Psychometric properties of the Impact on Participation and Autonomy Questionnaire.

OBJECTIVE: To examine the homogeneity, test-retest reliability, construct validity, and concurrent validity of the Impact on Participation and Autonomy Questionnaire (IPAQ). DESIGN: Cross-sectional study with a test-retest subsample. PATIENTS: One hundred twenty-six persons from 5 diagnostic groups recruited from the outpatients clinics of 2 rehabilitation centers and the rehabilitation department of an academic hospital. INTERVENTIONS: The IPAQ and 3 other self-administered questionnaires (Sickness Impact Profile [68-item version], London Handicap Scale [LHS], Medical Outcome Study Short-Form Health Survey). The IPAQ was completed twice by 75 respondents within approximately 2 weeks. RESULTS: The IPAQ addresses autonomy and participation in 5 domains: autonomy indoors, family role, autonomy outdoors, social relations, and work and educational opportunities. Cronbach's alpha for the several domains ranged between.81 and.91, indicating good homogeneity. On item level, weighted kappas ranged between.56 and.90. On domain level, the test-retest reliability of the IPAQ was good: intraclass correlation coefficients ranged between.83 and.91. Convergent validity was largely supported by the correlations between 4 domains of the LHS and the IPAQ. Discriminant validity was best demonstrated by low correlations between the IPAQ and 2 domains of the LHS representing theoretically different constructs. CONCLUSION: The IPAQ is a reliable and valid instrument for assessing autonomy and participation in chronic disorders. Its responsiveness requires further study.

Critical illness polyneuropathy: a summary of the literature on rehabilitation outcome.

PURPOSE: To evaluate the available literature on outcome of critical illness polyneuropathy (CIP) and to identify rehabilitation problems. METHODS OF STUDY SELECTION: A literature search in electronic databases. Primary articles on outcome in CIP using the classification of ICIDH or Quality of Life were enrolled in this study. Because of the types of study design, the lack of information regarding validity and the variability, a quantitative analysis was not possible. Instead, the overall results were evaluated in a qualitative approach. RESULTS: In the literature minimal attention was given to the rehabilitation aspects and long-term outcome of patients with CIP. Outcome measures were mainly used on the level of impairments and in lesser extent on the level of disabilities. One year after the onset of CIP, muscle weakness and decreased sensory function were frequently observed. Reported functional disabilities were dependency in Activities of Daily Living (ADL) and walking disabilities. CONCLUSION: The specific course and long-term outcome of CIP remains unclear. Research on the course and long-term functional outcome in CIP is necessary in order to identify rehabilitation problems and to formulate treatment strategies specifically directed towards the outcome of CIP.

The effects of knee-ankle-foot orthoses in the treatment of Duchenne muscular dystrophy: review of the literature.

OBJECTIVE: A systematic review of the available literature on the effectiveness of knee-ankle-foot orthoses in the treatment of Duchenne muscular dystrophy. DESIGN: A computer search was carried out (MEDLINE 1966-97, CINAHL 1982-97) using the key words muscular dystrophy, rehabilitation, locomotion, braces or orthotic devices. References in relevant publications and nonindexed journals were also examined. Criteria to include and exclude articles were formulated. We used a systematic review procedure to evaluate the literature. Seven methodological criteria were formulated. RESULTS: Thirty articles describing 35 studies met the inclusion criteria for our review, nine studies were selected based on completeness of information on study population, treatment and quantitative presentation of the effect outcome. Operations on the lower limbs were performed on most patients. A concomitant programme of rehabilitation was not described thoroughly. A percentage success of treatment was calculated for eight studies. Median percentage after one year was 75.1, after two years 47.9 and after three years 24.3. The median for the means of independent walking was 24 months, the median for the means of assisted walking was 36.2 months and the median for the means of standing ability was 50.5 months duration. CONCLUSION: The scientific strength of the studies reviewed is poor. It seems that the use of knee-ankle foot orthoses can prolong assisted walking and standing, but it is uncertain whether it can prolong functional walking. The boys that benefit most have a relatively low rate of deterioration, are capable of enduring an operation and are well motivated.

Handicap questionnaires: what do they assess?

BACKGROUND AND PURPOSE: There is an increasing need to get insight into the social and societal impact of chronic conditions on a person's life, i.e. person-perceived handicap. The purpose of this study is to report how current handicap questionnaires assess handicap. METHOD: A literature search using both Medline and the database of the Dutch Institute of Allied Health Professions (NPi) was conducted for handicap questionnaires. Questionnaires were included if addressing handicaps or life roles, environmental influences and social consequences of a disease. Excluded were questionnaires focusing on only impairments, disabilities or quality of life. RESULTS AND CONCLUSION: 20 questionnaires were identified. Handicap is not uniformly defined in these questionnaires. Based on different concepts, the various questionnaires encompass different domains and different aspects are emphasized in similar domains. Fourteen questionnaires assess society-perceived handicaps, and do not address the life roles, care needs or individual problem-experience. Six questionnaires are to some extent person-perceived, but a generic person-perceived handicap questionnaire could not be identified. It is concluded that development of a generic person-perceived handicap questionnaire is essential for adequate assessment of needs, outcome, and relevance of rehabilitation interventions from the individual's point of view.

The development of a handicap assessment questionnaire: the Impact on Participation and Autonomy (IPA).

OBJECTIVE: To report on the feasibility and psychometric properties in terms of homogeneity and construct validity of a newly developed handicap questionnaire focusing on person-perceived handicaps: the Impact on Participation and Autonomy (IPA). DESIGN: Cross-sectional. SETTING, subjects and outcome measure: One hundred consecutive individuals from the outpatient clinic of the department of rehabilitation of an academic hospital administered the new questionnaire IPA. RESULTS: The results show good homogeneity and construct validity of the IPA. Factor analysis showed that the scale consists of four factors, explaining 68% of the total variance: social relationships, autonomy in self-care, mobility and leisure, and family role. Homogeneity of the four subscales was considered good, Cronbach's alpha ranged from 0.84 (family role) to 0.87 (social relationships). Feasibility in terms of the number of missing values and administration time needed was satisfactory. CONCLUSION: The first results suggest that the IPA promises to be a useful handicap questionnaire. Further research is needed to establish test-retest reliability, convergent validity and responsiveness to change.

Isomyosin expression patterns during rat heart morphogenesis: an immunohistochemical study.

An immunohistochemical study of cardiac alpha and beta myosin heavy chain (MHC) expression during rat heart morphogenesis was performed. In tubular hearts (embryonic days, ED10-11) coexpression of both cardiac alpha and beta MHC was found throughout the heart, except for the left free wall of the atrium, where only cardiac alpha MHC is detected. A transition of coexpression to single expression of either cardiac alpha or beta MHC begins at the same time in both atria and ventricles but requires a longer time for completion in the ventricules; in the atria transition takes place during the period ED 12-13 and in the ventricles during ED12-15. Furthermore, expression of cardiac alpha and beta MHC was detected in the sinus venosus, and cardiac alpha MHC expression was detected in the pulmonary veins. A comparison of the results obtained in chicken embryos revealed that in tubular hearts the expression pattern is similar, whereas in later developmental stages two major differences were observed: 1) transition of coexpression to single expression in rat ventricles appears to take a longer developmental period; 2) the persistence of areas of coexpression in the sinoatrial junction, dorsal mesocardium, atrioventricular junction, and outflow tract, as found in the chicken embryo in later developmental stages, is not found in the rat heart.

Isomyosin expression in developing chicken atria: a marker for the development of conductive tissue?

Isomyosin expression patterns in embryonic chicken atria during the first two weeks of development were analyzed immunohistochemically. In the 3-days embryonic chicken heart (HH19-20), strong coexpression of both isomyosins can be found as band-like zones at the lateral sides of the sinoatrial junction. The zones converge on the bottom of the atrium and continue as a band around the atrioventricular canal. In the 5-days heart (HH27-28) the coexpression area encompasses the entire sinoatrial junction and extends into parts of the sinus venosus and into the dorsocaudal atrial wall. In the 7-days heart (HH 32-33) the relative extension of coexpression areas reaches its maximum. Coexpression is also found in a ring-like band in the ventral (bottom) wall of the atria peripheral to the ring-like band in the atrioventricular junction. The latter band has now become continuous with the coexpression area in the bottom of the interatrial septum. Caudally coexpression extends behind the atrioventricular cushions towards the interventricular septum and cranially coexpression of the atrioventricular junction has become continuous with that of the ring around the outflow tract (cf Sanders et al. 1986). In the second week of incubation a decrease of coexpression is observed. The isomyosin expression pattern described in this study has put forward additional arguments that the conductive tissue originates from areas that continue to express both isomyosins relatively late in development.

Regeneration in surgically produced cleft palate in rabbits. A microscopical study.

In order to study the effects of surgical procedures on the regeneration process in cleft palate, an artificial cleft was made in 4-weeks old female New Zealand rabbits. The 47 animals were divided into 5 groups: group 1: controls, group 2: cleft made in the bony palate, using a dental burr (phi 3 mm), group 3: cleft as in group 2, oral mucosa stitched, group 4: cleft made without perforating the nasal mucosa; oral mucosa stitched ("Veau"), group 5: cleft made as in group 4; before stitching the oral mucosa pieces of autologous rib bone were introduced into the cleft ("Schuchardt"). At the ages of 4, 5, 6, 8, 12, 18 and 24 weeks, one or two animals per group were sacrificed. The palatal region was microscopically studied using 10 mu paraffin sections stained according to Herovici. It was found that it is difficult to make a reproducible cleft on the histological level. A large interindividual variation in local damage due to the surgical procedure was observed; this might contribute to the variation in growth results of the facial skeleton reported in literature. Signs of excessive bone resorption were found. The presence of bone in places where sutural tissue should be present, observed in some growing animals, will contribute to reduction of palatal width, medial collapse of the palate and asymmetry of the facial skeleton. There appeared to be no striking differences between the three methods of repair, used in group 3, 4 and 5 respectively.

The local expression of adult chicken heart myosins during development. II. Ventricular conducting tissue.

The development of the ventricular conducting tissue of the embryonic chicken heart has been studied using a previous finding that morphologically recognizable atrial conducting tissue coexpresses the atrial and the ventricular myosin isoforms. It is found that, by these criteria, at 9 days part of the ventricular conduction system consists of a myocardial ring located around the infundibula of the aorta and truncus pulmonalis. Part of this ring is formed by the retro-aortic root branch. The ring continues via the septal branch into the atrioventricular bundle and its branches, that all express both myosin isoforms. The retro-aortic root branch could be traced back as a part of the myocardial wall of the truncus arteriosus at the 4 days embryonic stage. At the 16th day of development, the septal branch, atrioventricular bundle and left and right bundle branches no longer express the atrial isomyosin, but two bundles originating from the septal branch still express both isomyosins, one being the retro-aortic root branch, the other being only immunologically recognizable and directed to the ventral side of the truncus pulmonalis; this latter we call the pulmonary root branch. Both bundles are remnants of the myocardial ring.

The conducting tissue in the adult chicken atria. A histological and immunohistochemical analysis.

A three-dimensional reconstruction from serial sections of adult chicken heart was made to verify whether Purkinje cells, that can be recognized by a number of well-known histological criteria, form specialized tracts in the adult chicken atria. This reconstruction revealed a loosely arranged network of Purkinje cells connecting the two atria. This network has not been described before. No tracts could be detected between the sinoatrial and the atrioventricular nodes. These atrial Purkinje cells express the atrial and ventricular myosin isoform, as determined by the use of monoclonal antibodies that were prepared against atrial and ventricular myosin isoform, respectively. Some atrial myocytes that are topographically closely related to the Purkinje cells and that cannot be distinguished from the surrounding myocytes with conventional histological criteria, express, apart from the atrial myosin isoform, also the ventricular myosin isoform. The similar expression pattern of these two cell types and their close topographical relationship suggest the presence of a more elaborate system specialized in conduction than the well-known conductive system found with conventional histological techniques.
