Indocyanine green-guided laser photocoagulation of focal spots at the edge of plaques of choroidal neovascularization.

OBJECTIVE: To determine whether indocyanine green-guided laser photocoagulation of focal spots at the edge of plaques of neovascularization is beneficial for patients with untreatable occult choroidal neovascularization by fluorescein imaging. PATIENTS: A pilot series of 23 eyes were identified that had untreatable occult choroidal neovascularization secondary to age-related macular degeneration with focal spots at the edge of a plaque of neovascularization on the indocyanine green study. After informed consent was obtained, indocyanine green-guided laser photocoagulation was applied solely to the focal spot at the edge of the plaque. RESULTS: Anatomical success with resolution of the exudative findings was noted in 15 (79%) of 19 eyes at 6 months, 13 (68%) of 19 eyes at 12 months, and six (37.5%) of 16 eyes at 24 months. The median follow-up period was 18 months (range, 3 to 44 months). Visual acuity was stabilized or improved in nine (69%) of 13 successfully treated eyes at 1 year. Of the successfully treated cases at 1 year, nine (69%) of 13 had a final visual acuity of 20/100 or better. CONCLUSIONS: Our preliminary study of indocyanine green-guided laser photocoagulation of focal spots at the edge of plaques of neovascularization suggests that this technique may improve the visual prognosis of these patients with presently untreatable disease. A prospective, randomized, controlled clinical trial is warranted to evaluate this treatment approach.

Retinal vascular involvement in acute toxoplasmic retinochoroiditis.

The present study deals with the retinal vascular involvement in 64 patients with toxoplasmic retinochoroiditis during the acute phase of the disease and its evolution. Vascular involvement was noted in all 64 eyes during the acute phase of the disease. In 59 (92%) out of 64 cases the vascular involvement was located in the same retinal quadrant as the active toxoplasmic lesion. In the remaining 5 eyes (8%) the vascular participation was found in all four retinal quadrants. In 3 (5%) out of 64 cases, the vascular infiltration was extremely severe and resulted in retinal vascular obstruction. In all three cases the vessel traversed the active toxoplasmic lesion. In 35 (55%) out of 64 patients the initial vascular picture changed in the course of the acute phase of the disease. In these patients, the lesions had extended to the adjacent vessels or to other parts of the involved vessel. In the further course of the evolution of the active toxoplasmic lesion, the vascular involvement did not persist indiscriminately. It was noted that in 14 (22%) out of 64 cases the vascular lesions gradually regressed and eventually disappeared together with the active toxoplasmic lesion and the formation of the retinochoroidal scar. In the remaining 50 (78%) out of 64 cases the vascular involvement either disappeared after the establishment of the retinochoroidal scar in 3-12 months (29 cases) or remained permanently (21 cases).

Ocular findings in patients with successful renal transplantation.

Successful renal transplantation has a favourable effect on the development of ocular disorders in periodic haemodialysis patients. Certain complications arise in the eyes of the recipients and are attributed mainly to the immunosuppressive medication. The purpose of our study was the recording of the ocular complications in patients with successful renal transplants after long-term stabilization of renal function and immunosuppressive therapy.

[Thalassemia and macular subretinal neovascularization].

A 12 year old boy who suffered from homozygous beta-thalassemia came to our clinic complaining of progressive deterioration of the visual acuity of the left eye. His visual acuity at this eye was finger counting at a distance of 50 cm. Fundus examination revealed a tortuous course of the fundus vessels, angioid streaks radiating from the region of the disk and a greyish elevated circular lesion measuring about half a disk diameter in the macular area. Fluorescein angiography showed the existence of a subretinal new vascular membrane. The possibility of the development of severe fundus complications in patients with thalassemia syndromes is discussed.

Erythrocyte uroporphyrinogen-I-synthase activity in beta-thalassaemic patients.

The haem pathway enzyme uroporphyrinogen-I-synthase (UPGS) was assayed in erythrocyte samples from twenty normal, twenty beta-thalassaemia heterozygotic and twenty beta-thalassaemia homozygotic subjects, after partial separation of the erythrocytes according to their age. UPGS erythrocyte enzyme concentration activity was significantly higher in the young than in the old erythrocytes of normal (66.5 +/- 11.8 v. 45 +/- 9.5 nmol h-1 1(-1), mean +/- SD, P less than 0.001) and beta-thalassaemia heterozygotic subjects (70.1 +/- 18.7 v. 49.8 +/- 14.5 nmol h-1 1(-1), P less than 0.001), but not in patients with homozygous beta-thalassaemia (46.0 +/- 12.8 v. 44.1 +/- 12.5 nmol h-1 1(-1), P = 0.65). Furthermore, UPGS enzyme concentration of both young and old erythrocytes of homozygous beta-thalassaemia was significantly lower than that of the young (P less than 0.001) but similar to that of the old (P greater than 0.2) erythrocytes of either normal or beta-thalassaemia heterozygotic subjects. Since severe chronic haemolysis due to haemoglobinopathies is associated with increased UPGS enzyme concentration, these results suggest that UPGS activity may be suppressed in homozygous beta-thalassaemia.

Twelve years' continuous wear of the same therapeutic soft contact lens: a case report.

PURPOSE: To describe a case of a patient who had worn the same therapeutic soft contact lens (TSCL) continuously for twelve years, since he had failed to attend normal follow-up visits. METHODS: Microbiological histological and scanning electron microscopic (SEM) studies of conjunctiva, cornea and TSCL were done. RESULTS: Cultures were negative. Corneal histology revealed mild stromal edema and mild epithelial parakeratosis. Corneal SEM was remarkable for the preservation to some extent of normal corneal epithelial specialization with microtricae and microvillae. SEM of the TSCL showed a ruffed multi-layer surface with several cracks including different types of cells. CONCLUSIONS: The patient showed surprising tolerance to the continuous wear of the same contact lens for 12 years.

Multifocal ERG in retinal detachment surgery.

PURPOSE: The main purpose of this study is to evaluate the visual function of eyes operated on for rhegmatogenous retinal detachment. METHODS: Fifteen patients were studied suffering from peripheral retinal detachment treated with the Lincoff-Custodis method. Visual field was tested with the Humphrey Field Analyzer and the analyzer program 30-2, which scanned a 0-30 degrees field. The multifocal ERG was recorded with the VERIS III (Visual Evoked Imaging System, Tomey, Nagoya, Japan). RESULTS: There was great loss of retinal response densities in the area of retinal detachment (1.3 +/- 0.7 nV/deg2). However, a decrease in retinal response densities was also recorded in the reattached retina (6.90 +/- 0.7 nV/deg2). CONCLUSIONS: MFERG is an objective tool for monitoring the functional abnormalities affecting the vision of eyes operated for retinal detachment.
