Progesterone receptor levels and distribution in different phases of the menstrual cycle and in estrogen-primed postmenopausal endometrium.

Progesterone receptor levels were measured in the cytosol and in the 0.4 M KCl nuclear extract of normal cyclic endometria in 49 women, classified into four categories: proliferative, midcycle, secretory, and estrogen-primed (four to 11 days); [3H]R5020 was used as a ligand. The women in each category were further subdivided into test and control groups; the women in the test group received a progesterone injection one to three hours before tissue collection. Both the nuclear progesterone receptor levels and the portion of the progesterone receptor in the nuclear extract were significantly higher in the test than in the control groups; among the test groups in the four categories, however, these changes were significantly smaller in women in the secretory category than in the other three categories. These results indicate that the progesterone receptor activation-translocation dynamics in human endometrium are dependent upon the hormonal status of the woman.

Placenta previa--the identification of low- and high-risk subgroups.

Among a group of 155 women with placenta previa (PP), two subgroups were identified: subgroup A consisted of 56 pregnancies (56 newborns) in women who had an asymptomatic antenatal course and who were delivered following a single episode of uterine bleeding; subgroup B consisted of 99 pregnancies (103 newborns) in women who had multiple antepartum bleeding episodes. The women in subgroup B were older, more parous, had more abortions and caesarean sections compared to those in subgroup A. Although the distribution of women in both subgroups with respect to the types of PP was similar, the prognosis in subgroup A was better with respect to various maternal and perinatal aspects. The recognition of low- and high-risk subgroups among women with PP may have practical implications for management protocols.

Effect of single-dose progesterone administration on estradiol receptor levels in normal human endometrium.

The effects of single-dose progesterone administration on cytosolic and nuclear estradiol receptor levels (RCE2, RNE2) in normal human endometrium were studied in a group of 17 women and the results compared to 18 controls. In both groups, RCE2 and RNE2 levels were highest in the mid-cycle phase and lowest in the secretory phase. Although a wide range of values was found in both groups, the levels of RCE2 in the two groups did not differ significantly. It seems that in human endometrium a single progesterone injection does not significantly change RCE2 and RNE2 levels.

Hormonal and cytopathological changes in vaginal and cervical smears from women undergoing chemotherapy for extragenital malignant diseases.

The effects of various chemotherapeutic drugs on the squamous epithelium of the cervicovaginal tissues were investigated in a test group of 74 and a control group of 44 women with advanced extragenital malignancy. The cytological findings were as follows: (A) cytohormonal changes reflecting estrogenic withdrawal in pre- and postmenopausal women, (B) reversible cytopathological changes reflecting a cytotoxic reaction, and (C) absence of premalignant and malignant changes.

Reevaluation of the pH, ferning and nile blue sulphate staining methods in pregnant women with premature rupture of the fetal membranes.

The accuracy rates of 3 diagnostic methods, namely pH determination, fern test and nile blue sulphate (NBS) staining, in the evaluation of women with prolonged premature rupture of fetal membranes (PRM), were investigated in a prospective study. Two hundred and sixty-two women, divided into control and test groups, were examined. A total of 1725 tests were performed. The results indicate that these tests have high inaccuracy rates which increase progressively when more than one hour has elapsed since the rupture of the membranes, and become unreliable after 24 hours. It is concluded that in cases of prolonged PRM these tests provide no better diagnostic information than that obtained by clinical evaluation.

Placenta previa with focal accretion.

A series of eight women in whom a diagnosis of placenta previa with focal accretion was established, is presented. They were selected from a group of 153 women with placenta previa who delivered during the period 1968-77. The women with placenta previa accreta did not differ from those with placenta previa per se with respect to age, parity, obstetrical history, and antenatal, intrapartum or perinatal outcome. Three women delivered vaginally and five by cesarean section. Postpartum hemorrhage occurred in four of the women, three of whom were delivered vaginally. Of seven women who were managed conservatively, one required a hysterectomy to control bleeding. It is suggested that cases of placenta previa with focal accretion be managed conservatively.

Intravascular leiomyomatosis.

Two cases of intravascular leiomyomatosis are described. In 1 case, a 66-year-old woman was seen with an extension of the tumor to the myometrium, cervix, and left parametrium and with radiologic evidence of tumor extension to the left external and common iliac veins and the inferior vena cava; subsequently, the tumor invaded the left renal, suprarenal and ovarian veins, hepatic veins, and the right atrium, causing severe heart failure and death. The second case is that of a 28-year-old woman with an advanced tumor in the interior vena cava, right atrium, and ventricle who died during cardiac surgery. Review of the literature is presented, with special comment on the management of this disease entity.

Placenta previa. The ultrasonographic placental localization and its influence on the mode of delivery.

In a group of 168 patients who presented with third-trimester bleeding, ultrasonographic placental localization did not provide either positive or negative false results concerning the placental site. In a group of 47 patients for whom an ultrasonographic diagnosis of placenta previa was established, an erroneous typing was done in 4 (8.5%). Of these four, one (2.1%) underwent an elective cesarean section instead of being allowed a vaginal trial of labor.

Recurrent placenta previa.

Among a group of 33 135 women who delivered at Beilinson Medical Center, Israel, during the years 1968 to 1978, recurrent placenta previa occurred in 3.2% of the women, i.e., 6 times higher than the incidence of placenta previa in the general population (0.5%). Analysis of the data revealed that women over 36 yr of age with a history of placenta previa are at risk for recurrent placenta previa. Maternal and perinatal morbidity and mortality were similar in cases of single and recurrent placenta previa.

Diagnosis of rupture of the fetal membranes by glucose and fructose measurements.

A new method is presented for differentiating amniotic fluid from cervical mucus, based on differences in the respective carbohydrate concentrations present in these fluids. Glucose and fructose are present in high concentrations in cervical mucus during pregnancy, average values being 240 and 30.4 mg/100 ml, respectively. Their concentrations in amniotic fluid, however, are low, averaging 39 and 3.3 mg/ml, respectively. Examination of 215 women between 36 and 42 weeks' gestation, using either glucose or fructose values, revealed a high degree of discrimination between amniotic fluid and cervical mucus. The best results, however, were obtained by employing the calculated linear sum 3.32 log glucose plus log fructose for each sample. Inaccuracy rates were low in comparison to those of other diagnostic methods, such as pH values, ferning, and Nile blue sulfate staining. A further advantage of the method is that it remains fairly accurate even when the time interval between rupture of the membranes and the investigation exceeds 24 hours.

Distribution of total progesterone receptor levels in various segments and tissues of the normal human uterus: the effect of short-term estrogen administration.

Total progesterone receptor levels (TRP) were measured in various tissues and segments of normal human uteri in basal conditions (late proliferative phase, control group, n = 6) and after short term estrogen administration (test group, n = 8). Similar trends of distribution were observed in both groups in basal endometrial TRP levels along the longitudinal axis, i.e. the highest levels were found at the height of the body and the lowest at the height of the internal os. Differences were found in the trends of distribution of the TRP levels between the two groups along the transverse axis of the uterus at the height of the fundus. In the control group TRP levels in the functional endometrium were higher than those in the basal endometrium and myometrium. In the test group, however, the trend was reversed. Since the RP is estradiol induced and dependent, these results indicate the possibility of a selective estrogen effect on various segments or tissues in the human uterus.
