[Lung nodule detection with ultra-low-dose CT in routine follow-up of cancer patients].

PURPOSE: To evaluate the value of an ultra-low-dose CT protocol (ULD-CT) in the dectection of lungnodules in follow-up of cancer patients. MATERIAL AND METHODS: Between April and August 2003, 72 consecutive patients with a history of malignancy were prospectively examined on a single-slice helical CT scanner (Aquilion, Toshiba) with ULD-CT (120 kV, 5 mAs, D-eff 0.12 mSv) and regular-dose CT (R-CT) (120 kV, 150 mAs, D-eff 4.8 mSv). The ULD-CT examinations were independently evaluated by two radiologists. Afterwards, consensus reading of the ULD-CT and R-CT examinations was done. Individual nodules were classified benign (benign calcifications) and non-benign. The studies were classified in 4 subgroups: 1) non-benign nodule evident; 2) non-benign nodule probable; 3) non-benign nodule possible, but not likely; 4) no-non benign nodule. RESULTS: ULD-CT detected 180 nodules in 47 patients, while R-CT showed 168 nodules in 39 patients. The sensitivity of ULD-CT was 87 % in detecting nodules regardless of size and 94 % in detecting nodules > or = 5 mm. Regarding the prediction of a case with non-benign nodules (subgroups 1 to 3), the sensitivity of ULD-CT was 94 % and the specificity 75 %. CONCLUSION: ULD-CT is a good diagnostic tool in detecting lung nodules > or = 5 mm. Radiation dose can be markedly reduced. A negative ULD-CT study excludes non-benign nodules in R-CT with a high probability. In case of a positive ULD-CT, R-CT can be immediately followed.

[First experience with a non-covered CHOO enteral stent in the stomach,duodenum, and jejunum].

AIM: To evaluate the technical performance of the non-covered CHOO enteral stent in the stomach, duodenum and jejunum. METHODS: In 8 patients (two men, 6 women) with malignant tumors of the stomach (n = 4), duodenum (n = 3) and jejunum (n = 1) stent implantation was performed under fluoroscopic control. In two patients endoscopy was needed additionally. The patients were followed for 1 to 168 days. RESULTS: Stent implantation was technically successful in 7 of 8 patients. In 6 patients symptoms improved. In two patients a second stent implantation was necessary 35 and 161 days after primary implantation, respectively. There was no stent migration or tumor ingrowth. CONCLUSION: The implantation of the non-covered CHOO enteral stent into the upper gastrointestinal tract is safe and effective.

Changes with aging in the bone mineral content of the lumbar spine and femoral neck in healthy women in Hungary.

The bone mineral content (BMC) of the lumbar spine and femoral neck was studied in relation to aging in healthy Hungarian women by duel photon-absorptiometric method. The data were compared with North-American and West-European values. The Hungarian values are lower than these for reported from North-America or France, but similar to the BMC values for Sweden. The observed vertebral and femoral bone loss could be well represented by cubic equations. The acceleration of bone loss seems to begin around 40 years. The rate of bone loss was similar to the published values but the decrease in bone loss in the 6th and 7th decades was more suggestive. With regard to the fracture-threshold below which the risk for non-traumatic fractures of vertebrae increases, about 60% of Hungarian women at age 50-59 and about 84% at age 60-69 are considered to be at risk. It is concluded that geographical and habitual differences might be important factors in the development and change of BMC for different populations.

Successful pregnancy after surgical repair of vesico-uterine fistula.

After a survey of the pertinent literature, a brief account of the signs and symptoms, diagnosis and therapy of vesico-uterine fistula is given and two cases are reported. In the first case hysterectomy had to be done because tissue destruction was too extensive for organ saving surgery. In the second case the extent of the lesion was compatible with preservation of the uterus. Healing was smooth, and free from complications in both cases. In Case 2, five years after operation the patient gave birth to a healthy female child of 3750 g weight. It is emphasized that in surgical repair of vesico-uterine fistulae preservation of the uterus should be sought.

Effects of prolonged aminoglutethimid and dehydroepiandrosterone treatment on rat bones.

Authors studied the effect of prolonged sex hormone deficiency on some parameters of bone metabolism in calcitonin sensitive young male rats. 30 rats were given aminoglutethimid (AG) for 36 days. Part of these animals received plus dehydroepiandrosterone (DEA) treatment. Corticoid deficiency was substituted by cortisone. In addition 22 rats were administered only cortisone, 11 animals cortisone and DEA, and 10 rats only DEA for 36 days. On day 37 the different parameters of treated rats were compared to findings in 31 control animals. It has been found that body weight, calcitonin hypocalcaemia, serum alkaline phosphatase, as well as bone ash weight pro unit body weight was significantly lower in AG treated rats as compared to controls and rats receiving DEA in addition to AG. Values of latter group did not differ from controls. It has also been established that neither cortisone substitution nor isolated DEA treatment did involve significant differences in the studied parameters of bone metabolism as compared to controls. The findings support the opinion that sex hormone deficiency, directly or indirectly results osteoporosis. The experimental model seems to be suitable for the study of androgen deficient osteopenia.

Influence of heavy smoking upon the bone mineral content of the radius of the aged and effect of tobacco smoke on the sensitivity to calcitonin of rats.

Using Norland-Cameron Bone Mineral Analyzer BMA 178 Type, it has been found that bone mineral content of the radius is considerably less in male heavy smokers aged 61 to 75 and 76 to 90, then in nonsmokers with similar age. Less difference was found within the group of 61 to 75 year old women. Furthermore, also it has been pointed out that the sensitivity to calcitonin of rats inhaling tobacco smoke is considerably less, than that of control rats. It is possible that smoking promotes the decrease in mineral content of bones i.e. the aggravation of osteoporosis, by the means of calcitonin resistance. The authors recommend tobacco abstinence to patients with osteoporosis and to persons who are particularly endangered by osteoporosis (e.g. in menopause, in post-gastrectomic state ect.).

Bone mineral content of the healthy aged.

Using Norland-Cameron photon-absorption technique, bone mineral content of 436 healthy aged was measured and compared with that of 198 healthy, aged 21-50. Bone mineral content of postmenopausal females decreased continuously with age and bone mineral content of males began to decrease at age over 70. Bone width and menopausal age seemed to be important factors influencing bone mineral content, but previous physical activity seemed to have no effect on the bone mineral content of the aged.
