[Treatment by omentoplasty of vesicorectovaginal and rectovaginal fistulae].

In order to close the vesicorectovaginal fistulas (15 cases operated on) or rectovaginal (5 cases operated on) of irradiation origin, the author modifies his classical technique for the omentoplasty of the fistulous orifice, in terms of an obstruction in the upper thirds or the half of the vagina by using the pediculated omentum. The surgery is performed in two surgical times during the same operation: 1. the vaginal time is compulsory as the first surgical time consisting of the avivement the upper part of the vagina by the excision of the vaginal mucosa surrounding the fistulous orifices; 2. the abdominopelvian time consisting of the mobilization of the omentum (Kiricuta's technique), its descending into the pelvis and then, by an extensive cystotomy, the omentum is inserted into the vagina through the vesicovaginal fistulous orifice. The adherence created between the omentum and the vaginal wall free of mucosa, results in the cure of the fistulas. There are presented different technical variants. Out of 20 surgical cases, 18 were cured per primam during 14 days. Only 2 cases died due to peritonitis.

[The simplified technic of treatment by omentoplasty of irradiated and injured large vesico-vaginal fistulas. Apropos of 130 surgical cases].

Beginning with 1954, I. Kiricuta suggested and successfully used the stopping of the orifice in the vesico-vaginal fistula by omentum. It is presented the own experience during the last 33 years on 130 surgical cases (50 surgical fistulas, 65 complex vesico-vaginal fistulas on the intense irradiated tissues, 15 fistulas after irradiation as well as neoplastic ones). Of these, 20 cases of vesico-rectovaginal or rectovaginal fistulas operated on very successfully but that raises particular technical problems will be presented in another paper. Two surgical techniques will be here presented: 1. The classical technique: the omentum mobilized, it is the case, following a special technique (fig. 1a, 1b, 1c) is then interposed as a screen between the bladder and the vagina (fig. 2 & 3), that results in the cure of the fistulas due to the fibroblastic features of the omentum. 2. The simplified technique presents the following features: after the wide cystotomy and the excision (freshening) the mucosa of the upper third of the vagina (fig. 5) by the fistulous vesico-vaginal orifice, the vagina is stopped with the descended omentum through the bladder into the vagina (fig. 5 & 6). This last procedures is very easy to perform. By using these techniques the healing was obtained in 100% (there was only one death due to the upper digestive hemorrhage).

Biochemical markers in ovarian cancer.

Fucose and sialic acid were measured in 41 patients with malignant ovarian tumors and in 25 patients with benign tumors. Supplementary, serial determinations done in 20 patients, with advanced ovarian cancer subjected to primary or secondary surgery (optimal tumor debulking) and subsequent chemotherapy enables to suggest the possibility to use these parameters as markers in monitoring ovarian cancer evolution and treatment.

[Chromosome changes on 20 lesions of cervix uteri ].

Cytogenetic studies on biopsies of cervix uteri (5 dysplasias, 2 carcinomas in situ and 13 invasive carcinomas) are reported in this paper in order to detect the numerical and structural chromosome changes. In dysplasias, the deviation number is mostly restricted around the diploid mode while in carcinomas the range of deviation is wider and more than one modal number could be identified. Structural aberrations of chromosome 1 were found in one case of dysplasia and in 10 cases of invasive carcinomas. A long acrocentric was observed in three carcinomas and one dysplasia, while a medium-sized metacentric in three carcinomas. Further markers : minute chromosomes (in two carcinomas) and double minute bodies (DMs, in one carcinoma). These studies and the early ones suggest that the changes of chromosome 1 may be a relative late appearance of malignant transformation. The possible role of DMs is also discussed.

Some aspects of the endocrine status in the inflammatory breast carcinoma (Mastitis carcinosa).

The main endocrine features in a group of 87 patients with inflammatory breast carcinoma with very rapid evolution (group I) and those of 400 patients with breast cancer with no inflammatory signs (group II) have been compared. The mean age at menarche showed a significantly higher degree in group I, where the later menarche was reported in 36.8% of cases (greater than or less than 16 years). The mean number of abortions has been decreased in the same groups. No significant differences regarding to parity, lactation and mean age of the menopause have been found in these two groups. The mean values for total urinary estrogens were significantly higher in group I. The main fractions of 17-ketosteroids were estimated in 60 women of group I and 61 women of group II. The mean levels of these hormones were more decreased in group I, the most significantly differences in these two groups were registered for ethiocholanolone. In the most cases with decreased levels of ethiocholanolone, the survival time was also shorter. Such data showed a more prominent endocrine imbalance in the inflammatory breast carcinoma in comparison with breast cancer patients of normal evolution.

Nuclear magnetic resonance at low temperatures in lyophilized and non lyophilized normal and tumoral tissues. III. Study of the solid--fluid interface.

Adsorption water on lyophilized and non-lyophilized, normal and malignant tumoral tissues was studied by nuclear magnetic resonance. Comparative study of variations in the proton signal amplitude (A) and the water proton line width (delta v) at temperatures within the 0 degree to --40 degrees C range showed: (a) The identical behaviour of the proton signal amplitude of water adsorbed on normal rat tissues (liver, skeletal muscle). (b) Similar behaviour of the amplitude in malignant tumoral tissues and hepatic embryonic tissue. (c) Dependence of the relative variation of the resonance line width at --25 degrees and --5 degrees C upon hydration of the sample suggested that there was no difference between malignant and normal tissues at the level of the macromolecular structure-adsorption water interface.

Significance of serum fucose, sialic acid, haptoglobine and phospholipids levels in the evolution and treatment of breast cancer.

Serum fucose, sialic acid, haptoglobine and phospholipids were determined in 167 women with breast cancer stages I--III, 30 with benign lesions of the breast, 42 women in various physiological states of the mammary gland (pregnancy, early childbed and lactation) and compared with 30 healthy women as control. Serial determinations of these parameters during the radio-surgical treatment were done in 28 patients with breast cancer stage III. Fucose and phospholipids levels were significantly increased respectively decreased in the group of patients with breast cancers but unmodified in the others. Sialic acid and haptoglobine -- increased in patients with cancer -- were also elevated in patients with early childbed and benign affections of the breast. The surveillance of these four parameters during the radio-surgical treatment of breast cancer evidenced a good correlation between their modified levels and clinical state of the patients. The increase in fucose, sialic acid and haptoglobine respectively the decrease in phospholipids levels was associated with the clinical evidence of recurrences and metastases.

[Plastic surgery using part of the epiploon. Prior vascularisation of the transplant on the dorsal surface of the hand (author's transl)].

The epiploon has proved to be excellent material for reconstructive surgery. The authors present a new technique by which the epiploon may be transplanted into almost any part of the body, using the hand as a vector. The epiploon is implanted beneath the skin covering the hand and acquires a new vascularisation. In a second phase, the epiploon is divided at the point at which it leaves the abdomen. The free epiploon, vascularised by the hand, may then be transplanted into a wide range of areas: head, leg, foot, etc. The epiploic flap formed in this way has many advantages over the Gilles-Filatov tubular skin flap.

Axillary lymph-node and breast carcinoma interrelations in organ culture.

When Stage I and II cancers of the breast and their axillary lymph nodes were grown in the same tissue culture, a phenomenon of lymphocytic migration from the nodal explants to the tumor explants was observed. The lymphocytes infiltrated in and around the tumor nodules with cytotoxic effects; concomitantly, there was lymphocytic depletion in the nodal explants. The lymphocyte migration was particularly apparent when the axillary lymph nodes showed hyperplasia of the paracortical area and/or sinus histiocytosis. No correlation was found between the migration and the histologic type of disease or the degree of malignancy, but a strong correlation was observed between 1) the migration and the presence or absence of metastases in the explanted lymph nodes, and 2) the extent of the metastatic involvement in vivo. The lymphocyte migration was present only in the patient either uninvolved lymph nodes or only a small number (1-3) of metastatic nodes.

Levels of principal 17-ketosteroid fractions in urine of breast cancer patients with or without malignant evolutive push.

The main fractions of 17-ketosteroids were estimated in 102 women with breast cancer. Sixty two patients were cases with rapid evolution (malignant evolutive push) or very rapid evolution (inflammatory breast carcinoma). The mean ethiocholanolone levels and partially androsterone levels were significantly decreased in patients with rapid evolution in comparison with the corresponding values found in patients with breast cancer of slow growth.

Differences in uptake of aminoacids by patients with various forms of cancer.

The serum concentrations of alphaaminonitrogen (AAN), lysine, valine and leucines were determined before and within the period of 24 hr after the administration of Trophysan (10% solution of glucose containing a mixture of aminoacids) in 100 patients with cancer (17 with gastrointestinal carcinoma, 34 with uterus carcinoma, stages I to III; 8 with breast carcinoma, stages II and III; 15 with bronchogenic carcinoma, 10 with various localizations and 15 with metastatic cancer) and in 22 patients with benign tumors. A significant decrease in the serum content of AAN, valine and lysine was noted in patients with cancer (stages I to III) at 24 hours after the administration of Trophysan. This effect was absent for the patients with benign tumors. The enhanced uptake of aminoacids found in patients with cancer is probable the result of the negative nitrogen balance associated with the malignant state.

Inhibition of experimental metastatic spread by nonspecific stimulants.

It has been demonstrated by a syngeneic metastatic model of the XVII mouse that pretreatment of the animals with BCG and Freund's adjuvant brings about a significant inhibition of metastatic growth. This inhibition is further increased by combined pretreatment with nonspecific stimulants and additional specific immunisation of the animals against the given tumor.

Clinical and hormonal aspects in breast cancer with special reference to malignant evolutive push.

It is tried to differentiate between various stages of malignant evolutive push and normal mammary tumors to establish an individual therapeutic procedure. Besides mammography and thermometry, investigations of urinary excretions of estrogenic and androgenic steriods are qualified for this purpose. Patients with malignant evolutive push showed an increased excretion of estrogens which reached its highest levels in patients suffering from inflammatory mammary carcinomas. The excretion of 17-ketosteroids shows a behaviour vice versa. Altogether mammary tumors with malignant evolutive push are characterized by a state of absolute and relative hyperestrogenity respectively. This phenomenon preferably depends on the clinical stage of the disease than on the age of the patients.
