A new mutation in exon 12 of the gp91-phox gene leading to cytochrome b-positive X-linked chronic granulomatous disease.

We have previously reported a patient with cytochrome b-positive X-linked chronic granulomatous disease. Although the O2- production of neutrophils from the patient was completely defective, we presented data suggesting that the patient's cytochrome b was present at a normal level and possibly had normal spectroscopic features. Thus, to look for a mutation in the cytochrome b heavy chain (gp91-phox) gene, DNA analysis of gp91-phox cDNA derived from this patient was performed. As a result, we found that five nucleotides (1521 through 1525) within exon 12 were deleted, and a new sequence of eight nucleotides was inserted. This mutation converted Gln507-Lys508-Thr509 into His-Ile-Trp-Ala. Mismatched polymerase chain reaction showed that the mother has both wild and mutated alleles, confirming that this case was transmitted in an X-linked fashion. This mutation is different from those previously reported by others. The translocation of p47-phox and p67-phox to the membrane fraction occurred, indicating the complete formation of nicotinamide adenine dinucleotide phosphate (NADPH) oxidase complex. We conclude that this case suggests that the structure encoded on exon 12 of gp91-phox is important for electron transfer.

Immunohistochemical study of transforming growth factor-alpha expression in normal and perforated tympanic membrane.

An immunohistochemical study of transforming growth factor-alpha (TGF-alpha) distribution was carried out to clarify the mechanism responsible for accelerated epidermal cell proliferation after perforation of the rabbit tympanic membrane. In the normal tympanic membrane, TGF-alpha expression was not observed, whereas after perforation, over the whole tympanic membrane including the margin of the perforation, TGF-alpha-positive cells appeared and were scattered in the epidermal cell layer. After healing of the perforation, a marked decrease of TGF-alpha-positive cells in the tympanic membrane was observed. This finding suggests that TGF-alpha induces proliferation of epidermal cells after the perforation stimulus.

Acute myelogenous leukemia in pregnancy: fetal blood sampling and early effects of chemotherapy.

This report documents the acute toxicity of anti-leukemic chemotherapy on the fetus in utero by umbilical blood sampling. A patient with acute myelocytic leukemia diagnosed at the 23rd week of gestation received combination chemotherapy, and carried the pregnancy to successful delivery at the 34th week. During the course of pregnancy, the fetal condition was evaluated by serial real time sonograms and umbilical blood sampling through cordocentesis. Fetal hematopoiesis was preserved against maternal chemotherapeutic agents, and no developmental abnormalities were observed. This is the first attempt to evaluate the acute effects of chemotherapeutic agents on the fetus in utero by real time umbilical cord sampling.

[Sudden deafness in the elderly].

We investigated the clinical features of 64 cases of sudden deafness in patients more than 60 years of age. Forty-eight cases (75%) had profound deafness, 13 moderate and three mild deafness. The audiograms of 25 cases showed a flat pattern, 13 a gradually descending curve, and 13 total deafness. In 60 cases, the unaffected ear also had hearing impairment of variable severity. Fifty-one of these 60 cases demonstrated a physiological hearing threshold shift termed presbycusis in the unaffected ear. Hearing impairment in 35 cases (55%) was cured or improved by medical treatment including steroids. But in the majority of cases, the hearing threshold level of the affected ear did not improve above that of the unaffected ear when the unaffected ear had a hearing impairment. In particular, hearing impairment in five cases with total deafness in the unaffected ear did not improve. The elderly often had various systemic complications, including nine cases of diabetes and seven of hypertension. We treated patients with diabetes using steroids under the consultation of diabetic specialists. As a result, there was no case of complications induced by steroids. We consider the effect of treatment of sudden deafness in the elderly to be best judged in various degrees of hearing impairment in the unaffected ear.

[Genetic engineering in clinical pathology].

CD36 is a glycoprotein that is expressed on platelets, monocytes, and endothelial cells. To analyze the mechanism of CD36 deficiency on the platelet membrane, we have examined the expression of mRNA by RT-PCR and the Sau96I site on PCR products. In cases 3, 5, and 6, we have detected the normal size, but few products by agarose gel electrophoresis and in all cases, several restriction enzyme patterns with Sau96I digestion have been observed. These findings suggest that transcriptional control may affect the expression of the CD36 molecule on the platelet membrane and a different mechanism may be present for the transcription of the CD36 mRNA in megakaryocytes from monocytes.

Antenatal subdural hemorrhage causing fetal death before labor.

Antepartum intracranial hemorrhage, which can cause fetal death prior to the onset of labor, is a rare event. This report documents the clinical findings and magnetic resonance imaging (MRI) of an uncommon case of antenatal acute subdural hematoma. MRI was extremely accurate in delineating the possible extent and location of the lesion. This is the first report to demonstrate MRI of subdural hemorrhage resulting in fetal death in utero.

Successful pregnancy in a patient with heart failure secondary to aortic and mitral regurgitation.

In this report, maternal and fetal risks were evaluated in a pregnant patient who had no previously established heart valvular disease. A 41-old-year patient with aortic and mitral regurgitations suffered from progressive congestive heart failure during 20 weeks of gestation. She carried the pregnancy to a successful delivery at 31 weeks. During this time, sudden deterioration in the symptoms occurred, but both the fetal and maternal outcomes were uneventful. This experience of a good outcome may contribute to the future management of pregnancies associated with heart disease.

[Analysis of the regulation of HLA class II genes by forskolin].

The expression of HLA class II molecules is mainly regulated transcriptionally and this regulation is thought to play an important role to the control of immune response. In this report, we have studied the effect of adenylate cyclase activator, forskolin, to the expression of HLA class II molecules on the cell surface of an human multiple myeloma cell line, RPMI8226. On the northern blot analysis and FACS analysis, we have revealed that forskolin upregulated the expression of mRNAs of DQB and DRB gene and their products on its surface. On the sequence analysis of upstream of HLA-DQB gene, we have identified not only Y-,X-, W-box, which were thought to regulatory region of truncated gene, but also cAMP responsible element (CRE) like regulatory region, which located upstream of W-box. On the gel retardation assay, when we used DNA probes that were specific for CRE like region and Y-box, we have found newly detectable bands, which appeared by forskolin treatment. These data suggest that forskolin upregulates HLA class II molecules by means of the interaction between CRE and cAMP responsible element binding protein (CREB).

Second meiotic nondisjunction of the rearranged chromosome in a familial reciprocal 5/13 translocation.

We describe a 20-week-gestation male fetus with partial dup(5p) and proximal dup(13q), 47,XY,t(5;13)(p15;q21), + der(13)t(5;13)(p15;q21) mat. This finding is attributable to second meiotic nondisjunction of the rearranged chromosome in a maternal balanced reciprocal translocation. To the best of our knowledge, there have been only 3 previous reports of a similar error in the segregation of the rearranged chromosomes. For the first time evidence has been given that this unusual segregation is due to maternal second meiotic nondisjunction, using QFQ banding heteromorphisms. Second meiotic malsegregation should be taken into account in the consideration of reproductive problems in carriers of balanced translocations.

Acardiac acephalic monster extremely larger than its co-twin. A case report.

The acardiac monster represents a rare but one of the severest congenital anomalies which occur only in multiple pregnancy. The affected fetus is sustained by a parasitic anastomosis to the umbilical blood vessels of the normal fetus. We report an interesting case of a macrosomic acardiac acephalic fetus weighing 4.5-fold of another variable donor fetus (approximately 6 kg) in a twin pregnancy.

Signal transduction mechanisms of Ia induction in B cells by interleukin 4 and immunoglobulin receptors.

Molecular mechanisms of signal transduction through receptors for interleukin 4 (IL-4) are still largely unknown. To elucidate the second messenger(s) of IL-4 action in mature B cells, we performed blocking experiments with inhibitors of various aspects of cellular responses, using Ia-inducing activity of IL-4 as a readout system. In the event, only agents that are shown to inhibit calcium ion (Ca2+) release from intracellular stores, such as 3,4,5-trimethoxybenzoic acid 8-(diethylamino)octyl ester (TMB-8) and ryanodine, could block the IL-4 action in a dose-dependent fashion. These results suggest that the process leading to the final expression of IL-4 action may be mediated, at some point, by the release of Ca2+ from intracellular stores. In the parallel experiments with antiimmunoglobulin (Ig) antibody, we found that amiloride, an inhibitor of Na+/H+ pump, blocks the Ia induction by anti-IgM antibody. Thus the Na+/H+ exchange system activated by anti-Ig antibody may be present in mature B cells.

Effects of estradiol-17 beta and estriol on their binding sites in the rabbit uterus.

1. Receptors for estradiol-17 beta (E2) and estriol (E3) were detected in the rabbit uterus. 2. Saturation analysis of estrogen binding sites in the cytosol showed that the dissociation constants of E2 and E3 for the high affinity binding sites were 1.8 +/- 0.5 nM and 2.3 +/- 0.3 nM, respectively, when dextran-coated charcoal was used to isolate free and bound ligands. 3. To eliminate non-specific (cross) bindings to their receptors, effects of unlabeled E2 and E3 on [3H]E3 and [3H]E2 bindings was examined. 4. [3H]E2 cytosol binding was observed to be specific for E2 and [3H]E3 cytosol binding was more specific for E3. 5. E2 priming to rabbits increased the binding sites for both E2 and E3, which was also more potent than E3 priming. 6. Moreover, the increase in E2 binding sites was greater than that in E3 binding sites. 7. These findings may suggest that there are separate binding sites for E2 and E3 in rabbit uterus and that synthesis of their binding sites is regulated by E2 but not E3.

Possibility of anti-estrogen action from estriol specific binding sites in rabbit uterus.

1. This study was designed to investigate the clomiphene or tamoxifen binding to receptor sites for estradiol-17 beta (E2R) and estriol (E3R) in the rabbit uterus. 2. Those so-called anti-estrogenic compounds tended to inhibit E2-E2R and E3-E3R bindings equally. 3. The inhibitor constant of clomiphene for E2R was approximately 3.8 x 10(-8) M at 4 degrees C and that for E3R approximately 1.8 x 10(-8) M at 4 degrees C in a given case, determined by charcoal assay. 4. It is suggested that the anti-estrogenic compounds demonstrate their effects after binding either to E2R or to E3R. 5. There were some tissue differences of the contents between E2R and E3R. For example, the uterus and the cortex contained E2R, and the pituitary E3R more than the other.

Acute pseudo-obstruction of the colon (Ogilvie's syndrome) during pregnancy.

A case of colonic pseudo-obstruction (Ogilvie's syndrome) during pregnancy is described. The patient, with placenta previa, was in management of bleeding and also of premature labor. She was successfully treated with conservative measures for the Ogilvie's syndrome, and thereafter underwent a cesarean section only. There had been no recurrence of the pseudo-obstruction after the surgical procedure.

Metastatic vulvar Paget's disease responding to combination chemotherapy: case report.

No effective chemotherapy for advanced extramammary Paget's disease has yet been established. A case of vulvar Paget's disease with systemic metastases was treated with a mitomycin C, vincristine and cisplatin combination chemotherapy, by which the vulvar lesion and serum CEA were clearly decreased. After surgery and 5-fluorouracil treatment, a similar combination chemotherapeutic regimen was administered to treat a recurrence, however, the lesions became resistant.

Alleviation of autoimmunity in BXSB mice by monoclonal alloantibody to Ly-5 (CD45).

One of the hallmarks of immunological abnormalities in mice that are prone to syndromes resembling systemic lupus erythematosus is polyclonal activation of B cells and a switch of serum immunoglobulins or autoantibodies from IgM to IgG that coincides with onset of autoimmune disease. Therefore, we investigated whether the development of autoimmunity can be alleviated in such mice by administration of monoclonal alloantibody directed against Ly-5, because this was known to reduce levels of polyclonal IgG antibody without affecting other immunoglobulin isotypes. Our results indicate that at least some forms of autoimmune disease may be effectively treated by thus reducing the concentration of IgG autoantibody in this manner.

Successful pregnancy in a patient with paroxysmal nocturnal hemoglobinuria: case report.

In this report, the maternal and fetal risks in a patient with paroxysmal nocturnal hemoglobinuria (PNH) were evaluated. A patient with PNH carried a pregnancy to successful delivery, with a healthy male infant, and suffered from postpartum cerebral subcortical thrombosis followed by recovery. An elevated level of serum lactate dehydrogenase isoenzyme (alpha-hydroxybutyrate dehydrogenase) was observed during pregnancy; the level returned to normal by delivery. However, the hemoglobin concentration during pregnancy and post-partum was kept at 8 g/dl. This is the first report of a relation between the onset of hemolysis and pregnancy. The management of pregnancy in such patients is discussed.
