A method for quantitating the nuclear area of gastric polyps using image analysis.

With the aid of an image quantifier, the nuclear area in Feulgen-stained sections from 29 gastric adenomas and 8 hyperplastic gastric polyps were recorded. In each specimen, the nuclear area and the total mucosal area were measured in three consecutive fields of (1) the lesion and (2) its adjacent normal gastric mucosa. The results, expressed as the percentage of the nuclear area to the total mucosal area, showed significantly higher (P less than .001) values in gastric adenomas than in hyperplastic gastric polyps. The highest mean of the difference between the nuclear values was found in gastric adenomas with invasive growth. The method reported herein appears to be useful for making the differential diagnosis between neoplastic and nonneoplastic polyps arising in the gastric mucosa.

Adenoma

[Allergy to mercurothiolate in an infant during heparinization of an intracaval catheter].

A 4-month old infant developed an immediate and proven systemic allergic reaction to mercurothiolate. The acute accident occurred while an intracaval catheter was being treated with a dry-frozen heparin which excipient contains mercurothiolate. This conservative agent is present in numerous pharmaceutical preparations for topical and systemic use.

Ulcerative colitis in protracted remission. A quantitative scanning electron microscopic study.

Colonoscopic biopsies from 32 patients were studied at the ultrastructural level using a scanning electron microscope (SEM). Sixteen of the 32 patients had a previous diagnosis of total ulcerative colitis (UCR greater than 10 years) in protracted remission. The colonic mucosa was normal at endoscopic and histologic examinations (UCRN). The remaining 16 patients had normal colonic mucosa, but had an adenoma or an adenocarcinoma elsewhere in the colon. Several ultrastructural parameters were investigated, such as the number of crypts per area, the distance between the crypts, the outline of mucosal units, the number of mucous cells, the outline of absorptive cells, and the number of villi per area. Quantitative determinations of SEM structures (including measurements with an interactive digital image analyzer system; MOP 30, Zeiss Contron) were made. The results showed no significant differences between the various parameters (except for the number of crypts per area) between patients with UCRN and controls. The possibility of a total (or quasi-total) restitutio ad integrum of the colonic mucosa in certain patients with UCR is discussed. An international policy regarding the colonoscopic surveillance of patients with UCRN should be elaborated. It is suggested that the time interval between control colonoscopic biopsies in patients with UCRN should be increased substantially.

Adenocarcinoma

A method for the quantitation of intestinal metaplasia of the stomach by morphometry.

A novel method to quantitate the extent of intestinal metaplasia in gastrectomy specimens is presented. Morphometric measurements of histochemically labeled mucin-producing goblet cells were done in three selected gastrectomies (one having a peptic ulcer, one with adenocarcinoma of the intestinal type, and one with adenocarcinoma of the diffuse type). The sectioning of the gastrectomy specimens was standardized. The results indicated that the intestinal metaplasia was significantly higher in the specimen with adenocarcinoma of the intestinal type (as compared to the other two specimens), both along the lesser and greater curvatures as well as in the fundic area. These quantitative results confirm nonquantitative reports based on subjective visual impressions. This quantitative histochemical method for measuring the actual length as well as the topographical distribution of intestinal metaplasia in resected stomachs will be used in future studies of intestinal metaplasia with the aim of disclosing possible differences among populations with disparate incidences of gastric carcinoma.

Adenocarcinoma

[Stress and immunity in cattle].

Stress of variable length may be caused in calf by parenteral administration of supercortisol and application of acute thermal load. Hormonal or thermal stress may have negative impact on postvaccinal immunity to paratyphoid (Salmonella dublin 81), as may be interpreted with reference to the O-agglutination titre of blood serum. The extent to which immunity is suppressed by stress was found to depend on the moment of stress application, duration of stress, and age of the calf.

Adrenocorticotropic Hormone

[Early stimulation: 10 years' experience].

Infancy psychomotor yield in lower socioeconomic levels can be damaged from the first year of life. This implies future consequences to individuals, family-and society. Early stimulation programs have demonstrated to improve psychomotor yield in the above mentioned children. In Temuco, Chile, in groups of children with different risks, that were incorporated to an early stimulation program, it has been seen that the lower the birth weight the greater the risk of future psychomotor deterioration, which is also true for children that survived neonatal meningitis. On the other hand, in children that presented hyperbilirubinemia, low Apgar scores and neonatal septicemia, no significant statistical differences in psychomotor development have been observed in comparison with those of similar birthweight but without these antecedents. In this study the Chilean Health Ministry's Evaluation and Psychomotor Stimulation Program is reviewed as to adapt it for children at greatest risk.
