Osteoarticular tuberculosis of the symphysis pubis presenting as a hypogastric cystic mass in a woman with primary Sjögren's syndrome.

A 79-year-old woman with primary Sjögren's syndrome and immune thrombocytopenia presented with a hypogastric cystic mass. Twenty-three months previously she received a 7-month course of prednisone at moderate doses to control the thrombocytopenia. A computed tomography showed a cystic mass, destruction of the left pubis with involvement of the symphysis and erosion of the right pubis. The cystic mass was surgically removed. Epithelioid granulomata were found on pathological examination and Mycobacterium tuberculosis was grown in Löwenstein medium. This is the first reported case of osteoarticular tuberculosis with such a presentation and only the second localized to the pubis.

The evolutionary history of Drosophila buzzatii. XXIII. High content of nonsatellite repetitive DNA in D. buzzatii and in its sibling D. koepferae.

The frequency and types of repetitive nonsatellite DNA of two sibling species of the repleta group of Drosophila, D. buzzatii, and D. koepferae have been determined. For each species, the analysis is based on a sample of more than 100 clones (400 kb) obtained from genomic DNA. A theoretical model has been developed to correct for the presence of a mixture of repetitive and unique DNA in these clones. After correction, a high content of repetitive DNA has been demonstrated for both species (D. buzzatii, 19-26%; D. koepferae, 27-32%). The repetitive sequences have been classified according to their hybridization pattern when used as probes against genomic DNA and by their in situ hybridization signals on polytene chromosomes. Data suggest that the main nonsatellite component of these species is simpler and more repetitive than that of D. melanogaster, pointing to a wide variability in content and class size distribution of repetitive DNA among Drosophila species.

Sexual isolation in Drosophila. I. Theoretical models for multiple-choice experiments.

Several simple models are developed to calculate expected mating frequencies in ethological isolation experiments. They take into account the effect that the peculiar sexual behavior of Drosophila species can have in multiple-choice experiments. These models depend on only three basic parameters: male competitive ability (C), female receptivity (R) and the coefficient of females acceptance (A). Two types of model can be distinguished: (1) models with discrete preferences, in which A is a measure of the percentage of females accepting a particular kind of male and (2) models with continuous preferences, in which A represents the probability of acceptance for each courtship. It is demonstrated that the information rendered by just one experiment, although effective for determining whether sexual isolation exists, it is insufficient to estimate its degree or to demonstrate that it is asymmetrical. Further developments of the models under more complex conditions as well as their implications for reinforcement and founder effect theories are discussed.

[Postoperative pain after thoracotomy. A study of 116 patients].

We have studied post-operative pain in 116 patients who underwent a thoracotomy. The pains were assessed using a visual analogue scale and were significant and identical whatever type of operation was used and irrespective of sex or diagnostic disease category. On the operative day only the surgeon seemed to have any influence. On the first post-operative day the pain was influenced by age and on the eighth day by socio-professional category. The insertion of drains had no influence on the pain. Massage and physiotherapy decreased the pain in a significant fashion. The importance of taking account of post-operative pain is underlined.

Chromosome location of a family of genes encoding different acidic ribosomal proteins in Saccharomyces cerevisiae.

DNA probes from the genes encoding the acidic ribosomal proteins L44, L44' and L45, as well as from reporter genes for chromosomes IV, VII, XII and XV, have been hybridised to Southern blots of Saccharomyces cerevisiae DNA resolved by pulsed field gel electrophoresis. The protein L44' and protein L45 genes have been found to hybridise to chromosome IV, identified by the CAT1 gene probe, while the protein L44 probe hybridises with a band containing chromosomes VII and XV, identified by the ATPase 1 and HIS3 genes respectively.

[Palliative treatment of patients in terminal phase. Experience at the Unité de Soins Palliatifs of the Hôpital International de l'Université de Paris].

The development of palliative care is relatively new in France. This study analyzed our observations made in the Palliative Care unit of the Hopital International de l'Université de Paris. The most common complaints were: being bedridden (38%), pain (23%), dyspnea (10%), intestinal irregularities (10%). The importance of being bedridden and the complexity of its handling are emphasized. Dyspnea is common when sought (54%) and hard to alleviate due to a lack of appropriate symptomatic therapies. Reduced survival rates were found for patients with Karnovsky indexes below 30% and taking morphine. Length of survival varied as a function of awareness of the diagnosis and family support.

Towards a physical map of the Listeria chromosome: the pulsed field electrophoresis approach.

The possibility of the electrophoretic study of very large fragments of the Listeria chromosome (DNAs up to 1000 Kb) may help to the understanding of the physical organization and functional mapping of the entire genome of this organism. Several experiences were done to apply this technology to Listeria. Agarose inserts were prepared with intact cells and protoplasts, and lysis was induced in situ. Inserts with a convenient amount of DNA were cleaved in situ again by diffusing restriction enzymes into the agarose and submitted to one dimensional electric field that was periodically inverted, to cause changes of direction of the DNA fragments. We obtained a good band resolution of bands with Eco R1 and HindIII. Experiments are in progress to select other restriction enzymes leading to larger fragments. This technique must be combined with the use of blotting with known DNA sequences, such as the corresponding to the haemolysin, to arrive to a comprehensive map of the Listeria chromosome. Cloning of Listeria genes involved in several biochemical characteristics of Listeria, or at least a good collection of mutants will be quite necessary for the progress of such approach.

Chromosome Mapping

[Peritoneal sarcoidosis, a rare localization].

A case of peritoneal involvement in the course of a mediastino-pulmonary and cutaneous sarcoidosis is reported. This form of the disease is rare (8 cases in the literature) and must be distinguished from peritoneal granulomatous reaction to surgical foreign bodies (e.g. talcum powder) or to particles of starch in food. In the case presented here, the patient's surgical history and the presence of birefringent bodies within peritoneal granulomas raise the problem of the link between local granuloma and sarcoidosis.

[Bronchopulmonary cancers: medical approach of the last weeks of life. Study of 191 patients].

The characteristics of patients with carcinoma of the lung during their last weeks of life were reviewed in 191 patients treated in the same department and who died between September 1982 and January 1985. The main symptoms were breathlessness (112/191) which posed difficult therapeutic problems preventing domiciliary treatment, and pain (87/191) which was more easily managed in the author's experience. The modes of death were also analysed, showing a number of sudden deaths (14). Therapeutic sedatives were the immediate cause of death in 32 cases, in the first part of the study. The authors make a rapid review of the different available palliative measures. This type of study is the only way of changing medical practices and strategies in patients dying in hospital.

[Mediastinal carcinomas in young men without primary tumors. A chemosensitive entity?].

Three patients out of 99 benefited with a complete remission following chemotherapy with Cisplatinum and Vindesine. The initial characteristics of these patients were studied, they recall a characteristic type of tumour already described and for which a hypothesis of a germ origin has been raised. In spite of their rarity the existence of complete remissions should encourage the use of chemotherapy in such patients who have a "broncho-pulmonary" cancer with a tropism for the mediastinum and without a specified primary tumour.

Adenocarcinoma, Mucinous

[Inoperable non-small cell bronchopulmonary cancers. Results of ambulatory polychemotherapy including cisplatin].

Between October, 1981 and October, 1983, 57 subjects with inoperable non small-cell carcinoma of the lung (localized in 37, metastatic in 20) were treated as out-patients, after oral rehydration, with a multiple chemotherapy regimen which included Cisplatinum, Doxorubicin, Vincristine and Lomustine. No severe haematological or renal reaction was observed. After 2 courses 23 patients were responders (complete, or partial, or minor response), 2 of them with complete response. Patients with localized tumour had complementary radiotherapy, while chemotherapy was pursued in responsive patients with metastasis. Median survival was 9 months in responders and 5 months in non-responders. Thus, chemotherapy including Cisplatinum in medium doses (80 ms/sq.m) can be given to out-patients without risk of renal damage. The results obtained in this study are comparable to those of studies performed with other drug combinations including Cisplatinum.
