Fungal infections in acute leukemia patients during selective decontamination of the digestive tract--a clinical, laboratory and pathological study.

Selective decontamination (SD) is used for prevention of bacterial as well as fungal infection in acute leukemia patients during remission induction therapy. We analyzed 138 treatment periods of 108 patients. The intensified chemotherapy resulted in severe granulocytopenia below 0.1 X 10(9) l for over 25.2 days. In 19 patients there was a suspect of major fungal infection, for which they were given antimycotics (in 15 cases amphotericin B and 5-fluocytosin). Fourteen of them died. Major fungal infections were documented in 5 cases. In 18% of the deceased we found major fungal infection (7 cases of Candida sp., 5 cases of Aspergillus, one case of Candida as well as mucor). There was a correlation between fungal infection, the late stages of the haematological malignancy and the lesions appearing on the oropharyngeal mucosa. However, no correlation appeared to exist in the serological and culture findings between the groups with and without fungal infection. The SD-regime is meant to suppress the Candida cell concentration in the digestive tract but has no influence on Aspergillus in the respiratory tract. The clinical occurrence of major fungal infections published in the E.O.R.T.C.--Gnotobiotic Project Group was 8.2%. According to the literature there is in the eighties a tendency for an increased incidence of aspergillosis. The use of the SD-regime as prophylaxis as well as early antifungal therapy appear to be of great advantage in reducing the frequency of fungal infections in immunocompromised patients.

Problems and results of selective decontamination in leukemia patients.

Infectious complications play a major part during the cytostatic treatment of patients suffering from acute leukemia, as well as in bone marrow transplantations. For infection prevention, the method of selective decontamination (SD) of the digestive tract was used. This procedure eliminates the potentially pathogenic aerobic bacteria and yeasts while leaving the anaerobic intestinal microflora unaffected. The protocol of the Gnotobiotic Project Group was used in treating 33 patients with SD and the results were compared with those from a comparable control group consisting of cases selected from files who had been treated without SD in the past. A statistical difference was obtained in favor of the SD patients regarding frequency and severity of infection, time between admission and the first signs of infection, days febrile, and in the additional necessity of antibiotics (p less than 0.01). SD involves in addition to a scheduled intake of tablets, microbiological surveillance, personal hygiene and an intact hemostasis, and optimal results can be obtained given the help of the patients' compliance. SD has won a secured place in treatment tailored upon the pathophysiological concept of infection prevention, even though there are problems remaining, e.g., gram-positive cocci, yeasts and infection of the oropharynx.

A novel cloning vector for the direct selection of recombinant DNA in E. coli.

A new plasmid cloning vector (pHE3) is described carrying dominant p-fluorophenylalanine-sensitivity (pheS) and chloramphenicol-resistance markers. This vector is used in combination with a p-fluorophenylalanine-resistant Escherichia coli recipient (strain RR28). Foreign DNA can be cloned into pHE3 leading to insertional inactivation of pheS. Transformation of RR28, and plating on minimal medium with chloramphenicol plus p-fluorophenylalanine (pfp) directly selects for colonies containing recombinant DNA.

[Mycological studies of patients with hemoblastoses].

40 patients with haemoblastosis were examined clinically and by cultures for presence of yeast mycosis. Moreover, we analyzed specific antibodies with the help of cell agglutination, the passive haemagglutination and the precipitation using Candida antigen. 29 test persons were found having a yeast mycosis, 3 patients without clinical symptoms in pharynx showed presence of yeasts by culture, 8 test persons were clinically and mycologically negative. In cell agglutination we recorded titres of less than 1:20 to 1:5 160 and in haemagglutination titres of less than 1:20 to 1:5 160 and in haemagglutination such of less than 1:20 to 1:320. 5 patients showed positive precipitation reaction with antigen extracted of membranes of Candida albicans cells. We observed different titre motions with these patients: during illness the titre either rised or decreased clearly, or the patients were in a condition of humoral anergy against the Candida albicans antigen. It is hinted at the necessity to watch over susceptible patients mycologically.

[The use of sterile nursing in hematology].

By means of own experiences gathered in the treatment of 18 patients in Life Island the problems and limits of this therapy method are represented. Microbiological findings are described in detail and commented. Sterile care is an essential part of an effective therapy of haemoblastoses, malignant lymphoma, and some solid tumours, particularly in the aplastic phase.

[Myeloproliferative syndrome].

In the present paper a description of the notion of the myeloproliferative syndrome and a survey of the clinical pictures belonging to this is given. Here the author enters into the most important diseases, such as polycythaemia rubra vera, osteomyelofibrosis, osteomyelosclerosis and megakaryocytic leukaemia, concerning diagnostics and therapy. On the basis of literature a comphrensive discussion of the problem is give, whereby it is referred to the fact that a differential diagnosis as eact as possible is necessary for the performance of an aimed therapy.

[Effect of gold therapy in rheumatoid arthritis on leukopoiesis and granulocyte phagocytosis].

In 12 patients with rheumatoid arthritis the investigations of leukopoiesis and granulocytic phagocytosis were carried out before and after a gold therapy. A marked reduction of granulocytic phagocytosis could be observed here which decreased after the gold therapy, yet remained below the values of a normal collective. Partly contrary informations are discussed. The disturbance of the phagocytosis capacity of granulocytes may possibly be due to a rheumatic factor.

[Patholgenesis of anemias in chronic liver diseases].

In a complex examination of clinically healthy persons, with postprandial glucosuria or affected with a hereditary diabetes mellitus differences were found in the dynamics of the insulin secretion in oral and venous glucose tolerance dependent on the character of the glucose tolerance. With the deterioration of the latter the insulinaemia increase and prolongs in oral tolerance, in venous tolerance, however, it will be lower and lower. The through analysis of the results refers to the pathogenetic role of the peripheral resistance to the insulin as well as the disturbed secretory function of the islet apparatus. The established connections allow of the derivation of pathogenetically proved criteria for the differentiation of normal, subnormal and pathologic glucose tolerance. The confronting of the insulinaemia with the adiposity of the persons examined detects the fundamental difference between persons with normal and disturbed carbohydrate tolerance. The increase of weight contributes to the manifestation of the genetic defect. From the prophylactic point of view apart from the significance of the adequate dietetic regimen the attention is also drawn to the effect of biguanides.

[The question of thrombocyte substitution as a prophylactic measure in splenectomy for idiopathic thrombocytopenic purpura (ITP)].

The problem of intraoperative thrombocyte transfusion in splenectomy of patients with ITP is discussed. The indication for splenectomy cannot be equalized with that for intra-operative platelet substitution. Thrombocyte kinetic examinations with a concurrent determination of the thrombocyte turnover enable those patients to be recognized by their bleeding tendency who are particularly endangered by operations. The intraoperative platelet substitution should be limited to those patients with ITP whose turnover is markedly lowered as a manifestation of reduced thrombocytopoiesis. In these cases it is advisable to shift splenectomy to a later date.
