[Stereology of the interstitial cells of the renal medulla in shock].

The authors carried out quantative stereologic studies on the electronograms of intersticial cells of the medulla of the kidney during shock. There was a considerable lowering of the number and volume of lipid granules in comparison with the control nontreated animals. The results were interpreted as a manifestation of the increased release of prostaglandins from the renomedullar intersticial cells as mediators of some local processes in the kidney, enduring shock.

[Serum lipid group response after intralipid loading].

A method is presented for the determination of serum lipid groups changes in conditions of venous fatty tolerance test as well as for time defining in reaching the initial levels of serum triglycerides. A variant of the quantitative thin-layer chromatography is applied, combined with carbonization and densitometry for lipid groups determination in the serum of 40 patients, subdivided into four groups: with chronic ischemic heart disease, with arterial hypertension, with obesity and a control ggroup. Lipid analysis was carried out by the 5th, 20th and 40th minute after the beginning of the test. After loading with Intralipid, it was established to develop quantitative changes not only in triglycerides but in the rest of the lipid groups as well. At the same time, the time for reaching the initial values of triglycerides, calculated by extrapolation, shows significant differences between the control group and those with ischemic heart disease and arterial hypertension. The investigation method adopted guarantees more thorough information about lipid changes after Intralipid infection, than the method with the determination of plasma opasity in corresponding time intervals. The criterion "time for reaching the initial values" characterized more fully the dynamic changes in triglycerides and could be used in the detection of latent anomalies in lipid metabolism.

Adaptation of the ammoniacal silver reaction to cytochemical demonstration of myelin basic protein.

A modification of Black and Ansley's ammoniacal silver reaction (ASR) for histones is proposed for visualizing myelin basic protien (MBP) in the nervous system. The reaction is performed on histological sections of tissues fixed in neutralized formalin-alcohol and delipidized in the course of the routine paraffin embedding. The deparaffinized sections are again treated with formalin in order to make the "unmasked" by the delipidization basic groups of MBP reactive to ammoniacal silver. After treatment with this reagent MBP of the myelin sheaths of the nerve fibres is impregnated brownish-black. Deparaffinized sections subjected to an extraction of MBP with hydrochloric acid exhibit a negative reaction at the level of the myelin sheaths the same reaction being preserved at the level of the nuclear histones. The reaction is positive in paper spots of nervous tissue extracts obtained with the same acid. These assays indicate the specificity of the modified ASR. The method can be used for studies on the processes of myelination and demylination in normal histogenesis and in pathology of the nervous tissue.

[Micromethod for studying lipid groups in the serum].

A microanalytical variant for the determination of lipids and lipid groups in serum is described, by the application of chromatographis technique and densitometric measurement of the separated zones. The variant scheme is as follows: lipid extraction, distribution of the extract for determination into total lipids, total cholesterine and chromatographic separation of the lipid mixture. Sulphonamic acid, embeded in the layer is used in the carbonization of the zones from the chromatogram. The conditions of carbonization and densitometry are indicated. The accuracy and reproducibility of the analytical determinations according to the scheme presented meet the requirements. The calculation of the results from the densitogram in the adopted mg per 100 ml is through the usage of test established correction factors. Seven lipid indices could be determined in 0.2 ml serum and manipulation time analysis for 10 samples is two hours. A special advantage of the method proposed is the possibility of accurately studying the dynamic of the lipid group in different morbid states or at experimental work. The method could be adopted, without principal changes for analysis of tissue lipids.

[Treatment of obesity with a high-protein, high-fat, "carbohydrate-free" diet].

Sanatorium treatment for 27 days was carried out for 52 patients with obesity of alimentary metabolism type, consisting of moderate loading and high-protein, high-lipid diet with 2430 to 2840 cal daily, with unusually low carbohydrate contents--10 g for the first week and 38 g--in the fourth. The authors established a favourable total and an average daily body weight loss of the patients, irrespectively of the relatively high caloric intake. No sense of hunger was reported from the majority of the patients. The subjective complaints observed are more frequent and more severe, as compared with the other treatment regimens with reducing but balanced dietetic regimens. The majority of the subjective complaints established could be associated with the objectively confirmed compensated metabolic ketoacidosis. The unfavourable changes in the followed-up laboratory indices are: considerable decrease of blood sugar, massive ketonuria, elevation of serum uric acid and deviations in the acid-base parameters. Serum lipid indices fell under the effect of the treatment and body weight reduction. What impresses is the serum cholesterol reduction in spite of the high exogenic import, the diminution of cholesterol-lecithin index and the marked elevation of free fatty acid in serum, manifestation of incresed lipolysis in the lipid depots. (he diet indicated is not balanced and not physiological. It must be applied casually in obesity treatment. Its application is possible only after a strict assessment of each individual patient with obesity with no accompanying diseases, that do not agree with the partial fasting and cannot be included in intensive motor regimen.

[Obesity in correlation with nutrition in representative settlements of the industrial-agrarian complex in Burgas].

The authors investigated the obesity incidence in connection with nutrition factors in 13 representative settlements of Industry-Agrarian Complex Burgas. They established that obesity covers 42,4 per cent of the population above the age of 14 and is 1,5 times more often among females as compared with males. The correlation among youths (age 14-25), mature aged subjects (26-50) and advanced aged ones (over 51) is 1:5,5:4,5. The light I and II stages prevail (88,6 per cent of all the cases) and the majority of the "pure" cases of obesity (73,6 per cent) have no complains.

[Changes in the liver transport of bile pigments in certain forms of jaundice under the influence of novobiocin and luminal].

The novobiocin and luminal test application increases the possibilities to differentiate premicrosome from postmicrosome jaundice and extends the knowledge on hyperbilirubinemia pathobiochemistry. Novobiocin (an antibiotic) inhibits the conjugation and glucuronidizing of bile pigments. Accordingly, the novobiocin test is strongly positive in any premicrosome jaundice form--Gilberts's disease, posthepatitis hyperbilirubinemia (PHHB) and hemolytic jaundice. It is negative in hepatocellular and hepatocanalicular jaundice, and in Dubin-Johnson's syndrome. In premicrosome jaundice with fluctuating bilirubinemia and normal bilirubin level at the time of examination, the novobiocin test may bring about manifestation of a latent or temporary compensated defect which proves to be most helpful in diagnosing. It is assumed that hemolytic jaundice is caused, apart from bile pigments hyperproduction, also by concomitant disorders in the intrahepatal transport of bile pigments. The latter hypothesis is corroborated by the data of the study. The luminal test is likewise markedly positive in premicrosome jaundice, and negative--in chronic hepatocellular and hepatocanalicular jaundice. In some instances with slight defect in the intrahepatal transport of pigments, the luminal test accounts for normalization of bilirubinemia and of the direct to total bilirubin ratio. The test is moderately positive in the syndrome of Dubin-Johnson. The latter fact supports the hypothesis that luminal, alongside the effect on transport mechanisms, exerts influence on bile pigments excretion also.

[Study of liver glucuronidase function in indirect hyperbilirubinemia and liver cirrhosis].

A discussion is made of the basic peculiarties and differences in the clinico-laboratory profile in various forms of pre- and postmicrosome jaundice. The importance of direct to total bilirubin ratio, and of quantitative bilirubin and urobilinogen bodies' determination in the urine is stressed. Bile pigments conjugation by the hepatic cells and free bilirubin conversion to glucuronose may be also assayed by resorting to some additional tests. The test with N-acetyl-paraaminophenol (NAPA) provides for an indirect assessment of the liver's glucuronidase function and has a good informative value. It is optimally characterized by the percentage of free (non-conjugated) NAPA with respect to the total. The latter indicator is normal in chronic hepatitis and in Dubin-Johnson's syndrome, and is at the uppermost normal limit in hepatic cirrhosis and cholestatic jaundice. It is strongly increased (in the average three times with respect to normal values) in Gilbert's disease and posthepatitis hyperbilirubinemia (PHHB). It affords some information on the severity of the defect in transport of bile pigments in the mentioned affections. In hemolytic jaundice a normal percentage of free NAPA is usually found. Glucuronose conversion of bilirubin hardly plays an essential role in the pathogenesis of hyperbilirubinemia i the listed above diseases. This is also confirmed by the NAPA test, performed subsequent to novobiocin loading. The percentare of free NAPA under the conditions just outlined is furthermore increased in Gilbert's disease and PHHB, while in hemolytic jaundice it remains within normal limits. In Gibert's disease and PHHB, a strongly pronounced delay in the excretion of substances is noted. Not infrequently, a similar disorder is also observed in hepatic cirrhosis. It is interpreted as an expression of an overall disturbance in hepatic blood flow and function of the heavily affected hepatic parenchyma.

[Influence of cyclophosphamide on the production of experimentale autoimmune encephalomyelitis (author's transl)].

The aim of the so carried out experiments is to establish whether the created disbalance between T- and B-lymphocyte populations influences the final stages of a cell-mediated reaction in vivo. The chosen model was one of experimental autoimmune encephalomyelitis (EAE). Guinea pigs were given a single challenge (intradermally in the hind foot-pads) of 70 microgram myelin basic protein (MBP)--isolated from bovine spinal cord--in complete Freund's adjuvant (CFA). Another group of animals was treated intraperitoneally with 200 mg/kg cyclophosphamide (CY) 3 days before the MBP-CFA challenge. The control group received only CY. At different times after the administration of MBP, CY or CY-MBP, the popliteal lymph nodes have been examined and the morphological changes together with the number of anti-MBP antibody synthesizing cells (ASC) were recorded. In the guinea-pigs treated with the CY 3 days before the antigen challenge the disease develops earlier and shows a degree of severity with a threefold greater mean score. The number of ASC is increasing up till the 10th day and slightly decreases toward the 20th day when there are clinical and histological signs of EAE. If MBP is administered after a 3-day action of CY, the number of these cells till the 7th day after the antigen challenge is strongly reduced, then surpasses the normal value about the 10th day and comes back to it towards the 20th day. The results so obtained confirm the concept of the T-lymphocyte mediation of EAE and give us grounds for discussing the CY action on the lymphocyte subpopulation.
