[Inner ear hearing loss following cerebrospinal fluid puncture: a too little appreciated complication?].

Lumbar puncture for myelography, spinal anaesthesia and aspiration of cerebrospinal fluid is a very common procedure. Although it has been known for a long time that hearing impairment may be a late complication, only a few case reports have been published, and not all have included audiograms. We present nine cases of hearing loss following myelography, lumbar puncture and spinal anaesthesia. It appears likely that this rare complication arises only in subjects with a wholly or partially patent cochlear aqueduct, allowing loss of perilymphatic fluid into the cerebrospinal space. Hearing loss was seen in eight of nine patients in the lower frequencies and in six of nine patients on both sides. Hearing recovered in six of nine patients. Transient hearing loss may occur more often than is generally assumed, and the symptom can remain unnoticed. Since not all of these hearing losses proved to be fully reversible, we suggest to inform patients about this complication for medico-legal reasons.

[Multifocal presentation of cystadenolymphoma (Warthin tumor) in the parotid gland and larynx].

Warthin's tumour of the larynx is an unusual finding. In present literature only four cases have been described. A fifth case of a 77-year-old woman suffering from Warthin's tumour of the left parotid gland and the left false cord is presented. Moreover, this patient had an inner and outer laryngocele. This combination of Warthin's tumour of the larynx with a laryngocele has not been described before in literature. The theories about the development of Warthin's tumours are discussed, in particular their relationship to the development of the branchial cleft apparatus.

[A comparison of digital luminescence radiography and conventional radiodiagnosis in sialography].

In 25 patients sialographies were obtained by both conventional film-screen system combinations and computed radiography (CR), the digital exposure dose being 50% of the conventional one. In CR two differently post processed images were obtained from one x-ray exposure: one "conventionally" processed image resembling a conventional film-screen system combination and a complementary "enhanced" image showing a higher edge enhancement at the same spatial resolution. CR proved to be equivalent to conventional radiography despite the reduction in exposure dose. Using high edge enhancement the peripheral glandular ducts as well as the central ducts superimposed by bony structures were optically accentuated. However, diagnostical information exceeding the conventional film-screen system combination was not obtained by CR. Artefacts by post processing procedures relevant for diagnosis were not observed.

Topochemical demonstration of zinc in meningiomas.

In agreement with the atomic absorption tests, histochemical demonstration of zinc in meningiomas using the dithizone method revealed a higher level of zinc in fibromatous meningiomas than in its meningiotheliomatous variant. Calcified psammoma bodies and vessels exhibited an intensely positive reaction on their surface. The metabolic function of zinc in the synthesis of fibre proteins, as well as in metallo-enzymes associated with mineralization, explains these findings.

[Treatment of tinnitus with iontophoresis and local anesthesia].

50 previously unsuccessfully treated patients suffering from tinnitus were given local-anaesthesia-iontophoresis. By iontophoresis it is possible to build up an electric field, in which a local anaesthetic (lidocaine hydrochloride) is able to penetrate through the tympanic membrane. Experimentally it has been proven that this local anaesthetic has a direct effect on the inner ear. In contrast to intravenous and intratympanal application there are no toxic side effects when applying local-anaesthesia-iontophoresis. Tinnitus was reduced completely or partly in 31 patients after repeated application to one or both ears. No connection was seen between the success of therapy and the origin of the tinnitus. Treatment was unsuccessful in 19 cases. No serious side effects were seen. Local-anaesthesia-iontophoresis can be recommended as an attempt to treat tinnitus, especially in those cases who have not yet responded to drugs.
