Drug-induced pemphigoid. A spectrum of diseases.

Drug-induced ocular pemphigoid has been reported to occur after long-term use of topical glaucoma medications. We have reviewed the literature on drug-induced ocular pemphigoid and have added our experience with five patients with glaucoma who developed ocular cicatricial (benign mucosal) pemphigoid after treatment with various topical glaucoma medications. We also describe a case of timolol-induced ocular pemphigoid. The presentation and progression of drug-induced ocular cicatricial pemphigoid can be variable. We propose that drug-induced ocular pemphigoid represents a spectrum of diseases ranging from a self-limiting, toxic form to a progressive, immunologic form.

Central anterior chamber depth after laser iridectomy.

We found the true anterior chamber depth to be unaffected by successful laser iridectomy in ten eyes treated by this modality for chronic angle-closure glaucoma. The apparent general deepening seemed to be the result of peripheral chamber deepening combined with the common use of strong cycloplegic agents, reversing the shallowing effect of preoperative miotic medications.

Anterior chamber depth measurement using the split-lamp microscope.

I devised a new and simple method to measure the anterior chamber depth that requires only a Haag-Streit 900 slit lamp. Viewing the anterior chamber at 45 degrees and matching the height of the corneal slit beam to the depth of the anterior chamber enables the observer to calculate the anterior chamber depth by multiplying the measured slit height by 1.4 or, more accurately, by use of the table provided. The accuracy over the range of clinically encountered depths of the anterior changer is +/- 0.2 mm relative to standard pachometry.
