin release by surviving canine renal cortical slices incubated media with ATP or cAMP at concentrations of 5 X 10(-5)--5 X 10(-3) M has been studied. Both adenosine compounds were significantly increasing renin release. A linear correlation was observed between their dose and the renin activity of the medium. The difference between the effects of ATP and cAMP appeared to be caused by phosphodiesterase, since the difference was eliminated if to the medium containing cAMP 5 X 10(-2) M theophylline, a phosphodiesterase inhibitor was added.

Effect of cyclic adenosine monophosphate on renal function and renin secretion.

The effect of cAMP, infused into the renal artery, on renal function and on the release of renin was studied in dogs displaying a normal serum Na level. Intrarenal infusion of 1 mg per kg per minute of cAMP caused a significant increase in RBF, dilatation of the efferent arterioles, the redistribution of renal blood flow and a significant increase in tubular Na-reabsorption. The antidiuretic action of cAMP is due partly to a hemodynamic and partly to a direct tubular effect. Decreased renin secretion was explained by the Na-load on the macula densa segment, being decreased by the intrarenal shifting of renal blood flow and with an eventual decomposition of cAMP.
