Lack of correlation between sensitivity characteristics of the tests for hepatitis C virus antibodies estimated with serially diluted and natural low-reactive control specimens.

Sensitivity characteristics of seven commercial ELISA test systems for the detection of antibodies to hepatitis C virus were assessed using control panels consisting of: (i) serial dilutions of pooled sera highly reactive for anti-HCV; (ii) serial dilutions of RIBA 3.0 HCV SIA positive control; and (iii) natural (non-diluted, non-spiked) sera low-reactive for anti-HCV. "Dilutional sensitivity" values estimated with these two kinds of highly reactive samples did not coincide and were not found to correlate with the proportion of natural low-reactive specimens detected by each test. Thus, laboratories assessing sensitivity of anti-HCV ELISAs should take into consideration the nature and properties of the control material used. Natural low-reactive control specimens are preferable because they adequately reflect the real serological picture of early stage of HCV infection.

[Genomic instability and AIDS].

The biochemical mechanisms underlying blood lymphoid cell genome destabilization in patients with HIV infection have been analyzed. Lymphocytes from HIV patients are characterized by increasing intensity of free radical oxidation together with activation of the xanthine oxidase D-form conversion into the O-form, enhanced activity of UV-endonuclease, and intensification of prooxidant-induced proteolysis. These changes increasing with the progress of the disease with a maximum at the AIDS stage form a metabolic basis for labilization of the lymph cell genome. The degree of biochemical manifestations of genome instability (levels of chromatin degradation products and intensity of formation of one-filament nicks of DNA) increase in the dynamics of HIV-infection. The data obtained are discussed in terms of the author's conception on the origin of AIDS from retroposons (retrotransposons?). A hypothesis is postulated on accumulation of autonomous genetic information on the basis of genome labilization under the influence of genotoxic factors. Clinico-biochemical data on the appearance of HIV proteins (p17, p24) in the blood of patients (previously negative for all HIV markers) in the presence of transfusions of HIV-negative blood and UV-irradiation of the autoblood are also discussed from this standpoint.

[Xanthine oxidase activity in mononuclear cells of human blood].

Xanthine oxidase activity has been revealed in human blood mononuclear cells. The enzyme is found in these cells only after solubilization. This may become the explanation for contradiction with other previous data claiming absence of xanthine oxidase in human blood mononuclears. The level of enzyme activity is 2.76 +/- 0.029 mu mol/g protein.min. The latter is readily inhibited with allopurinol and folic acid.

[AIDS in children based on autopsy data from the Kalmyk ASSR].

6 cases of HIV infection were revealed by enzyme immunoassay and immunoblotting among 144 children who had died in Elista. Another 5 children might have had HIV-infection but laboratory analyses either were lacking or gave doubtful results. Laboratory results were negative in 133 children. Similar severe changes of the lymphoid system typical for immunodeficiency were found in all children. These changes in HIV infection were characterized by lymphogenic generalized RNA infection. Complications (secondary diseases) in the form of DNA-virus infections (cytomegalia and herpes simplex), pneumocystosis and other viral and bacterial processes were observed in children of all groups. The disease was found mainly in Kalmyks. Blood examination of healthy Kalmyks revealed some differences in the form of significant decrease of the lysosomal cationic proteins content and the peroxydase activity of neutrophil leucocytes as compared to the same indexes obtained in Leningrad. There was also a tendency to the decrease of T-lymphocyte content. The children dying from immunodeficiencies lived mainly along the litoral of an ancient sea.

[Plasmapheresis in combined treatment of severe forms of abscesses and gangrene of the lungs].

With an adjuvant course of plasmapheresis, a combined treatment of severe acute abscesses and gangrene of the lungs brought about a marked short-term clinical response in 48 out of 50 treated patients. The responders had their temperature returned to normal, amount of sputum significantly reduced. They recovered good appetite and gained weight. There was also attenuation of DIC syndrome and endotheliosis, improvement of cellular and humoral immunity. Upon the discharge 35 patients had complete or partial response, 10 patients developed a chronic process. Lethal outcomes reached 10%.

[A histochemical method for detecting xanthine oxidase activity in the liver].

A histochemical method is described for localizing xanthine oxidase--the key enzyme in the purine catabolic pathway. The above method is based on the reduction of p-Nitroblue tetrazolium during hypoxanthine enzymatic oxidation, phenazine methosulfate being an intermediate electron acceptor. The patterns of the reaction product distribution suggest that the Kupffer cells and the sinusoidal endothelium possess the highest xanthine oxidase activity.

[Alanine aminotransferase--a surrogate marker of viral hepatitis].

Examinations of 4457 blood donors (about 80% were men aged 18 to 30) revealed an increased level of serum glutamic-pyruvic transaminase (SGPT) in 82 (1.8%). Specific markers of viral hepatitis C were detected in 15.9%, of hepatitis B in 12.0%, of hepatitis D in 2.0%, of hepatitis E in 8.0%, of hepatitis A in 2.0% of sera with high SGPT levels. Mandatory screening of blood donors revealed specific markers of viral hepatitis in 15.9% of cases, additional testing detected these markers in 10.9% cases more. A conclusion is made that an increase of SGPT activity is an independent surrogate marker of viral hepatitis.
