Ear involvement in human brucellosis.

A case report of mixed-type hearing loss following brucella infection is presented. Damage to the ear in brucellosis is well documented in the Polish literature whereas, surprisingly, it is not mentioned in the otolaryngologic and microbiologic English literature. Otolaryngologists should be aware of possible ear damage in brucellosis, in order to be helpful in the diagnostic process, with the aim of starting antibiotic treatment as soon as possible, in order to prevent irreversible ear damage.

On the vertical caloric nystagmus.

Eye movements in response to caloric stimuli are recorded traditionally in the horizontal plane only. Twin channel electronystagmographic traces of 124 patients undergoing evaluation of vertigo were reviewed. Thirty traces (24%) were found to include a "true" vertical component to the evoked nystagmus. There was no correlation between the existence and the nature of any vestibular pathology and the presence of the vertical nystagmus. Thus, a vertical component to the caloric nystagmus (the so-called "perverted" caloric nystagmus) is not necessarily a pathological sign, and can be a normal variant, caused by excitation of the vertical semicircular canal - most probably the posterior one. The mechanisms of heat transfer in the temporal bone and the source of this vertical caloric nystagmus are discussed in the context of the published literature.

Square wave jerks--incidence, characteristic, and significance.

Square wave jerks (SWJ) are saccadic eye movements which, when recorded with open eyes are considered to be a pathological sign, caused by fixation instability, and pointing to a central neurological lesion. In reviewing 429 electronystagmographic traces, 85 (20%) were found to include the pattern of SWJ. Among them, in 38 traces, SWJ have been recorded with open eyes during fixation. Many of these cases were found to suffer from definite peripheral vestibular pathology, and some of them did not reveal any sign of neurological pathology in the clinical evaluation and by electronystagmography. Thus, SWJ are non-specific rapid eye movements, which do not necessarily point to neurological pathology. The literature is reviewed and the possible neural mechanism for the production of SWJ is discussed.

Cell attachment, growth characteristics and surface morphology of human upper-respiratory tract epithelium cultured on extracellular matrix.

The advantageous utilization of nasal polypoid tissue as a source for human epithelial cells and the dramatic effect of extracellular matrix on growth and differentiation of these cells were demonstrated. Plating on extracellular matrix induced rapid, firm, cell attachment and flattening of the explants, promoted cell outgrowth as well as long-term survival of epithelial cells in primary cultures. Prominent ciliary activity was observed on the cells of the explant and on the cells in the outgrowth. These cells could be maintained on the extracellular matrix coated dishes for prolonged periods even after removal of the explant, with the cells in the outgrowth covering the region occupied by the removed explant. Prominent ciliary activity, which is considered one of the main criteria for cell viability and differentiation, continued also in the absence of the explant. The present system for cultivation of human upper respiratory tract epithelial cells on extracellular matrix might prove of value in analysing effects of chemotherapeutic agents that influence normal differentiation as well as the effects of viral and chemical carcinogens on these cells in human respiratory disease.

Radiation induced carcinoma of the middle ear.

Radiation induced carcinoma of the middle ear is rare. Only four cases have been reported; an additional case is now described. The treatment approach for radiation induced carcinoma of the middle ear has not yet been established. Radiation therapy for advanced cases is discussed as an alternative to surgical treatment. Previous reported cases are reviewed.

Beneficial effects of propylene and polyethylene glycol and saline in the treatment of perennial rhinitis.

Although propylene-and-polyethylene-glycol and saline have been used in clinical studies as placebos, their possible therapeutic role as wetting agents in the treatment of perennial rhinitis was investigated. Clinical and laboratory response to these agents was measured in eighteen patients during a 2-week baseline period and with 4 weeks of active treatment in a double blind randomized study. After 2 and 4 weeks there was a significant improvement compared to baseline in nasal function (P less than 0.05) and blockage index (P less than 0.01) combining both groups, with no difference between treatments. Patients had less sneezing at 2 and 4 weeks (P less than 0.01), and less stuffiness at 4 weeks (P less than 0.01). There was a significant correlation between improvement in blockage index and nasal biopsies when both were judged independently of the other. This study has demonstrated that wetting agents offer both subjective and objective improvement in the treatment of perennial rhinitis and merit consideration prior to (or along with) other agents with known systemic side effects.

Otitis externa - bacteriological survey.

Otitis externa is one of the most common problems faced by the otolaryngologist, and in some clinics constitutes up to 40% of patients. Although not lethal, it may be a most debilitating disease. The external ear is an epithelium-lined cul-de-sac with many sweat and cerumeniferous glands whose secretions are an excellent medium for bacterial growth. Bacterial surveys done in the USA and in Israel 30 years ago proved Staphylococcus aureus to be the major pathogen. During the years the major pathogen changed, and in recent surveys Pseudomonas aeruginosa was found to be the dominant pathogenic bacterium. The purpose of this article is to present the results of a bacteriological survey done in Israel on patients suffering from otitis externa in the years 1979-1980. A discussion is presented with regard to the meaning of the review. We tried to establish whether a certain factor could be considered to be the cause of otitis externa.
