[Treatment of mild and moderate hypertension with enalapril (multicenter study of enap and enap N in Ukraine)].

AIM: Assessment of efficiency and safety of enalapril (enap) and its combination with hydrochlorotiaside (enap-N). MATERIALS AND METHODS: 127 patients with mild and moderate blood hypertension entered an open non-comparative multicenter trial. 60 of them received enap (group 1), 67--enap N (group 2). Group 1 patients were given enap for 2 weeks in a dose 10 mg/day. If this dose was not adequate to normalize blood pressure, it was raised to 20-40 mg/day. Patients of group 2 received enap-N one tablet a day for 3 weeks. If the pressure persisted higher than 140/90 mm Hg, the treatment was continued for 3 weeks more in a dose of 2 tablets a day. RESULTS: Blood pressure lowered under 140/90 mm Hg in 40 patients of group 1 (66.7%). Systolic pressure dropped by 10 mm Hg minimum and diastolic by 5 mm minimum in 18 group 1 patients (30%). Enap-N reduced blood pressure under 140-90 mm Hg in 44 of 67 patients (65.7%). Systolic and diastolic pressure dropped, respectively, in 23(34.3%) patients. CONCLUSION: Enap and enap-N tablets were found highly effective and well tolerated. Side effects were caused by lowering of blood pressure.

[An amino acid metabolic complex in optimizing the cardiac hemodynamic characteristics of patients with ischemic heart disease].

On the basis of findings from examination of 82 patients with ischemic heart disease presenting with exertional angina, functional class II-III, aged 62-86 years, efficacy was studied of an amino acid metabolic complex consisting of glutamic acid and methionine combined with basic antianginal drugs and single basic therapy with nitrates and calcium antagonists. Criteria for efficacy were condition of central cardiohemodynamics and inotropic indices. Glutamic acid and methionine combined with basic agents were found to make for enhancement of contractile function of ischemized myocardium, with the left ventricular local contractility being improved against the background of reduction in postload.

[Relationship between activity of atrial natriuretic peptide and process of myocardial remodelling in patients with cardiac failure].

Echocardioscopy in M- and B-modes with measurement of routine parameters of central hemodynamics and thickness of carotid intimal-medial segment as well as radioimmunoassay of plasma concentrations of atrial natriuretic peptide (ANUP) were performed in 159 patients aged 42-63 years (17 healthy subjects and 142 patients with cardiac failure associated with ischemic heart disease and sinus rhythm, without history of myocardial infarction). The results were indicative of possible use of ANUP as a marker of initial cardiac insufficiency and a corrector of the disease prognosis.

[Structural and functional parameters of the myocardium and atrial natriuretic peptide in atrial fibrillation].

The plasma level of atrial natriuretic factor and its relation to intracardiac and systemic hemodynamic parameters were examined in patients with coronary heart, rheumatic and mitral valvular diseases, neurocirculatory dystonia which were complicated by atrial fibrillation. The concentration of atrial natriuretic factor was found to vary with the left ventricular myocardial mass, left atrial contractility, antiarrhythmic therapy intensity and efficacy.

[Hypotensive effect of nifedipine in hypertension patients with different functional activity of the sympathetic-adrenal system].

Sympatho-adrenal activity (SAA) was assessed in 67 male patients with essential hypertension and 20 normal subjects. The patients were divided into 2 groups: those with increased (group 1) and normal (group 2) basal SAA. Hyperreactive and normally reactive patients were identified on the basis of plasma norepinephrine elevation in response to one-hour walking in both groups. Therapy with nifedipine alone for 3 weeks was ineffective in hyperreactive first-group patients. The efficiency of nifedipine treatment for essential hypertension may be dependent on baseline sympatho-adrenal activity and reactivity.

[Regional hemodynamic function in chronic obstructive bronchitis with arterial hypertension].

The state of regional hemodynamics (pulmonary, cerebral and peripheral) was studied in patients suffering from chronic obstructive bronchitis with symptomatic "pulmonogenic" hypertension. Pronounced disorders of regional hemodynamics accompanied by pulmonary hypertension, an increase in the tone of the cerebral and peripheral vessels and their lowered blood filling in patients with a stable phase of this type of hypertension, were established. Multimodality therapy including antihypertensive drugs (Cordanum, clonidine, hydrochlorothiazide, hydrolasin) significantly improved regional hemodynamics.

[Arterial hypertension in chronic bronchial obstruction and various problems of its treatment].

A total of 127 patients with bronchial asthma of infectious-allergic genesis and chronic obstructive bronchitis were examined for the central and regional hemodynamics as well as for the blood levels of serotonin and monoamine oxidase. The combination of chronic bronchial obstruction with stable "pulmogenic" arterial hypertension was associated with a significantly decreased cardiac index, a reduction in the pulse blood filling of the brain as well as an increase in the peripheral vascular resistance, the pressure in the pulmonary artery, serotonin levels and monoamine oxidase activity. A multiple modality therapy (hydralazin, talinolol, hydrochlorothiazide) elicited an improvement of the hemodynamic and normalization of the biochemical parameters.

[Pulmonary hemodynamics and external respiratory function in patients with chronic obstructive lung diseases. The diagnostic value of complex radiologic study].

A study made of the external respiration function and pulmonary hemodynamics in 43 patients with chronic obstructive pulmonary diseases by fluorography and tomography disclosed highly significant signs of impairement of the external respiration function and pulmonary hemodynamics at early stages in the course of these conditions' development. The above findings are confirmed by a series of non-invasive techniques of investigation, such as radiopulmonography, jugular phlebography, tetrapolar pulmonary rheoplethismography.
