Several reports have appeared in the literature linking abnormalities in the Retinal Pigment Epithelium (RPE) related with changes in the metabolism of copper and zinc. It has not been yet elucidated if the changes in the metabolism of these metals are secondary to a progressive disease that affects the RPE, or the changes found in this retinal layer are a manifestation of a basic metabolic disease. In the present study a group of 22 diabetic patients with different degree of retinopathy was evaluated. Eleven of these patients underwent laser treatment, and blood and urine copper and zinc were assessed prior to the treatment and at intervals of one day, one week and one month post treatment. All these patients were compared with a control group. We found a rise in the mean concentration of copper and zinc in serum, and zinc in the urine, in those patients who underwent more than 600 laser applications. The significance of these findings and their relationship to the RPE is discussed.
To determine the risk associated with previous herpes simplex type-2 (HSV-2) infection and possibly other virus infections, a prospective study of cervical neoplasia in more than 10,000 women was performed in the 1975-1983 period. The subjects were selected at random from an alphabetical listing of eligible women living in one district of Prague. At enrollment colposcopy and cervical cytology were performed, a blood sample was taken and data regarding education, socio-economic status, personal habits and sexual and reproduction-associated attributes were obtained from each woman. A total of 10,683 women were enrolled; a complete set of data was obtained in 10,389 women. Women with normal or non-significant findings were invited for further colposcopical and cytological investigations after 2 years and 4 years, the other women were followed at 3- to 6-monthly intervals. In women with highly significant findings, histological investigation was performed. The total of 150 cases of moderate to severe dysplasia (i.e. cervical intraepithelial neoplasia, grade II, CIN II), 83 cases of carcinoma in situ (CIN III) and 21 cases of invasive carcinoma (INCA) were detected. More than 60% of the patients were ill at enrollment, the other cases developed in subjects with originally slightly suspicious (27 CIN II, 17 CIN III, 3 INCA) or negative findings (30 CIN II, 12 CIN III, 3 INCA). Analysis of the data indicated significantly positive correlation of one or more of these clinical conditions with a number of sexual and reproduction-related attributes of which early age at first intercourse was most consistent. Among the other attributes, the smoking habit was associated with the highest risk of developing the disease. A negative correlation of cervical neoplasia with several attributes was demonstrated; of these diathermoelectrocoagulation of the ectopic epithelium and transformation zone of cervix was the most important single protective factor. On the basis of these findings, control subjects were selected for serological studies.
Sera obtained at enrollment in the study from patients suffering from moderate to sever dysplasia (cervical intraepithelial neoplasia grade II), carcinoma in situ (cervical intraepithelial neoplasia grade III) and invasive carcinoma, or developing any of these conditions in the course of the prospective study, and from control subjects, were examined for herpes simplex type-2 (HSV-2) antibody presence. The controls were matched with the patients by age, age at first intercourse, number of sexual partners, smoking habits and history of diathermoelectrocoagulation of the ectopic epithelium and transformation zone of cervix. Only those subjects were selected as controls who remained free of pathological colposcopical and cytological findings throughout the observation period, i.e. for at least 4 years after their serum sample was obtained. The microneutralization test (MNT) and type-2-specific solid-phase radioimmunoassay (SPRIA) were used as serological tests. No difference in the prevalence of HSV-2 antibody between the patients and controls was revealed by either test. Various combinations of the results from the two tests also failed to show any difference between patients and controls. Moreover, no significant differences were observed in the prevalence of HSV-2 antibody between patients suffering from the various pathological conditions and those diagnosed at enrollment and later in the course of the study. These results do not provide any support for the hypothesis of the involvement of HSV-2 in cervical neoplasia.

The acute intravenous effects of therapeutic doses of procainamide, lignocaine, metoprolol, digoxin and atropine on the monophasic action potentials (MAP) and effective refractory periods of the right ventricle (VERP) were studied in 48 healthy volunteers. Procainamide prolonged the VERP in the apex region. Lignocaine shortened the MAP duration at 90% repolarisation. Metoprolol did not affect any of the measured variables in spite of a significant decrease in heart rate. Digoxin produced a significant increase in the VERP at the outflow tract, but not in the apex region and the MAP variables did not change. Following atropine, the VERP at both recording sites decreased but the MAP signal was unaffected. In summary, the effects of procainamide, lignocaine, metoprolol and digoxin were in good agreement with previous studies in normal ventricular muscle cells in vitro. In addition, the findings following atropine, digoxin and procainamide are indicative of a parasympathetic innervation of the endocardial surface of the right ventricle.
Herpes simplex virus type 1 (HSV-1) and type 2 (HSV-2) antibodies in humans and animals were determined by a micro-solid-phase radioimmunoassay with Helix pomatia lectin/Sepharose 4B-purified antigens from HSV-1- and HSV-2-infected cells. A low but constant heterotypic reactivity was found with HSV-1 antigen, whereas HSV-2 antigen was type specific, as evidenced by (1) its reactivity with sera from HSV-2- but not with those from HSV-1-immunized animals, (2) its reactivity with monoclonal antibody to glycoprotein C but not with that to other HSV-2 glycoproteins, and (3) the negative results obtained in serum samples from 57 children, of whom 49 possessed HSV-1 neutralizing antibody. Antibody to HSV-2 (anti-HSV-2) was detected in serum samples from 14 of 16 subjects treated for genital herpetic lesions. The prevalence of anti-HSV-2 in women did not differ in the 25-35- and 36-45-year-old age groups but did correlate with the number of sex partners, with only 10% of women reporting one sex partner and 33% of those reporting more than 10 sex partners having anti-HSV-2.
The right ventricular repolarization phase was studied in 48 healthy men between 20 and 40 years of age. The assessment of the repolarization time included the measurement of ventricular effective refractory periods and monophasic action potentials during constant ventricular stimulation. Computer-based analysis of the monophasic action potential allowed the duration at 90% and 50% repolarization, the amplitude, the maximal upstroke velocity and the total rise time of the depolarization to be determined. These results may serve as reference values in further studies on ventricular repolarization using the same monophasic action potential recording technique.
The presence of Epstein-Barr virus genetic material was demonstrated in thin sections of biopsy specimens in 6 of 7 carcinomas of human palatine tonsil by the in situ hybridization test. The biopsy specimens from 4 tonsillar carcinomas exhibited a strong positive reaction; two biopsy specimens were weakly positive. Two tumor-free biopsy specimens were negative. None of the thin sections reacted with 3H-labeled herpes simplex virus DNA probe.
The bioavailability of Megacillin-oral-Trockensaft (active substance: potassium salt of phenoxymethylpenicillin, penicillin V potassium) was compared with that of another commercially available drug containing the same active substance. In a cross-over study, 12 healthy volunteers were administered by oral route 10 ml of each preparation (equivalent to 600 000 U = 392.2 mg potassium salt of phenoxymethylpenicillin) under standardized experimental procedure. Relative bioavailability was assessed by determination of phenoxymethylpenicillin concentrations in the plasma, employing both microbiological assay as well as high-performance liquid chromatography, by computation of the areas under the plasma concentration curves, and by calculation of the time periods necessary for the attainment of maximum plasma concentrations. In order to assess differences between the two forms in duration of efficacy, calculation of time intervals were based on plasma concentrations which were above 0.5; 1.0 or 1.5 micrograms/ml, respectively. Results of this comparative study indicate that Megacillin-oral-Trockensaft is superior to the other commercial preparation. The considerably better bioavailability of Megacillin-oral-Trockensaft is attributed to a substantially higher absorption rate and to a 2.4 times greater extent of absorption. Due to the distinct advantage in the bioavailability of Megacillin-oral-Trockensaft peak plasma concentrations of phenoxymethylpenicillin 5-6 fold higher and are reached faster when compared with those following intake of the other form tested. In practice, the superior bioavailability of Megacillin-oral-Trockensaft guarantees quicker initiation of therapeutic activity and greater safety (higher plasma concentrations, prolonged effect).
Thin sections of biopsies from five patients with supraglottic laryngeal carcinoma who possessed antibody to early antigen of EB virus were examined for the presence of EB virus nuclear antigen (EBNA) and EB virus DNA by cytological hybridization in situ. Both EBNA and EB virus DNA were found in the tumour cells of three of the patients. The present findings suggest an association between EB virus and supraglottic carcinoma of the larynx.
I have tried to distinguish between intimacy and nurturance in the adult psychoanalytic relationship. Intimate relatedness refers to a coparticipation in the relationship, with both parties presenting their experience and opening themselves to the other. A nurturant analytic relationship is characterized by a parent-child configuration in the analysis. The patient is often seen as not formed or not possessing a self and the analyst's task is to provide the sort of nurture that will help the patient develop a self. This can overlook whatever self the patient has developed and work counter to the formation of a solidified identity and a sense of autonomy. Crucial to the central thesis is the view that past deficiencies cannot be retrieved. The effort to make up for early deficiencies by providing a new parental experience is sometimes a defense against the hopelessness that patients can come to grips with their lives as they are. An intimate analytic inquiry can help patients see and accept who they are and where they have come from. Judicious use of the analyst's self can aid patients in relating intimately, recognizing in the process that they have a self that they, with the analyst's help, can integrate and put to more fruitful use.
In ten cases of nontraumatic retinal tears with initial vitreous hemorrhages so dense that all retinal detail was obscured, medical management included hospitalization, sedation, and binocular patching. It took an average of 4.3 days for the vitreous to clear so that the tears could be examined. All the tears were on or anterior to the equator, and all were in the superior quadrants. Surgery was performed promptly once the tears were visible even though much of the retina was still covered with hemorrhage. After total clearing of the vitreous, no additional retinal tears were found in any of the ten cases. Postoperative complications included hemorrhages in three cases, macular pucker in one case, and a paramacular star in one case.
Polycythemia is consistently produced in animals undergoing experimental hydronephrosis. This hematologic response is thought to be mediated by a microcirculatory insult and a more accelerated elaboration of erythropoietin by the hydronephrotic kidney. While an association between hydronephrosis and polycythemia has been reported in humans, it is a relatively rare event. Further studies are necessary to explain why this should be so, as well as to confirm that the polycythemia in humans is secondary to an increased production of erythropoietin.
The relative contents of Epstein-Barr virus (EBV)-associated nuclear antigen (EBNA) and average numbers of EBV genome equivalents per cell were determined (i) in Raji cells superinfected with P3HR-1 virus, (ii) in Raji cells induced by 12-O-tetradecanoyl-phorbol-13-acetate (TPA) and sodium n-butyrate and (iii) in P3HR-1 cells induced by TPA and sodium n-butyrate. This treatment was followed by increases in the percentage of early antigen (in both Raji and P3HR-1 cells) and virus capsid antigen-positive cells (in P3HR-1 cells) and increases of approximately 20-fold in the average number of EBV DNA equivalents in superinfected Raji cells and in TPA- and sodium n-butyrate-induced P3HR-1 cells. However, the content of EBNA in these cells dropped. This was revealed by a decrease in both the complement-fixing antigen content and in the proportion of EBNA-positive cells as determined by anti-complement immunofluorescence. Thus, the positive correlation found previously between the content of EBNA and the number of EBV genome equivalents per cell in proliferating lymphoblastoid cultures does not seem to apply to the situation in either superinfected Raji cells or in P3HR-1 cells induced by TPA and sodium n-butyrate.
A method of opening an opacified, retrolental, intrapupillary membrane in a pseudophakic eye, using the argon laser, is presented. The procedure was safe, and the membrane remained retracted one year after treatment.
