Time course of left ventricular remodeling after myocardial infarction: a two-dimensional echocardiographic study.

In order to forecast the clinical course of acute myocardial infarction (MI), the time course of the functional changes of the left ventricular myocardium that result in remodeling was evaluated with two-dimensional echocardiography (2DE). The study group comprised 45 patients with anterior MI treated with successful percutaneous transluminal coronary angioplasty. 2DE studies were performed on days 1, 3, 7 and 14; months 1 and 3 and 1 year after MI, and the following parameters were recorded: (1) infarcted wall thickness, (2) traced length of the endocardium and of the epicardium on end-diastolic apical long axis images, and (3) wall motion score (total of asynergy scores of 16 segments of left ventricle (LV); normal: 0, hypokinesis: 1, akinesis: 2, dyskinesis: 3). According to the peak creatine kinase (CK) level, patients were classified into L group (CK > or =8000 U/L, n=16), M group (8000> CK > or =4000, n=13) and S group (CK <4000, n=16). The following results were obtained. (1) There was progressive thinning of the infarcted myocardium up to 1 month after (1 day: 9.3+/-1.7, 14 days: 6.3+/-1.7 vs 1 month: 5.9+/-1.8 mm, p<0.05; vs 1 year: 5.9+/-1.9 mm, NS). (2) Dilatation of the LV cavity occurred shortly after MI and continued up to 14 days (endocardium at 14 days: 176.8+/-13.6 vs 1 day: 164.1+/-11.4 mm, p<0.01; vs 1 year: 176.3+/-12.7 mm, NS). (3) The wall motion score improved rapidly by 14 days, and continued to improve gradually to 1 year (1 day: 12.2+/-3.4, 14 days: 6.8+/-4.0, 1 year: 4.6+/-3.1). (4) The expansion ratio (endocardial length at 14 days/1 day) was significantly greater in the L group than in the S group (p<0.05). Comparing the groups, the LV cavity of the L group remained dilated up to 14 days, whereas that of the S and M groups was dilated up to 7 days (L group 14 days: 179.3+/-11.9 vs 1 day: 156.9+/-9.2mm, p<0.01; vs 1 year: 180.0+/-14.1 mm, NS) (S group 7 days: 171.7+/-13.6 vs 1 day: 161.5+/-7.2 mm, p<0.01; vs 1 year: 172.7+/-14.4 mm, NS) (M group 7 days: 170.5+/-10.5 vs 1 day: 157.7+/-14.5 mm, p<0.05; vs 1 year: 177.08+/-9.6 mm, NS). Serial 2DE on days 1 and 14 after MI were useful for evaluating the course of LV remodeling and to forecast cardiac function in the chronic phase of MI. Determining the length of hospital stay on the basis of infarction size is justified.

Factors related to the severity of myelopathy in atlantoaxial instability.

STUDY DESIGN: This study retrospectively reviewed the factors seemingly associated with neurologic symptoms in 46 patients with atlantoaxial instability. OBJECTIVES: This study revealed the most significant factors related to the severity of myelopathy and surgical prognosis of atlantoaxial instability. SUMMARY OF BACKGROUND DATA: Spinal morphometry has been reported to provide a useful means of predicting the prognosis of cervical spondylotic myelopathy after surgery. However, no quantitative and statistical evaluation of this method has been reported in atlantoaxial instability. Furthermore, there is a poor correlation between neurologic deficit and plain radiographic findings of atlantoaxial instability. METHODS: Four times of clinical information were investigated, six parameters were measured on radiographs, and two morphometric parameters of the spinal cord were measured. These parameters were used as predictor variables. Three multiple regression analyses were carried out, using pre- and post-treatment Japanese Orthopaedic Association scores as criteria variables. RESULTS: Pretreatment Japanese Orthopaedic Association scores were found to correlate with the spinal compression ratio, history of trauma, and the C1-C2 ratio. Postoperative Japanese Orthopaedic Association scores correlated with preoperative Japanese Orthopaedic Association scores, the C1-C2 ratio, and the compression ratio. The multiple coefficient of determination was 93.2%. The recovery rate correlated with the C1-C2 ratio and age. CONCLUSIONS: Neurologic symptoms were found to be closely related to the morphology of the spinal cord. Compression ratio and the C1-C2 ratio were found to be useful in understanding neurologic symptoms. The C1-C2 ratio seems to serve as an important predictor of the prognosis of atlantoaxial instability.

Effects of aminoglutethimide phosphate on litter size and birth weight in superovulated rats.

In this study, litter sizes, birth weights and survival rates of neonates in superovulated rats treated with aminoglutethimide phosphate (AGP) were studied. Young (8- to 9-week old) and adult (14- to 16-week old) rats were treated with 20 or 40 IU PMSG between 1100 and 1300 hours on the day of early diestrus, followed by identical doses of hCG 52 to 54 hours later. These rats received 10 mg AGP on Days 0 to 2 of pregnancy (AGP rats). The duration of pregnancy in the AGP-treated and control rats was about the same. Mean live litter sizes in young and in adult AGP rats treated with 20 IU PMSG and 20 IU hCG were significantly higher (15.5 and 15.4; P<0.01) than in the control rats (11.5 and 12.6). When AGP was given to young and adult rats pretreated with 40 IU PMSG and 40 IU hCG, they produced means of 17.7 and 17.3 live neonates, respectively. The birth weights for young (5.55 to 5.88 g) and adult (5.89 to 5.96 g) AGP rats were smaller than those of respective control rats (6.44 and 6.40 g; P < 0.05-0.01). When the number of neonates was adjusted to 8 per dam for nursing and rearing, all of them were alive at 5 weeks after birth and showed normal growth. However, body weight gains of the progeny of AGP dams were lower than those of controls. These results indicate that rats treated with large doses of gonadotropins and AGP produce larger litter sizes than nontreated controls.

Effects of aminoglutethimide phosphate administration on the incidence of pregnancy and number of live fetuses in superovulated rats.

This study examined the effects of aminoglutethimide phosphate (AGP) administration on the incidence of pregnancy and number of live fetuses in superovulated rats. Immature (31- to 33-day-old) young (8- to 9-week-old) and (14- to 16-week-old) rats were treated with 20 or 40 IU of PMSG and the same dosses of hCG at 52 to 54 hour-intervals (GTH rats). One half of the GTH rats were injected with 10 mg AGP twice a day for the first 3 days of pregnancy (AGP rats). Pregnancy rates of all AGP rats (95 to 100%) were much higher than the GTH rats (12.7). Young and adult AGP rats treated with 20 IU of GTH had 14.7 and 15.3 live fetuses per dam, respectively. These numbers of live fetuses increased to 17.8 and 18.4 after treatment with 40 IU of GTH. These values were significantly greater than those of the control rats (12.3 to 12.8; P<0.01), but the fetal weights were lower. The results show that AGP administration improves pregnancy rates and the number of live fetuses in young and adult superovulated rats.

Developmental ability of porcine ova matured in porcine follicular fluid in vitro and fertilized in vitro.

Developmental ability of porcine ova matured in porcine follicular fluid (pFF) with FSH in vitro and fertilized in vitro was examined by culturing in BMOC-2. Forty-eight hours after insemination, 35.6% of ova cleaved normally, and this rate was significantly higher (13.0%) than that of the ova matured in a modified Krebs-Ringer bicarbonate solution. Twenty-four percent (29 120 ) of ova matured in pFF with FSH developed to the four-cell stage and two of them developed to the eight-cell stage 66 h after insemination. Most cleaved embryos stopped developing at the four-cell stage and neither the morula nor blastocyst stage was observed throughout the culture period as reported in the in vivo matured ova. In culture at 37 degrees C, the appearance of two-cell and four-cell embryos was delayed from that of in vivo embryos, but their development was significantly accelerated by culturing at 39 degrees C. These results show that pFF is an excellent maturation medium for porcine oocytes, and the developmental capacity of the ova matured in pFF seems to be similar to that of in vivo matured ova. Culturing at 39 degrees C was found to be more suit-able for the development of ova than 37 degrees C.

[A case of advanced gastric cancer showing complete cure by immunochemotherapy with FT-207 and OK-432 confirmed by thorough histologic examination of total gastrectomy matter].

A 57-year-old woman was referred to our hospital because of epigastric tumor on Jan. 16, 1980. Two advanced gastric carcinomas were recognized in the antrum and the corpus by X-ray and endoscopic examinations. The biopsy diagnosis was poorly differentiated adenocarcinoma. She was treated with FT-207 suppositories and subcutaneous injection of OK-432 without operation because of a suspicion of ascites and Virchow's metastasis. After five months, the tumor of the corpus disappeared, and, after nine months, that of the antrum changed to a scarlike lesion. On July 30, 1981, total gastrectomy with extended nodal dissection was performed. Histological examination revealed complete absence of carcinoma cells in the stomach and regional lymph nodes.

[Clinical trial on the effect of tegafur (SF-SP)].

SF-SP capsules containing sustained release granules of tegafur were orally administered 800 mg, b.i. d. for more than 4 weeks in 20 cases of advanced cancer, 11 of whom were evaluable (stomach 7, colon 1, liver 1, bile 1 and pancreas 1). The evaluation of antitumor effects was based on criteria of the Japan Society for Cancer Therapy, which are are almost the same as those of WHO. One partial remission out of 7 cases of stomach cancer was obtained, and its duration was 40 weeks. Toxicities were anorexia, nausea and vomiting, diarrhea and stomatitis, one case each of anorexia, and nausea and vomiting occurred within 10 days after SF-SP administration. No bone marrow depression, hepatic and renal disorders occurred after long-term SF-SP administration. SF-SP seems to be useful in the treatment of patients with gastric cancer.

Pseudosarcoma of the esophagus.

A 64-year-old man with pseudosarcoma of the esophagus is described. Morphologic examinations of this rare tumor were made, and the origin of the sarcoma-like cells of the tumor is discussed. The findings for the sarcoma-like cells of the tumor support the idea that the sarcoma-like elements arose through transformation of epithelial cells. The patient was unable to undergo surgical treatment and died of a generalized fungal infection.
