[Sarcoidosis and Whipple's disease: an association or a relationship?].

Whipple's disease is a rare, systemic disorder characterized by arthritis, serositis, diarrhea, malabsorption, lymphadenopathies, weight loss, fever, cutaneous hyperpigmentation and central and peripheral nervous system disorders. There are approximately 60 published cases with pulmonary involvement, in general pleuritis, hilar adenopathies and interstitial affectation. In some occasions, sarcoidosis has been diagnosed prior to WD, postulating either a causal association between both entities, or a relationship between them, existing in WD and initial phase which mimics sarcoidosis. We present a patient diagnosed of sarcoidosis who during the evolution of the disease developed gastrointestinal symptoms compatible with WD.

[Preliminary results of a program of home oxygen therapy using transtracheal catheter].

We present our experience with oxygen-therapy with transtracheal catheter in the period between January 1990 and December 1991. This procedure was offered to 130 patients who met criteria of home oxygen-therapy being accepted only by twelve. Ten patients maintained oxygen-therapy with transtracheal catheter with excellent results and a notable improve in their quality of life. One patient died twenty-three days after beginning the procedure, due to evolution of main disease finding no role of the catheter in the disease. Another patient showed a vagal reaction after placement of the catheter, which forced to perform resuscitation with connection to mechanical ventilation taking the catheter off because familiar request. Patients who benefit this type of oxygen-therapy the trans-tracheal catheter has showed its usefulness. With few complications and showing no problems in its home handling. Oxygen savings with this technique have been of 55%. Trans-tracheal catheter was much more effective than face mask to correct desaturation after exercise (p less than 0.001).

[Current status of lung transplantation].

Lung transplant has started to develop clinically during the last decade being a valid alternative for a great number of pulmonary diseases. A review of the three types of transplant is presented: single lung, double lung, and heart-lung transplant. We specially point out the indications of those surgical procedures. Furthermore, we expose the attitude towards most of the post-surgery problems: post-surgical follow up, immunosuppression therapy, diagnosis and treatment of graft rejection, main opportunistic infections as well as obliterans bronchiolitis, which is the long term most important complication.

[Tuberculin test: usefulness of an old technique].

Tuberculin test was first used as a diagnostic test of tuberculous infection during the first decades of this century. From the up to date, no other method has proven to be more efficient for this task. In the present work we try to explain what the test consists of, what is its use, how it is interpreted, and which are the indications for the Mantoux intradermal reaction. We emphasize the importance of performing this technique correctly so as to not invalidate the results of this test which is essential both from a individual as from an epidemiological point of view.

[The nutritional status and respiratory function of patients diagnosed with COPD].

Given that patients affected with chronic obstructive pulmonary disease show a progressive weight loss, and the great socioeconomic repercussions of these diseases due to their high incidence in the general population, we started this study with the objective of analyzing the possible connection between the nutritional state and the ventilatory function of a group of patients from our midst, who did not have continuous oxygen therapy at home. We studied a total of 43 patients who had been diagnosed with COPD (excluding those with a BMI < 32), evaluating anthropometric, biochemical and pulmonary function parameters. Among the obtained results, it should be noted that 84% of the patients were normally nourished and only 16% were undernourished. In the lung function analysis, we found a pattern of air flow obstruction. We found a significant correlation between the nutritional state and the type of COPD (p < 0.01), with the emphysematous patients being more undernourished than those suffering from bronchial disease. We also found a significant correlation between the types of COPD and the levels of prealbumin, RBP and albumin, and a positive correlation between the evolution time of the disease and the levels of albumin, PO2 and FEV1. With the obtained results, we do not consider it necessary to establish a nutritional support protocol in ambulatory patients suffering from COPD, whose conditions are similar to those of the patients in our study group.
