High co-prevalence of genogroup 1 TT virus and human papillomavirus is associated with poor clinical outcome of laryngeal carcinoma.

BACKGROUND: The aetiology and factors leading to the progression of laryngeal cancer are still unclear. Although human papillomavirus (HPV) has been suggested to play a role, reports concerning the effect of HPV infection on tumour development are controversial. Recently, transfusion transmitted virus (TTV) was suggested to play a role in certain infections as a causative or coinfecting agent. AIMS: To investigate whether the development and progression of laryngeal squamous cell carcinoma is associated with coinfection with TTV and HPV. METHODS: The prevalence of TTV and HPV was investigated using the polymerase chain reaction in tissue samples from 40 healthy individuals, 10 patients with recurrent papillomatosis, five patients with papillomatosis with malignant transformation, and 25 patients with laryngeal carcinoma. The obtained prevalence data were compared and analysed statistically. RESULTS: In the 11 patients with carcinoma who had metastasis or relapse there was a high rate of coinfection with genogroup 1 TTV and HPV (eight of 11), whereas in the 14 without tumour progression no coinfection was found. Coinfection was associated with significantly lower tumour free survival in patients with carcinoma (p < 0.001). Furthermore, four of five patients who had papillomatosis with malignant transformation were coinfected with genogroup 1 TTV and HPV. CONCLUSIONS: Although the nature of cooperation between HPV and TTV needs to be investigated further, coinfection with genogroup 1 TTV and HPV appears to be associated with poor clinical outcome in laryngeal cancer.

3D reconstruction based on hard tissue microtome cross-section pictures in dentistry.

The aim of this study was to examine the accuracy of the computerized 3D surface analyzing and volume measuring method in dentistry. Two different types of test objects were used in the first part of the measurements. Each sample of the two groups was cross-sectioned using a hard tissue microtome. The sections were photographed on both sides and were projected on a graphical tablet and analyzed using a computer program. The measured and calculated parameters were compared. In the second part, 200 microm thick horizontal sections were prepared from 11 human incisor roots using the hard tissue microtome. This way, five sections were prepared from the apical 2 mm of each root. The effects of section thickness and number were modeled by decreasing the inclusion rate of the obtained number of sections from 10 to 2 and its influence on the calculated results was determined. This method was suitable for the approximation and analysis of 3D parameters. The results indicated that using 200-300 microm section thickness, the measured values were approximately 8-21% lower than the real parameters.

[Prevalence of the various types of periapical lesions and the significance of histologic evaluation].

To determine the prevalence of radicular cyst and chronic apical periodontitis among human periapical lesions 299 lesions were analysed during the past 6 years. The specimen were obtained during extraction or endodontic surgery. Of the lesions assessed, 195 (65.2%) were found histopathologically to be cystic, 96 (32.1%) were diagnosed as chronic apical periodontitis and 8 (2.7%) were observed as "other lesion". Studies to determine the diagnostic features and prevalence of these lesions have failed to reach a consensus view, though the treatment and the prognosis may differ to the lesion present.

Human herpesvirus 8 in hematologic diseases.

Human herpesvirus type 8 (HHV-8), also known as Kaposi's sarcoma-associated herpesvirus (KSHV) is a new member of the g-herpesvirus family. It is an unusual herpesvirus in that it carries a large number of genes that encode oncoproteins or cell signaling proteins. In addition to being the causative agent of both HIV-associated and non-HIV-associated Kaposi's sarcoma this DNA tumor virus has been implicated in the pathogenesis of several diseases. These include multiple myeloma (MM), Waldenstöm's macroglobulinemia (WM), multicentric Castleman's disease (MCD), body cavity-based lymphoma (BCBL), and various other conditions such as sarcoidosis and pemphigus. While the causative role of the viral infection is fairly certain in the development of BCBL and multicentric Castleman's disease, HHV-8 may act through a different mechanism to induce plasma cell malignancies. It has been suggested though the finding is still controversial - that infection of bone marrow stromal dendritic cells by HHV-8 might be a key factor in the etiology and pathogenesis of monoclonal gammopathies. The aim of this review is to provide a short introduction into the tumorigenic potential of HHV-8 as well as to detail the available data and possible mechanisms on the involvement of this virus in different hematologic diseases.

[Screening of patients referred to our clinic for odontogenic focal diseases].

During 1997 and 1998 261 patients suffering from suspected focal diseases were investigated for the presence of persisting chronic inflammatory disorders in the oral cavity. In 83% of the patients pulp necrosis, chronic inflammation of the apical area and the periodontal tissues could be revelated. The most commonly diagnosed disorders were pulp necrosis, chronic apical periodontitis and periodontitis complicata respectively. Most frequently these diseases could be detected in chronic cardiac disorders, uveitis and dermatological diseases. In the future follow-up studies parallel with the elimination of the dental disorders monitoring of blood and serum parameters will be conducted in order to obtain data of th systemic effect of the persisting chronic dental diseases.

[Analysis of activated cells in apical granuloma].

The ratio and in situ distribution of CD3+ T-lymphocytes, CD4+ and CD8+ subsets, CD14+ macrophages, CD56+ natural killer cells and CD25+ activated T-lymphocytes and CD68+ activated macrophages were determined in 20 chronic peripical granulomas by immunohistochemical method using monoclonal antibodies. CD3+ T-lymphocytes made up about 50% of the mononuclear cells. CD14+ macrophages were distributed all over the area but their proportion was much less that that of the T-lymphocytes. CD56+ natural killer cells made up a small proportion of the cells. The major proportion of the T-lymphocytes and macrophages were in activated stage within the lesion.

Quantitative significance of IgE-producing plasma cells and tissue distribution of mast cells in apical periodontitis.

A total of 30 dental periapical lesions were used for estimating the frequency of IgE-producing plasma cells by the unlabelled antibody peroxidase antiperoxidase method. Mast cells were detected in adjacent sections by the naphthol-AS-D-chloroacetate esterase technique. IgE-producing plasma cells were present in the central granulation tissue in 26 of the 30 specimens. The average ratio of IgE-containing plasma cells was 0.22% as related to all plasma cells. The central granulation tissue was devoid of mast cells, but numerous mast cells were found in the fibrous capsule of periapical lesions.

[The role of acute phase proteins in the pathogenesis of chronic periapical granuloma].

Alpha-1-antitrypsin, alpha-2-macroglobulin, C-reactive protein, haptoglobin and complement component C 3 concentrations and oxidase activity of ceruloplasmin were studied in sera of 36 patients with chronic periapical granuloma before and after surgical treatment. Mean serum concentrations of the investigated proteins were slightly elevated at diagnosis, with the exception of haptoglobin. Alpha-1-antitrypsin and ceruloplasmin levels decreased significantly 7 days after apicectomy, the four other proteins were unchanged at this sampling time. Each investigated protein decreased significantly 3 months after apicectomy. This is the first longitudinal study of "acute-phase proteins" in chronic periapical granuloma. The results suggest that inflammatory reactions contribute to the maintenance of chronic periapical lesion.

[Histologic study of periapical granulomas].

Sixty-three periapical granulomas were histologically examined. They were divided into four types: exudative, granulomatous, granulofibrous and fibrous types. The most frequent forms were the granulomatous types. The granulomatous type consisted of four zones: necrotic, exudative, granulomatous and fibrous. In the granulomatous zone lymphocytes, plasma cells, macrophages, Russel bodies, foam cells, neutrophil leukocytes were found. Mast cells were found in the fibrous zone among the collagen fibres.

[Whole blood, chemiluminescence in patients with periapical granuloma].

Spontaneous and mannozyme-induced whole blood chemiluminescence was studied in 14 patients with chronic periapical granuloma. Investigations were performed before surgery and 7 days as well as 3 months after apicectomy. Spontaneous chemiluminescence was significantly higher in patients versus normal controls. The elevated spontaneous chemiluminescence decreased significantly after surgery. Results indicate that, in these patients, granulocytes are in a metabolically and functionally activated state in vivo.

[Congenital hypophosphatasia as a possible cause of early loss of deciduous teeth].

The suspicion of congenital hypophosphatasia is frequently raised byte premature falling out of milk teeth. The diagnosis is corrobarated by the characteristic bone deformities, by the high selection of phosphoetanomalin and by the diminished alkaline phosphataze enzyme activity of the serum. The case of a four years old boy patient is reported on.
