Which factors influence therapeutic decisions in Parkinson's disease?

Development of dyskinesia is a common phenomenon during the long-term course of Parkinson's disease. During the last few years, some but not all pathogenetic mechanisms causing dyskinesias in PD have become better understood. Severity of Parkinson's disease and levodopa dosing are the main clinical risk factors. Most concepts underline the significance of pulsatile D1-receptor stimulation for the development of dyskinesias. The interactions between D1- and D2-mediated STR-Gpi pathways and colocalized neuropeptides are important but not fully understood. Glutamatergic overactivity might also be a significant pathogenetic factor. According to these pathophysiological concepts, therapeutic strategies focus mainly on continuous postsynaptic DA-receptor stimulation by long-acting DA-agonists or highly selective D2-agonists. Another strategy is the use of NMDA antagonists.

Skin function and skin disorders in Parkinson's disease.

Cutaneous symptoms (seborrhoea and hyperhidrosis) in Parkinson's disease were investigated. In 70 treated patients with Parkinson's disease and 22 control subjects, non-invasive bioengineering methods (sebumetry, corneometry, pH) were carried out on the forehead, sternum and forearm. In addition, concomitant dermatoses and medication were recorded. 18.6% of the patients had seborrhoea on the forehead (>220 microg/cm2), 51.4% showed normal sebum values (100-220 microg/cm2) and 30% a sebostasis (<100 microg/cm2). Males has significantly higher sebum values than females. No relationship between the seborrhoea and the therapy for Morbus Parkinson was found. Patients with hyperhidrosis (n = 36) had significantly lower pH values (p < 0.05) on the forehead than those without hyperhidrosis. 22 patients (31.9%) reported a cold/hot flush and a further 13 (18.8%) had clinical rosacea. Seborrhoea is rare in treated Parkinsonian patients but hyperhidrosis is frequently found. Furthermore, a particular lack of vasostability (flush) appears to be an autonomic dysregulation in the skin related to Morbus Parkinson, which has not been studied to any extent to date.

Workshop III: late motor complications of Parkinson's disease.

The aim in the current treatment of Parkinson's disease is to delay L-Dopa administration and to keep the L-Dopa dosage as low as possible. Such a treatment strategy can delay the onset of late motor complications and reduce their severity. L-Dopa remains the most potent anti-parkinsonian medication, but its use for the initial therapy of Parkinson's disease is limited to elderly patients. In all other cases, dopamine agonists, budipine, amantadine and selegiline are primarily used. With the occurrence of late motor complications continuous dopamine receptor stimulation becomes essential. In this situation, combination therapy has to be individualized, with dopamine agonists playing a key role. In addition, COMT inhibitors, budipine, amantadine and selegiline may be used. Anticholinergic drugs are of very limited importance in the current treatment of Parkinson's disease.

[Psychosocial aspects of the long-term management of Parkinson patients].

1309 hospitalised parkinsonian patients were examined in respect of mental changes and the consequences for their participation in the life of the community. The paper expresses the association of their disability with the degree of rigidity, tremor, akinesis, and the gravity of the overall disorder.

[Electroencephalographic findings in liver function disorders].

Abnormal rhythmisations in the EEG are described in hepatoportal encephalopathy as an expression of a disturbance of more profound cerebral structures. The EEG gives an actual information about the state of the cerebral functions of all stages of a hepato-cerebral decompensation and allows a reliable prognostic estimation during the observation of the course. Practically significant are also bioelectric findings of a disturbance of the cerebral function with clinically symptom-poor course in compensated liver cirrhosis or obstructive icterus.

[Technic for the presentation and evaluation of pathognostically significant erythrocyte changes].

In this paper is described in all details a method of staining and evaluation of blood smears which was originally proposed by Clara Fonti and was revised by the authors. With the help of this method characteristic changes of the erythrocytes can be demonstrated. The changes are depending on growth processes. Without exception they appear in an increased range in the growth of malignant tumours.

[Studies on the cancer-diagnostic evidence of erythrocyte changes during the course of neoplasm treatment].

In the present communication is reported on the results of further observations of the course of the index of the change of erythrocytes of 12 patients in connection with four clinical findings and parameters. The data got in these cases continue the previously presented reports on the results. In 9 out of 12 cases there is a correspondence with the indices of the change of erythrocytes and the clinical data belonging to this; in the remaining three cases the indices of the change of erythrocytes cause to presume a continuation of the malignant processes, though the other findings do not indicate this at present.
