A prospective audit of cost of sedation, analgesia and neuromuscular blockade in a large British ICU.

Bottom-up costs of sedative, analgesic and neuromuscular blocking drugs used in the intensive care unit have not been reported. We performed a prospective audit of the cost of these drugs using a bottom-up approach by prospectively recording the daily amount of drugs administered to patients over a 3-month period. Of 172 admissions, complete data were collected for 155 (92%). Propofol and alfentanil were the drugs most commonly used, being administered to 136 (88%) and 106 (68%) patients, respectively. The total cost was 14,070 pounds sterling, which was 81% of the pharmacy figure (based on central purchasing). Ninety-four per cent of the cost was for drugs administered to the 50% of patients who stayed in the intensive care unit longer than 48 h. The median (interquartile range [range]) cost per day was 9.30 pounds sterling (3.60-20.10 [0-61.20]). This represents less than 1% of reported total daily cost of intensive care per patient.

Electric cautery lowers the contamination threshold for infection of laparotomies.

BACKGROUND: Interplay between wound resistance factors and bacterial innoculum determines the risk of surgical infection. Since cautery causes more damage than the scalpel, our hypothesis is that lower numbers of bacteria are required to infect wounds made by electric cautery than to infect wounds made with a scalpel. METHODS: Abdominal fascia was incised in 375 rats by cold knife, cutting current, or coagulation current. Wounds were innoculated with increasing numbers of bacteria and histologically scored at 7 days for necrosis, inflammation, and abscess. RESULTS: Coagulation current causes more inflammation, necrosis, and abscesses than the scalpel at all bacterial levels. Electric cutting current is intermediate, causing more damage than the scalpel only after contamination reached 10(5). Above this threshold most wounds were infected in all groups. CONCLUSIONS: Electric coagulation current should be used only when the need for meticulous hemostasis outweighs the considerably increased risk of infection. Electric cutting current is less destructive but also less hemostatic; indications for its use are difficult to identify.

Use of polypentapeptides of elastin to prevent postoperative adhesions: efficacy in a contaminated peritoneal model.

We investigated the use of a sheet of polypentapeptide of elastin as a physical barrier to adhesion formation in a contaminated peritoneal wound model. A total of 88 rats were studied with random assignment of animals to three study groups; control (29), polypentapeptide steam sterilized (30), and polypentapeptide gas sterilized (29). Animals were anesthetized and a laparotomy was conducted to reveal the cranial portion of the ileum. The abdominal wall muscle peritoneum was excoriated until hemorrhage was noted. In sham animals, there was no physical barrier placed between bowel loop and the abdominal wall. In two of the study groups, the polypentapeptide sheet was placed directly over the excoriated area. The intestinal loop was then loosely secured to excoriated area with 2-O nylon (stay suture) which was tied subcutaneously in all groups. Four puncture wounds were made with a 20-gauge hypodermic needle in the bowel that was apposed to the excoriated peritoneal musculature which allowed leakage of intestinal contents and contamination. On Day 7 postsurgery, the animals were anesthetized and the stay suture was removed. On Day 14, all animals were sacrificed and adhesions were graded. The incidence of significant adhesions was 28% for the barrier group versus 90% for control animals (P < 0.05). The results of this study indicate that the polypentapeptide of elastin sheet is an effective physical barrier in this surgically induced contaminated wound model.

Porcine peritoneal sepsis: modeling for clinical relevance.

The characteristics of two types of intraperitoneal (i.p.) soilage sepsis models, autologous fecal inoculum (FEC) and a pure culture of Escherichia coli (EC), were studied in 26 male Yucatan minipigs (20-30 kg). Early (1-4 h) and late (24-72 h) changes were different between the two groups. The EC group was characterized early by hypotension, low cardiac output, and increased systemic and pulmonary vascular resistances, along with leukopenia, hypoglycemia, lactacidemia, and elevated blood urea nitrogen. Of the pigs in the EC group that survived the early effects, there were few significant differences in physiological parameters, compared to control pigs, that would indicate ongoing pathological processes. In contrast, the FEC group pigs demonstrated early hypotension, but with increased cardiac output and reduced systemic vascular resistance. Other parameter changes were similar to those seen in the EC pigs, but to a lesser degree, with the exception of elevations in serum lactate dehydrogenase. Also in contrast to the EC group, most of the changes in the FEC group persisted in later days, and FEC pigs demonstrated leukocytosis. There were also greater elevations in circulating lipopolysaccharide (LPS) concentrations in the EC group that returned later to baseline levels. In the FEC group, there were persistently elevated LPS concentrations over 72 h. These observations suggest that pigs challenged with intraperitoneal E. coli demonstrated an initial acute peritonitis and damaging physiologic effects of high levels of circulating LPS. Survivors in this group improved and were physiologically stable after 24 h. Pigs that received i.p. autologous feces developed an early acute peritonitis phase with lower levels of circulating LPS, and later developed pronounced peritoneal reaction as demonstrated by multiple abdominal abscesses, pyogenic granuloma formation, and adhesions with physiological evidence of developing sepsis over 72 h. These observations indicate that i.p. EC models evoke a systemic response not unlike intravenous administration of LPS or EC, however, the FEC model produced a systemic response akin to a slower developing septic process.

Sonographic evaluation of pregnancies with maternal varicella infection.

We reviewed 37 cases of mothers who had varicella-zoster virus (VZV) infection during early pregnancy, who subsequently underwent fetal sonography in the 13th to 37th week of gestation. Of the five infants later shown to have congenital varicella infection at postnatal examination (1 case) or autopsy (4 cases), all showed ultrasonographic abnormalities suggesting infection, and all died by 4 months of age. None of the 32 unaffected infants demonstrated in utero ultrasonographic abnormalities. No statements about relative risk of being affected by the virus can be made from this data.

The surgical significance of Persian Gulf sand.

Foreign bodies, in particular sand and soil particles, can cause considerable reaction in wounds and remain in tissues indefinitely. The introduction of foreign material as a contaminant in wounds and intracavitary injuries to military personnel deployed in the Persian Gulf region can be a complicating factor in treatment. Samples of desert sand from the military operational areas of the Persian Gulf were analyzed and the acute local and systemic responses of intraperitoneal contamination determined in an experimental rat model.

Patterns of femoral head migration in osteoarthritis of the hip: a reappraisal with CT and pathologic correlation.

Although medial, superior, and axial patterns of migration of the femoral head in osteoarthritis of the hip have been well described, it is not clear what anatomic and biomechanical factors determine the direction of migration. The authors studied 22 patients with bilateral (11 patients) or unilateral (11 patients) osteoarthritis by means of conventional radiography and computed tomography (CT) to define any relationships between migration in the coronal plane and that in the transverse plane and to determine whether femoral anteversion, acetabular anteversion, femoral neck-shaft angle, or acetabular inclination were related to particular migration patterns. Anterior migration was evident in 14 of the 19 hips with a superior migration pattern, whereas posterior migration was present in five of the seven hips with a medial migration pattern. In the remainder of cases, no migration in the transverse plane was present. Femoral anteversion as determined with CT, femoral neck-shaft angle, angle of acetabular inclination, and acetabular anteversion angle in this relatively small sample were all found to be within normal limits and appeared to have no influence on the occurrence of a specific pattern of femoral head migration.

Nutritional rickets in San Diego.

Despite the ability of infants to synthesize vitamin D through exposure to sunlight, nutritional rickets occasionally develops in infants even in areas with perennially sunny climates. In San Diego, a 1-year-old breast-fed infant presented with classic signs of nutritional rickets. Unsupplemented breast milk, limited exposure to sunlight, and darkly pigmented skin were predisposing factors. Because of this occurrence, we conducted a survey of vitamin supplementation practices among pediatricians in San Diego. Twenty-nine percent of 160 respondents do not prescribe vitamin D supplements for breast-fed infants. Those in practice less than ten years were even less likely to prescribe a vitamin supplement compared with their older colleagues. The patient report, coupled with a literature review, suggest the need for vitamin D supplementation for all nursing infants.

Radiographic evaluation of joints resurfaced with osteochondral shell allografts.

The radiographic features of 41 cadaveric osteochondral shell (low ratio of subchondral bone to articular cartilage) allografts placed in 24 patients for articular resurfacing as an alternative to arthroplasty are presented. Underlying causes of joint disease included ischemic necrosis (20 grafts), osteochondritis dissecans (nine), chondromalacia patellae (10), and posttraumatic osteochondral fracture with degenerative disease (two). Congruity with the adjacent native articular surface and the opposite side of the joint was evident on immediate postoperative radiographs in all patients, and proved to be critical to the ultimate success of the procedure. On follow-up radiographs over a period of 2-28 months, successful incorporation of the allograft was characterized by progressive loss of the relative increased density of the graft, in association with diminished lucency related to new bone formation at the graft-native bone interface, as well as maintained alignment. Graft failure was associated with positional changes including collapse, persistent increased density, and poorly defined fragmentation that occasionally simulated infection radiographically and resulted in intraarticular bodies. Resurfacing of diseased articulations with osteochondral shell allografts constitutes a potentially desirable alternative to total joint arthroplasty, particularly among younger patients. Consequently, an awareness of the expected radiographic alterations associated with graft incorporation and failure is important.

Identification of the phosphocarrier protein enzyme IIIgut: essential component of the glucitol phosphotransferase system in Salmonella typhimurium.

The phosphoenolpyruvate-dependent phosphorylation of glucitol has been shown to require four distinct proteins in Salmonella typhimurium: two general energy-coupling proteins, enzyme I and HPr, and two glucitol-specific proteins, enzyme IIgut and enzyme IIIgut. The enzyme IIgut was solubilized from the membrane and purified about 100-fold, free of the other protein constituents of the phosphotransferase system. Enzyme IIIgut was found in both the soluble and the membrane fractions. The soluble enzyme IIIgut was purified to near homogeneity by gel filtration, hydroxylapatite chromatography, and hydrophobic chromatography on butylagarose. It was sensitive to parital inactivation by trypsin and N-ethylmaleimide, but was stable at 80 degrees C. The protein had an approximate molecular weight of 15,000. It was phosphorylated in the presence of phosphoenolpyruvate, enzyme I, and HPr, and this phosphoprotein was dephosphorylated in the presence of enzyme IIgut and glucitol. Antibodies were raised against enzyme IIIgut. Enzyme IIIglc and enzyme IIIgut exhibited no enzymatic or immunological cross-reactivity. Enzyme IIgut, enzyme IIIgut, and glucitol phosphate dehydrogenase activities were specifically induced by growth in the presence of glucitol. These results serve to characterize the glucitol-specific proteins of the phosphotransferase system in S. typhimurium.

Hemiazygos vein aneurysm: CT documentation.

A case of an idiopathic aneurysm of the hemiazygos vein diagnosed by CT is presented. Then a patient with hemiazygos vein dilatation due to a known etiology is shown for comparison.

MR imaging of vasculitis of the testis.

Vasculitis of the testis, although common pathologically, has been difficult to diagnose clinically and to detect noninvasively. This report describes the added benefit of MR imaging in directing clinical management in a patient where the first manifestation of vasculitis was scrotal pain and swelling. This subject was diagnosed and treated for epididymitis. Although he had a normal appearing testis on sonography, magnetic resonance showed testicular changes more characteristic for focal infarcts which led to testicular biopsy and to altered diagnosis and treatment.

Intrapancreatic accessory spleen mimicking pancreatic mass on CT.

Accessory spleens occur in approximately 10% of the population and are usually located in the region of the splenic hilum. In 16% of patients the accessory spleen will be located within the pancreatic tail. The diagnosis of an intrapancreatic accessory spleen can be suggested when CT, with and without intravenous contrast agent, demonstrates that the mass enhances in a manner identical to the spleen.
