[Clinical regularities in the relapsing and continuing course of phobic disorders].

To evaluate clinical regularities of the development of phobic disorders with continuous and recurring course there were examined 67 patients with phobias. The duration of disease was from 5 to 50 years. There were 3 variants of relapsing and 4 variants of continuous course of phobic disorders taking into consideration either presence or absence of the tendency to their progression. In the group of relapsing patients 3% of the patients showed no tendency to progression. There was also a relapsing variant with a tendency to progression (6.0%). Continuous-undulating without tendency to progression (8.9%), continuous-undulating with a tendency to progression (19.4%), continuous-progressive (25.4%) and stationary (28.4%) variants. The authors emphasize that revealed peculiarities in the dynamics of phobic disorders with either presence or absence of the tendency to progression may help to improve the therapy of such states and to make more precise the prognosis of the disease.

[Characteristics of hypochondriacal conditions in children and adolescents].

Based on clinical and catamnestic examinations of 75 children and adolescents with hypochondriac disorders, variants of borderline and psychotic disorders were distinguished. Comparative age-associated assessments of the psychopathological manifestations of the disorders were carried out. During transition from the preschool to the junior and adolescent age, the rise was noted of the number of the clinical varieties of hypochondriac disorders, complication of their structure and of the emotional experiences. The authors indicate the manifestation of hypochondriac disorders in children and adolescents both within the framework of schizophrenia and neuroses.

[Dynamics and outcome of neuroses].

Based on the data obtained during examination of 320 neurotic patients the grouping of the varieties of the time-course of changes in neurotic disorders has been suggested. Variants of the lingering neuroses have been distinguished with regard to the presence or lack of the tendency towards progression and exacerbation of the symptomatology. Single attacks of neurotic disorders, repeated and recurrent neuroses were delineated within the framework of attack-like dynamics. Outcomes of neurotic disorders have been systematized.

[Delimitation of morbid and nonmorbid querulousness].

The differences are established in the manifestations and course of litigious-paranoid disorders of psychogenic personality-related origin and nonpathological querulousness. Pathologic litigiousness is characterized by a larger constitution-personality predisposition, lesser situational dependence and possibility of psychopathologic classification of querulous manifestations. An excessive intensity and length of querulousness, as related to the objective value of the psychogenesis, the more pronounced trend to litigiousness manifestations, progressive loss of their relation to situational cues, aggressive traits in behavior, are all characteristic of litigious-paranoid disorders. These and other features of litigious-paranoid disorders can be used as differential diagnostic factors in differentiating between pathologic and nonpathologic querulousness.

[Induced litigious-paranoid disorders with onset at an advanced age].

In some old patients with a psychogenic quarrelsome paranoiac symptom complex the latter develops by the mechanism of psychic induction. Pathological quarrelling shows a number of clinical dynamic peculiarities in patients in whom it has been induced as against those who have initiated it.

[Clinico-dynamic features of depressive neurosis with presenile or senile onset].

A study of 72 patients with depressive neurosis of old age has demonstrated that when neurosis develops in involutional or senile age it is characterized by a peculiar pattern of the psychopathology course and outcomes. To a considerable degree this peculiarity corresponds to the orientation and depth of changes of age-related social, psychological and biological nature.

[Psychogenic pathologic personality development in advanced age].

A study of 51 patients with psychogenic abnormalities developed in old age has shown that the development of such disorders is a comparatively frequent outcome of neuroses and reactive psychoses with the onset in the presenile period and only as an exception takes place in patients with the onset of psychogenic disturbances in old age. Depressive development has proved most typical for advanced age. Such manifestations as chronic subdepression, diffuse hypochondria, egocentrism and rent orientations are universal for all clinical variants of senile pathological development of personality. According to a regression analysis the development of psychogenic disturbances in the second half of life is facilitated by the objective significance and persistence of traumatic situations, a low professional level of patients and a number of other socio-psychological and biological characteristics of the person.

[Age and mental disorders with onset in the presenium and old age].

A study of 294 patients with psychogenic disorders appearing in the elderly, detected certain traits in the psychopathology and development of these disorders, depending upon their manifestation in the presenium or in senescence. Psychogenic diseases with an onset in senescence differed from those appearing in the involutional period by a more sluggish onset, monotonous, fragmentary, obliviated symptoms and a more favourable development. The differences in the symptomatology and development of psychogenic disorders corresponded to peculiar changes in the age basis of the mentioned periods of life.

[Clinico-dynamic features of obsessive-phobic neurosis with onset during involution and old age].

A study of 46 patients with obsessional neurosis with an onset in the presenium and senium demonstrated significant modifications of the clinical picture and its development. The study also showed that changes in the clinical picture were due to age changes proper, involutional-climacteric and cerebrovascular factors. It is shown that with the age manifestations of obsessive-phobic neurosis become more incomplete, alleviated, stereotypical, with an increased tendency towards a nonprogressive development (attack-like or inert).

[Psychopathology and the dynamics of psychogenic hysterical disorders arising in the presenile period and in old age].

As a result of the study of the nervous-mental state of 45 patients the author revealed considerable peculiarities of the clinical picture and the course of psychogenic hysterical disturbances depending on manifestations in the elderly or old age. It is shown that reactive hysterical states with the onset in the old age are distinguished by more elementariness, stereotypness, monotony of manifestations, absence of inclination for progression and transformation into a pathological development of a personality (what is frequently observed in the debut of hysterical disorders in the old age). The clinico-dynamic differences of psychogenic hysteria with the onset in presenium and old age are explained by different influence of common aged factors, climacteric and cerebro-vascular factors.
