Clinical correlates of enuresis in ADHD and non-ADHD children.

Enuresis and attention deficit hyperactivity disorder (ADHD) are common childhood disorders that often co-occur. Although each has been linked to neurodevelopmental immaturity and increased risk for psychopathology, the clinical correlates of enuresis remain unclear. Subjects were 140 6-17-year-old boys with DSM-III-R ADHD and 120 non-ADHD controls. Information on enuresis and psychiatric diagnoses was obtained in a standardized manner blind to the child's clinical status. Our results show that (1) enuresis did not increase the risk for psychopathology in children with or without ADHD; (2) enuresis was not associated with psychosocial adversity or developmental immaturity; (3) enuresis was associated with increased risk for learning disability, impaired intellectual functioning, and impaired school achievement in normal control children but not in children with ADHD; and (4) the same pattern of findings was obtained after stratifying children with enuresis by primary versus secondary and by nocturnal versus diurnal subtypes. These results suggest that the clinical implications of enuresis may differ for ADHD and non-ADHD children.

Influence of Epstein-Barr virus genomes on patient survival in Hodgkin's disease.

In previous studies, Epstein-Barr virus was considered a possible etiologic factor in Hodgkin's disease. Two hundred twenty-nine cases of Hodgkin's disease were investigated for the presence of Epstein-Barr virus DNA using the polymerase chain reaction technique on formalin-fixed, paraffin-embedded lymph node tissue to clarify the clinical importance of the incidence of this genome. In 42 cases (18.3%), genomic DNA was not amplifiable. The remaining 187 cases included the following subtypes: lymphocyte-predominant type (n = 13), nodular sclerosis type (n = 98), mixed cellularity type (n = 68), and lymphocyte-depleted type (n = 8). Sixty-six cases (35.2%) were positive for Epstein-Barr virus DNA. In the statistical analysis of available follow-up data from 130 patients, no influence of a positive Epstein-Barr virus DNA finding on length of survival time was revealed. This was true within the cohort of all patients and within the histologically defined subtypes of Hodgkin's disease. In this investigation, detection of Epstein-Barr virus DNA by polymerase chain reaction showed no prognostic relevance for patients with Hodgkin's disease.

Primitive neuroectodermal tumor arising in the skin. Differentiation from neuroendocrine carcinoma of the skin.

We report a 25-year-old male patient with primitive neuroectodermal tumor presenting as a subcutaneous tumor on the right buttock, which was initially diagnosed as neuroendocrine carcinoma of the skin. Subsequently, local recurrence and metastatic spread to the lung occurred. The tumor was resistant to highly aggressive chemotherapy. The patient died 6 months later with severe pulmonary and lymph node involvement.

[Influence of Epstein-Barr virus genome on patient survival in Hodgkin's disease].

In the literature EBV is considered a possible etiologic factor for Hodgkin's disease (HD). We investigated 187 cases of HD for the presence of Epstein Barr virus using the polymerase chain reaction (PCR) technique on formalin fixed, paraffin embedded lymph node tissue to clarify the clinical importance of the incidence of this genome. The 187 cases included all subtypes. 66 cases (35.2%) were positive for EBV DNA. The statistical analysis of follow-up data from 130 patients revealed no influence of EBV DNA on survival time. In our investigation detection of EBV DNA by PCR showed no prognostic relevance for patients with HD.

Total unilateral lung gangrene in Hodgkin's disease: treatment by thoracostomy.

Total gangrene of the left lung developed in a 30-year-old male patient with a pulmonary recurrence of Hodgkin's disease after mediastinal irradiation and chemotherapy. Clinically, tension pyopneumothorax and severe septic shock were present. Surgical repair was done by thoracostomy, resecting three ribs. A 2 x 0.5-cm hole in the necrotic wall of the left main bronchus was covered with an intercostal muscle bundle. The necrotic pleural surfaces were treated openly by daily change of dressings. The patient recovered satisfactorily and underwent four further courses of chemotherapy without any complications.

[Whipple's disease: abdominal lymphoma, intermittent fever and recurrent arthralgias].

A diagnostic laparotomy was performed on a 41-year old woman who, having suffered from recurrent arthralgias for several years and intermittent fever for many months, was found to have extensive enlargement of abdominal lymph-nodes, raising the suspicion of malignant lymphoma. Frozen-tissue biopsy was highly suspicious of Hodgkin's lymphoma, but light- and electron-microscopy revealed Whipple's disease, confirmed in an endoscopically obtained duodenal mucosal biopsy. The arthralgia quickly responded to antibiotic treatment. This case demonstrates the potential difficulties and errors in the diagnosis of Whipple's disease.

[Extension of the rosette test in the estimation of TAA-binding cells in breast cancer and mastopathy patients].

The antigen specific rosette test was used for the evaluation of a specific sensibilization of a tumour bearer to TAA. The frequency of the rosettes formed by peripheral blood leucocytes of breast carcinoma patients with antigen-coupled particles is significantly higher than that produced by leucocytes of normal donors. Following operation normally the number of TAA specific rosettes decrease, in 4 cases with recidives we found high numbers of TAA binding cells. In another 11/98 patients following ablatio mammae later than half a year we found positive test results and also in 17/60 patients with mastopathy. The clinical relevance of these results must be proofed in further investigations.

[Determination of TAA-binding cells in peripheral blood of breast cancer patients (author's transl)].

Circulating human breast tumor-associated antigen (TAA) specific lymphocytes were detected in breast carcinoma patients by antigen specific rosette technique. About 60 percent of patients with breast cancer showed a positive reaction in our test system, the number of rosettes were considerably increased in comparison to the values of blood donors. Following operation normally the number of TAA specific rosettes decrease, contrary, in all cases with recidives, tested, we found positive test results. There is evidence, that our modification of the specific rosette-technique is suitable for detection of malignomas and can be important for the postoperative control of patients with breast carcinoma.
