[Prophylaxis of recurrent multiple echinococcosis of the liver in children].

The article presents an experience with treatment of 47 children aged from 3 to 14 with multiple echinococcosis of the liver. The authors have made a comparative analysis of the antiparasitic remedies and of a wide spectrum bactericidal solution of Betadin. The results of microscopic, histological and electron microscopic investigations have shown Betadin to be highly effective and harmless for the intraoperative prophylactics of recurrent echinococcosis. It is expedient to use Betadin for multiple echinococcal lesion of the liver in children.

[The causes of incompetence of the duodenal stump and gastroenteric anastomosis and the means for its prevention in the surgery of duodenal ulcers].

Causes of the incompetence of stump sutures in resection of the stomach observed in 90 out of 3479 patients with chronic duodenal ulcers were analyzed. Most patients were found to have a combination of general and local factors. Among the local factors the leading one was appearance of a large inflammatory infiltration around the "active" ulcers. The endoscopic autohemoapplication to the ulcer included in the complex of preoperative preparation of the patients reduced the frequency of incompetence of sutures from 2.58% to 1.33%.

[The morphological changes in the lung after its dissection with a CO2 laser and a plasma scalpel in an experiment].

An advantage of the laser scalpel over the plasma scalpel was shown in experiments in animals (60 observation). On the 3rd-5th days after cutting with a laser the inflammation symptoms subside and on the 7th day there appear fine fibrous connective tissue since the operation with the plasma scalpel is followed by the formation of the fine fibrous connective tissue as late as on the 10th-14th days, the inflammatory reaction being observed up to the 30th day.

[Effects of low intensity infra-red laser irradiation on ultrastructure and proliferation of liver cells in experimental hepatitis and cirrhosis].

With the aid of light, electron transmission and scanning electron microscopy and radioautography and stereometry, the influence of low-intensive laser irradiation (LILI) (infrared) was studied in normal rat liver and in experimental cirrhosis and hepatitis. It was revealed that arsenide-gallium laser irradiation causes the change of intracellular structure. These changes show the intensification on their specific function manifest in an increase of relative volume of intracellular structures. The changes of microvessels show the activation of microcirculation. The elevation of the index of the labelled nuclei testify to increased proliferation. The similar influences of LILI on the liver ultrastructure and proliferation in hepatitis and cirrhosis are accompanied by the reduction of the pathological changes of the liver--the hepatocytes oedema, granular, vacuolar and fatty dystrophy.

[Gastric ulcers in liver cirrhosis and their exposure to low-intensity infrared laser radiation].

By means of light, scanning electron microscopy and planimetry, the pathomorphology of gastric ulcers (GU) was investigated in patients who had died from liver cirrhosis (LC) and in rats with experimental portal hypertension (PH). It was noticed that GU are frequent (13.3%) in patients with LC. Experimental GU induced in rats with PH persist longer and their areas are larger than in ulcers without cirrhosis. The treatment of ulcers through abdominal wall with low-intensive infrared laser irradiation (LILR) causes significant acceleration of their epithelialization in the rat without cirrhosis. Ulcers in the animals with cirrhosis do not repair under LILR influence, but their area significantly decreases after forming collateral shunting. The congestion in the portal system is the main pathogenetic element in gastroesophageal ulcerogenesis. The laser therapy without decreasing portal hypertension is not efficient.

[Interaction of indigenous parietal microorganisms with cells of the digestive tract mucosa].

By means of light, electron microscopy and stereomorphometry the interaction of microorganisms (MO) and cells of the mouth, stomach and gut mucous membrane was studied in different pathologic conditions on clinic and experimental material. No penetration was noted of MO into cells of the keratinized squamous epithelium, they were present in intercellular spaces. In the stomach, MO as a rule interact with mucocytes altering their surface. Spirillum-like MO sometimes penetrate into the parietal cells. In the small bowel MO may penetrate the cells causing lysis of apical membranes and photocytosis changing the microvilli structure. MO may enter the goblet cells during the secretion. They may settle in the cell cytoplasm without causing its alteration. In crypts, near Paneth cells, MO were subjected to lysis. The peculiarities of MO interaction with cells depend on their structural-functional status.

[Structural bases of the barrier-protective function of the stomach and small intestine].

The gastrointestinal tract acts as a barrier-protective tool in addition to its digestive and transport functions. The structural bases of the barrier protective function of the stomach and small intestine were shown in health, duodenal ulcer (DU), experimental gastroduodenal ulcers (GDU), vagotomy, and peritonitis in case of the interaction of these structures with parietal microflora (PM) which may be a valid criteria for assessing the barrier-protective function. PM increases in number in DU, GDU, vagotomy, and peritonitis. Various modifying interventions can normalize this parameter. Identifying the components of protective levels makes it possible to differentiate the affect of protective levels and makes it possible to differentiate affect some links of the barrier-protective function in order to restore it or prevent its disturbance.

[Routes of resorption of peritoneal fluid in the diaphragm in liver cirrhosis (morphologic study)].

The diaphragm peritoneum from 12 operated patients and 34 patients who died from liver cirrhosis with or without ascites was studied by means of light microscopy and electron transmission and scanning microscopy. Considerable lesions are found in the peritoneum: cuboidization of mesothelial cells, basal membrane thickening, dilation of stomata, lymphatic lacunae and collectors lumina. Liver cirrhosis with ascites is frequently followed by lymphatic vessels thrombosis and firm attachment of the diaphragm to the liver resulting in the inhibition of the ascitic liquid elimination. Thus both the enhancement of liquid transudation into the abdominal cavity and the disturbance of the drainage function of the diaphragm peritoneum take place.

[Changes in the peritoneum of the small intestine and diaphragm in experimental portal hypertension].

Diaphragm and small intestine peritoneum morphology was studied in experimental portal hypertension in rats with the help of luminescent, transmission and scanning electron microscopy techniques. Structural organizations of these peritoneum portions and performance function were different: fluid transudation realized through the small intestine peritoneum and resorption occurred via diaphragm peritoneum. Morphological signs allowed to judge about the increasing of fluid transudation in abdominal cavity and diaphragmatic resorption in early period of portal hypertension. Morphological alterations appeared in peritoneum resorption sites (pumping diaphragmatic hatchs) according to progress of portal hypertension that indicated decompensation process of peritoneal fluid absorption and led to ascites.

[Effect of copper vapor laser on the microrelief and ultrastructure of gastric mucosal glandulocytes].

It was investigated the influence of low intensive irradiation by the copper++ vapor laser (lambda-510.6 nm) on the glandulocytes of gastric mucosa of 28 white rats. The doses of endogastric irradiation were 6.78, 20.34 and 33.90 J/cm2. It has been shown that after irradiation of gastric mucosa with the copper++ vapor laser the microrelief and ultrastructure of glandulocytes changes testified to stimulation of specific secret function. This changes took place under irradiation doses from 6.78 to 20.34 J/cm2. The doses exceeded 20.34 J/cm2 caused the alterations of the epitheliocytes. Thus it is necessary to take into account that during laser therapy of the ulcers with copper++ vapor laser, doses of 20.34 J/cm2 caused the alterative effect on the epitheliocytes.

[Chief cells of gastric fundal glands in patients with duodenal ulcer before and after vagotomy].

Chief cells were studied ultrastructurally and stereometrically in 93 patients at various intervals (from 2 weeks up to 4 years) after complete vagotomy as well as in 87 patients at the same intervals after selective proximal vagotomy. The chief cell structure indicated their high level of secretory activity in patients with duodenal ulcer regardless of the type of initial secretion (hypo-, normo- or hypersecretion). Vagotomy, apart from the decrease of the chief cell volume in the glands results in the reduction of their intracellular structures. The secretory granule structure was changed indicating the disturbance of cell differentiation, more pronounced after selective proximal vagotomy.

[The role of reinnervation of the stomach in the development of recurrent ulcers].

The interrelationship of the state of the acid- and enzyme-forming function of the stomach with the alteration of its nerve system, parietal and main cells at different time after vagotomy was studied. The following reappearance of hypersecretion and the formation of a peptic ulcer of the anastomosis in the patients appears to be a results of reinnervation of the stomach.
