[Chlamydia infection and CIN].

Under the hypothesis that a chronic infection of the cervical epithelium leads to a reduction in the immunological response, a study was carried out to compare the possible relationship between chlamydial infection and CIN. 187 patients were referred to hospital for histological confirmation because of a cytological smear result (PAP III and IV a). Smears were examined for chlamydia antigens using immunofluorescence. The presence of antichlamydial IgA and IgG was performed in serum using an indirect immunohistochemical method. These tests confirmed CIN in 163 patients. The infection rate with chlamydia was 52.9%, 15.9% of CIN patients showing an active chlamydial infection. Inclusion of topographical findings, together with differential evaluation of colposcopic results, with reference to the transformation zone, allow for a separation between inflammation and dysplasia. The confirmation of a positive chlamydial infection and colposcopic criteria may help to reduce the high percentage of false-positive findings in the problem-group of PAP III patients. It appears that reservations about smear controls after doxycyclin therapy--including treatment of sexual partners--is justified.

[Differential diagnosis of papillomas in the area of the genitals].

Infection of the genital organs by human papillomavirus (HPV) is the most commonly diagnosed sexually transmitted disease. Papillomaviruses lead to a contact infection of the epithelium and present with different clinical and histological signs. With the exception of the best known clinical manifestation Condylomata acuminata, subclinical infections can be only diagnosed from a stepwise examination. Physiological results from both men and women may be falsely explained as being caused by viruses, and can lead to an incorrect therapy. Genital papilla (Papillae coronae vulvae et glandis) present as atavistic cutaneous papilla a normal situation but with functional significance. Differences in clinical symptoms as well as the possibilities of further differentiation can be obtained by means of pictures taken during colposcopy.

[Combined cholestatic jaundice due to cholelithiasis and primary biliary cirrhosis].

An 82-year-old man was admitted because of painless jaundice and pruritus. Multiple small gallstones were seen on ultrasonography. Hepatic needle biopsy revealed bile stasis, intrahepatic bile ducts proliferation and inflammatory infiltration of the portal spaces. As the jaundice deepened, cholecystectomy was performed and a gallbladder full of small calculi was removed. Wedge biopsy of the liver led to the diagnosis of primary biliary cirrhosis (PBC). Colchicine was started and since then his condition has improved and become stable. PBC in an old man is very uncommon. The combination with gallbladder stones imposed special diagnostic and therapeutic problems. Although gallbladder stones are more common in PBC than in the general population, the combination of the 2 clinical entities is not sufficiently emphasized in the literature.

Local injection treatment of tennis elbow--hydrocortisone, triamcinolone and lignocaine compared.

Corticosteroid injections are the mainstay of treating tennis elbow even though their effectiveness has not been well established by controlled studies. A survey of consultant rheumatologists confirmed a widespread preference for this treatment but they varied in their choice of steroid dose and preparation. We examined the value of some practices by comparing local injections of 2 ml 1% lignocaine with either 10 mg triamcinolone or 25 mg hydrocortisone made up to 2 ml with 1% lignocaine (Study 1). The investigation was conducted double blind. Within the first 8 weeks, pain relief was greater for triamcinolone than hydrocortisone although the differences were not statistically significant. The response to both steroid preparations was significantly better than for lignocaine up to this point but at 24 weeks, the degrees of improvement were similar for all three groups and many patients still had pain. Relapse was common. In a separate but similarly designed study, triamcinolone 10 mg was compared with 20 mg of the same agent. Improvements of pain were similar and followed the same time scale. Post-injection worsening of pain occurred in approximately half of all steroid treated patients in both studies and this was sometimes severe and persistent. It was less frequent amongst those given lignocaine alone. Skin atrophy was reported in all groups but was more frequent amongst those given triamcinolone in Study 1. In conclusion, more rapid relief of symptoms was achieved with 10 mg triamcinolone than with 25 mg hydrocortisone or lignocaine alone and there was less needed to repeat injections. Results obtained with 20 mg triamcinolone were similar to those of the smaller dose.(ABSTRACT TRUNCATED AT 250 WORDS)

Aquagenic pruritus: prevalence and clinical characteristics.

We conducted a study of the prevalence and clinical characteristics of aquagenic pruritus (AP) in 996 employees at a general hospital. Forty-five of them claimed that they had AP, thus giving a reported prevalence of 4.5%; 12 were excluded for various reasons and a detailed workup was carried out in 33 people. In more than 75% of the cases AP had started before age 30 and lasted for over 10 years in 42%. The onset of AP occurred within 5 min of exposure in 76% of the cases and usually lasted between 10 and 30 min. In most cases no association could be demonstrated between AP and each of the following factors: place, season, time of day, water temperature, type of soap, towelling or mental stress. AP in at least one other family member was found in 33% of the cases.

[Primary left lower lobe tuberculosis and diabetes mellitus].

A 31-year-old man presented with fever, cough and hemoptysis. Primary pulmonary tuberculosis of the left lower lobe was diagnosed. Concurrently he developed severe insulin-dependent diabetes. This case stresses the special connection between primary atypical pulmonary tuberculosis affecting the lower lobes and diabetes mellitus.

Finger flexion function in rheumatoid arthritis: the reliability of eight simple tests.

The inter- and intra-observer errors of eight tests of finger flexion function were estimated from the results obtained by three observers assessing 10 patients with rheumatoid hand involvement. Measurements of finger flexion and muscle power involved both conventional and novel techniques using simple and easily constructed apparatus. For each test, measurements were in agreement between observers and were reproducible on three occasions. These tests may now be used with confidence by other investigators.

[Lidswelling in a case of Melkersson-Rosenthal-syndrome (author's transl)].

Report on a case with extreme swelling of the upper- and lowerlids, which recurred repeatedly for thirteen years, with unsettled etiology and which did not respond to every conservative therapy. Surgical excision and biopsy showed histological changes similar Melkersson-Rosenthal-syndrome. But it must have been an abortive form, as other symptoms, were missing.
