Epidemic and sporadic hepatitis E virus transmission in West Kalimantan (Borneo), Indonesia.

A cross-sectional survey was conducted in West Kalimantan (Borneo), Indonesia to geographically profile hepatitis E virus (HEV) prevalence in the riverine areas recognized as the foci of epidemic HEV transmission in 1987. Additionally, a contiguous, although distinct, population with no identifiable historical exposure to epidemic HEV was surveyed downstream for comparative purposes. Eight hundred eighty-five sera were assayed by enzyme immunoabsorbent assay for anti-HEV IgG and anti-hepatitis A virus (HAV) IgG markers. A very high percent (90%) of both the outbreak and comparison populations was anti-HAV IgG positive by the age of nine years. In contrast, the prevalence of anti-HEV IgG in the outbreak area (50%) was significantly higher than in the comparison area (23%) (P < 0.0001). In both the outbreak and comparison areas, anti-HEV IgG prevalence increased with age ( < 0.0001), except for the group > or = 50 years of age. The prevalence (53%) of antibody to HEV in the population > or = seven years of age from the outbreak area (alive during the actual 1987 outbreak) was significantly (P < 0.0001) greater than among the children < seven years of age (born after the outbreak) (15%). However, anti-HEV IgG prevalence among the population from the comparison area did not differ significantly between the > or = seven- (23%) and < seven- (20%) year-old age groups. The percentage of anti-HEV IgG-positive individuals among males (47%) from the outbreak area was lower (P < 0.05) compared with females (55%). While overall usage of river water for drinking purposes was not universal, dependence on river water as a primary source was significantly higher (P < 0.001) in households from the outbreak area (60%) compared with the comparison area (30%). This study indicates persistence of an anti-HEV IgG response in a large percentage of the population seven years after an epidemic of HEV infections. Also, the relatively high prevalence (15%) of anti-HEV in children < seven years of age from the outbreak area reflects continuing, sporadic infections.

Second report of AIDS related attitudes and sexual practices of the Jakarta Waria (male transvestites) in 1995.

As the second part of a community based educational campaign to convey the risk of HIV/AIDS to commercial sex workers in Jakarta, from May-July 1995, a total of 253 male transvestites (WARIA) were questioned about their sexual behavior patterns and their knowledge and attitude toward HIV/AIDS. In the previous report, 1991-1993, there was one out of 830 WARIA found HIV positive in November 1993. In this study, 1995, a total of two out of 253 WARIA were confirmed of HIV infection in July 1995. Most of them still have incorrect knowledge on HIV/AIDS transmission mode, they are still practicing high risk sexual attitudes such as an exchange partner rate of 5 men per three weeks, low condom use (1.2 out of the last 5 sexual contacts). The reasons for not using condoms were forgetfulness 35.3% and partner does not like condom 38.2% Most of WARIA know about condoms (94.5%), but it is difficult to access condom use from small shops around them. Therefore, to prevent further spread of HIV/AIDS in WARIA, condom should be used constantly and properly. It has been shown from another study, that more information, better availability and better promotion of condoms can increase condom use. Thus, attention should be placed on various ways of distributing condoms for WARIA in Jakarta, especially community-based distribution by peer leaders, social marketing and commercial sales.

AIDS related attitudes and sexual practices of the Jakarta WARIA (male transvestites).

As part of a community based educational campaign to convey the risk of HIV infection and AIDS to commercial sex workers in Jakarta, over 600 male transvestites (WARIA) were questioned about their sexual behavior patterns and their knowledge and attitude towards HIV infection and AIDS. Most expressed a genuine fear about AIDS, but they stated they did not have sufficient information to determine if their risk of infection, at this time, was great enough to command a change in their sexual conduct. Among the group who felt they were not at risk for HIV infection, 40% had 8 or more different sex partners per week. Low risk receptive oral sex, thigh massage (simulated vaginal sex) and masturbation of the client were routine activities but high risk receptive anal sex without condoms was most common. Despite such high risk behavior, the WARIA community remains free of HIV infections. However, if they refuse to accept the endorsement of their peer leaders and they fail to alter their sexual behavior because of an unawareness of their risk, that status will change soon.

Age associated morphological changes in the lymphoid system of tropical cattle.

Superficial (superficial cervical , subiliac) and deep (medial iliac) lymph nodes, tracheobronchial and mesenteric lymph nodes, Peyer's patches, thymus and spleen were collected from 165 apparently normal animals ranging from fetuses of six months gestational age to cows approximately 10 years old. Additionally, palatine tonsils were collected from 58 other animals of comparable ages. Weights of animals and most of the above organs were obtained and in 39 animals, representing seven age groups, quantitative histological studies were made on Peyer's patches or lymph nodes to ascertain any differences attributed to age or anatomical location of node. With the exception of thymus, weights of all lymphoid organs increased with age until puberty or maturity after which a levelling of organ weights was apparent. Organ weight-bodyweight ratios, however, were highest in fetuses or young animals after which they decreased (somewhat irregularly) with age. Few trends were observed relating histological findings to age. Depth of Peyer's patches and the follicular-non-follicular ratio (FNFR) in the cortices of superficial cervical plus subiliac nodes, however, increased rapidly (to approximately nine months and three years of age, respectively), reached a plateau, and then decreased. Pigment deposition in all nodes tended to increase with age. In regard to anatomical location, the medial iliac node (probably because of its deep location) tended to have the lowest corticomedullary ratio and FNFR, smallest germinal centre size and least pigment.

Epidemic dengue 3 in central Java, associated with low viremia in man.

An outbreak of dengue type 3 was studied in Central Java, Indonesia, in 1978. In contrast to previous dengue 3 epidemics in Central and East Java, this outbreak was less explosive, associated with mild illness, and low viremia. The dengue virus isolation rate from serologically confirmed patients was only 32% compared to 65% for an epidemic in Bantul a year earlier. Neither dengue hemagglutination-inhibition antibody titers nor day of illness on which specimens were collected accounted for this difference. These data suggest that some naturally occurring strains of dengue virus (endemic strains) are associated with low viremia and generally cause only mild illness in man.

Epidemic dengue hemorrhagic fever in rural Indonesia. I. Virological and epidemiological studies.

Virological studies were carried out during an epidemic of dengue hemorrhagic fever in Central Java, Indonesia in 1976. Dengue virus was isolated from the acute sera of 45 of 69 patients (65%). The isolation rate was higher in primary than secondary cases. Dengue 3 was the predominant serotype being transmitted (27 isolates), but both dengue 1 (8 isolates) and dengue 4 (10 isolates) were also being transmitted. A composite picture of magnitude and duration of viremia showed that many patients were circulating over 10(8) MID50 per milliliter dengue 3 virus for the first 3 days of illness and that viremia persisted for 5-6 days in some persons. If all shock cases were considered, there was no relationship between dengue serotype and severity of disease. All three confirmed fatal cases, however, were associated with dengue type 3 infections.

Dengue virus isolation in Indonesia, 1975-1978.

Virus isolations from dengue hemorrhagic fever patients in Indonesia are reported from 1975 to 1978. All 4 dengue serotypes were endemic in Jakarta, but dengue 3 was the predominant virus isolated. This type was also the most frequently isolated virus from patients outside Jakarta and had the widest distribution in Indonesia. The sensitivity of the mosquito inoculation technique for isolation of dengue viruses is discussed.

Two years' investigation of epidemic hepatitis E virus transmission in West Kalimantan (Borneo), Indonesia.

Two years' follow-up investigation of a hepatitis E virus (HEV) outbreak in West Kalimantan, Indonesia in 1991 was carried out to investigate the epidemiology of epidemic HEV transmission and the persistence of the immunoglobulin G (IgG) antibody response. Sixty cases identified as anti-HEV IgG positive during the outbreak in 1991 were matched with 67 controls and examined, together with 318 members of their families. Overall, the prevalence of anti-HEV IgG among the 445 subjects (representing 127 households) was 59%. There was no significant difference in anti-HEV prevalence between cases (72%) and controls (61%). Loss of detectable anti-HEV IgG after 2 years was demonstrated in 17 of 60 subjects (28%) who were originally positive for anti-HEV in 1991. The mean number of anti-HEV positive subjects per household was 2.04. Cross-sectional prevalence of anti-HEV IgG increased significantly with age (P = 0.01). When communities were grouped into areas of low (< 40%), medium (40-59%) and high (> or = 60%) anti-HEV prevalence, use of river water for drinking and cooking (P < 0.001), personal washing (P < 0.0001), and human excreta disposal (P < 0.001) were associated with high prevalence communities. Conversely, boiling drinking water was negatively associated with increased prevalence (P = 0.02). Subnormal rainfall during the month (August) leading up to the 1991 outbreak (19 cm compared to the monthly mean of 209 cm in 1985-1993) may have contributed to favourable epidemic conditions.
