[Extrapleural mesothelioma].

Malignant mesothelioma of the peritoneum, pericardium and tunica vaginalis is a rare neoplasm usually associated with exposure to asbestos. The disease is most common in males over the age 40 years with sings and symptoms of neoplastic disease with or without a palpable abdominal mass, abdominal pain and ascites. Diagnosis is often possibly only after direct vision and histologic examination. Different types of peritoneal mesothelioma occur in adults and in children. Only small proportion of all patients with mesothelioma can be cured surgically. Both radiotherapy and standard chemotherapy have only limited usefulness. The optimal management malignant mesothelioma continues to be defined.

[Primary extragonadal germ cell tumors].

Extragonadal germ cell tumors (ECT) account for only 2-5% of all germ cell neoplasms in adults. These tumors in part biologically distinct from their testicular counterparts and have distinct clinical features. The author presents a review of articles in the literature about hematologic neoplasia associated with mediastinal germ cell tumors. These hematologic neoplasms are not related to therapy. The optimal management ECT continues to be defined.

[Primary forms of extralymphatic non-Hodgkin's lymphoma].

In 1981-1990, 584 patients with non-Hodgkin malignant lymphoma, 209 women and 375 men, aged between 17 and 75 years, were treated at the I Department of Medicine and Department of Hematology, Silesian Medical Academy in Katowice. In this group the primary localization outside lymph nodes was seen in 26 patients (9 women and 17 men), i.e. 4.6% of all patients with malignant non-Hodgkin lymphomas. Most frequently digestive tract was involved-19 patients (73.1%). Twenty three patients were treated surgically followed by chemotherapy and/or irradiation. Not operated patients were treated with either chemotherapy or both radio- and chemotherapy. Complete remission was achieved in 17 patients (65.4%), partial remission in 4 (15.4%) patients. Five patients did not respond to therapy (19.2%). A mean survival time was 49.4, 14.3, and 6.4 months, respectively.

[Leiomyosarcoma of the stomach and small intestine].

Three cases of leiomyosarcoma are presented. The primary lesion to the stomach was seen in 2 patients while the small intestine in the remaining patient. Two patients underwent radical surgery. All patients were treated with multiple drug regimens. Radiotherapy was additionally carried out in one patient. One patient who underwent radical surgery and given maintaining chemotherapy is alive in the complete remission since 6 years while two remaining patients died within 3 and 6 years because of recurrence and haematopoietic proliferation.

[Results of combined therapy in patients with nonseminomatous testicular tumors].

The results of multidrug therapy of 44 male patients with non-seminomatous testicular tumours are discussed. The diagnosed tumours were of all stages of the clinical advances and were treated with combined chemotherapy (surgery and PVP regimen). Thirty two patients (73%) are followed up between 7 and 103 months and survived, complete remission is noted in 29 patients (66%) including 6 patients who survived over 5 years. Symptoms of the tumour are found in 3 patients. Twelve patients (27%) died. The treatment did not produce life-threatening complications. The obtained results have been compared with those reported by other authors.

[Failure of therapy in non-seminomatous tumor of the testes].

Sixty four patients with non-seminomatous tumors of the testes were treated with chemotherapy with cisplatin during 12 consecutive years. A group of 22 (34.3%) patients in whom therapy failed was analysed. Nineteen of them (29.6%) died, and 3 (4.6%) live with the symptoms of disease. At the beginning of therapy 13 (59%) patients presented with III degree of clinical advancement of cancer. Complete remission after induction treatment was achieved in 6 (27.7%) patients but all experienced recurrence (4 early and 2 late). Prolonged survival since hemicastration in the group of patients with complete remission amounts to approximately 36.5 months whereas in the group of partial remission only 13.6 months. These results are similar to those reported by leading medical centres.
