[Thyroliberin: new physiological effects and prospects of clinical use].

In addition to the well-known activities of TRH (stimulation of respiration, antidepressant action, increase of motility, antishock action, therapeutic effects against ataxia, and so forth), some new properties were established. TRH was found (1) to stimulate the contractility of lymph vessels in ultra-low doses and to be useful in the treatment of acute pancreatitis; (2) to normalize cerebral circulation in rats and neonates after asphyxia; (3) to improve the electrophysiological parameters of the retina and pigmented epithelium in rats and man, to exert a long-term positive influence on the health status of patients suffering from senile retinal macular dystrophy; (4) to potentiate the analgesic effect of low doses of morphine in a definite range of doses (analog PR-546). It is of paramount importance that most of the indicated activities of TRH may hold promise for clinical application.

[The use of thyrotropin-releasing hormone in clinical practice as a lymphatic stimulator in the treatment of acute pancreatitis].

Experiments were conducted to study the modulating effect of thyrotropin-releasing hormone, (TRH) on the contractility of lymphatic vessels of the rat mesenterium. Later, TRH was applied in the clinic in the treatment of edematous and destructive forms of acute pancreatitis in 15 patients. as a result, echoscopy showed diminished edema of the pancreas, amylasemia decreased, the concentration of middle mass molecules in the blood reduced and their transport in the urine intensified. The agent was infused into the veins and into the lymphatic vessels, which allowed its dose to be reduced by half.

[Treatment of endotoxemia in generalized suppurative infection].

The authors describe the experience gained for many years in treating generalized purulent infection in surgical patients with sepsis and peritonitis. Provide the clinical characteristics of endotoxicosis in different types of surgical sepsis (traumatic, peritoneal, gynecological, therapeutic). Special attention is focused on the development carried out for many years of the methods of conservative and particularly active detoxifying with the use of their different combinations. Application of a complex of active treatment methods permitted the lethality to be reduced by 25%.

[Methods of oxidation in the treatment of hepatopathy of various etiologies].

The clinical experience gained with the use of different modified methods of oxygenation is described. The use of the methods in the treatment of patients suffering from severe hepatopathy associated with hyperbilirubinemia and hyperfermentemia, grave encephalopathy till liver coma enabled the treatment results to be appreciably improved.

[Plasma ceruloplasmin in patients with hepatorenal insufficiency].

Changes in plasma ceruloplasmin levels were studied by enzyme immunoassay in 81 patients with the acute hepatorenal insufficiency syndrome (AHIS). Changed plasma ceruloplasmin concentration was detected in the first days of the disease, this change depending on AHIS severity and unrelated to the disease cause. A lowering of plasma ceruloplasmin level below 0.75 mg/ml is considered to be a poor prognostic sign. Measurement of plasma ceruloplasmin is a test available for clinical laboratories, it is highly sensitive, accurate, and reproducible.

[Active methods of correction of endotoxemia in generalized suppurative infection].

The characteristics of the natural history of acute sepsis and the nature of the intoxication syndrome are analysed an on ample material (120 patients), and microbiological and immunological diagnostic findings are shown. Endotoxicosis in sepsis is attended by marked disorders of the immune status in the form of varying degrees of immunodeficiency. The authors generalize their experience with the management of endotoxicosis in sepsis. The article first shows the results of the combined use of various methods of endotoxicosis correction (homosorption, hemodiafiltration, plasmapheresis, endolymphatic therapy, ultraviolet irradiation of autoblood, hyperbaric oxygenation, application sorption) in the treatment of 120 patients with sepsis of various etiology. Active methods of detoxification via the blood and lymphatic system were first applied in the management of endotoxicosis in patients with sepsis.

[Evaluation of the clinical significance of determining the level of plasma fibronectin in acute poisoning].

Changes in plasma fibronectin (FN) levels in acute toxic conditions have been studied in 24 patients. A significant reduction of FN concentration was found within the first days of the disease; further changes in the FN levels are related to the disease outcome and have a prognostic value. An inverse correlation was found between the FN concentration and other blood biochemical parameters (LDH, creatinine, and urea). Developed renal insufficiency in the presence of hypofibronectinemia suggests a poor prognosis of the disease.

[Development of acute renal failure in crush syndrome and the methods of their complex treatment].

The author analyzes the many-year clinical experience gained in the treatment of 225 patients suffering from acute renal insufficiency resultant from the crush syndrome. He discusses the mechanism of the prolonged crush syndrome development, complicated with acute renal or hepatorenal insufficiency. Symptomatic conservative therapy is described in detail. Special attention is paid to the principles and methods of present-day active therapy (hemodialysis, hemoperfusion, hemofiltration, plasmapheresis, plasmadiasorption; lymphologic detoxication methods, i.e. drainage of the thoracic lymph flow, endolymphatic infusion of etiopathogenetic agents). The author emphasizes that the treatment may be effective only when a complex of methods for detoxication and impaired homeostasis correction is employed.
