Diagnostic revolution of microhematuria by real time confocal scanning laser microscope: Hyodo-Iino-Miyagawa method, third report.

The real time confocal scanning laser microscope provides excellent three-dimensional images free of out-of-focus information. The objective of this study was to evaluate the usefulness of the laser microscope for the diagnosis of microhematuria. Characteristics of the test were evaluated in 81 patients with definite causes of hematuria. 30 erythrocytes in urinary sediments were examined for each patient, and those in whom less than 20% of the erythrocytes were poikilocytes were considered to have urological diseases and those in whom 80% or more of the erythrocytes were poikilocytes to have nephritis. According to these criteria, the sensitivity and the specificity of the examination to nephritis were 100 and 98.1%. 91.4% of the patients with urological disease had the nonglomerular type. The time required for the examination was less than 2-5 min in samples containing 1-3 erythrocytes in one field under an ordinary light microscope (x400).

Effects of smoking on testicular function, semen quality and sperm fertilizing capacity.

PURPOSE: The effects of smoking on testicular function and sperm physiology were studied. MATERIALS AND METHODS: Left testicular biopsy was performed in 49 smokers and 28 nonsmokers. Seminal specimens from these men were analyzed. RESULTS: Testosterone levels in the left testicular vein, left testicular androgen-binding protein secretion rate (in vitro), sperm motility, percentage of morphologically normal spermatozoa, sperm morphometric parameters and outcome of sperm function tests were significantly lower (p < 0.05) in smokers than in nonsmokers. CONCLUSIONS: Morphological sperm abnormalities due to secretory dysfunction of the Leydig and Sertoli cells may be the cause of impaired sperm fertilizing capacity in smokers.

Phase II study of intravesical chemoprophylaxis of epirubicin after transurethral resection of bladder tumors. Tottori University Oncology Group.

We performed intravesical instillation of epirubicin in 53 patients with the aim of preventing recurrence of bladder tumors. The patients had undergone transurethral resection of superficial bladder tumors (only transitional-cell carcinoma) within the preceding week, between January of 1990 and July of 1991. Recurrence was found in 11.3% (6/53) of cases during follow-up periods lasting from 1 to 20 months. Side effects occurred in 5.7% (3/53) of the patients. The cumulative nonrecurrence rate was 96.0% for a follow-up period of 6 months and 78.4% for 12 months. The results of this study suggest that intravesical instillation of epirubicin is very useful as adjuvant therapy after transurethral resection of superficial bladder tumors.

Reproductive capacity of the nucleus of the male gamete after completion of meiosis.

PURPOSE: Our purpose was to investigate the possibility of achieving fertilization and subsequent normal embryonic development by injecting round spermatid nuclei into rabbit oocytes. RESULTS: Two-to four-cell-stage embryos developed after round spermatid nuclear injections into rabbit ooplasma could further develop in vitro up to the expanding blastocyst stage or in vivo up to complete gestation. CONCLUSION: The current findings show that the haploid set of chromosomes of round spermatid can pair with the chromosomes of the ootid to participate in complete fertilization and subsequent embryonic and fetal development. In addition, we suggest that postmeiotic modifications of the round spermatid are not required for the pairing of male gamete chromosomes with those of the ootid.

Bilateral effect of unilateral vasectomy on testicular testosterone biosynthesis.

Twelve immature male dogs underwent a left vasectomy (group A). An additional five underwent a sham operation (group B). Sixteen weeks after the surgery, the bilateral mean values for caudal epididymal sperm content, the percentage of motile spermatozoa, intratesticular testosterone concentration, and testicular secretion of androgen-binding protein (in vitro) were significantly lower in group A. The mean peripheral serum testosterone responses 3 hours after human chorionic gonadotropin stimulation (3,000 IU) were significantly lower in group A than in group B (6.3 ng/mL v 9.5 ng/mL). These findings indicate a bilateral deficiency in both Leydig and Sertoli cell secretory function in unilaterally vasectomized dogs, resulting in impaired bilateral spermatogenesis and sperm maturation. The authors suggest that unilateral injuries of the vas deferens during hernia operations in children may result in bilateral testicular dysfunction.

Differences in seminal parameters in specimens collected via intercourse and incomplete intercourse (coitus interruptus)

Whatever the mechanism of such improvements may be, the results in this study point out that coitus interruptus in the human may not be the method of choice for collection of semen specimens, especially in patients with spermatogenic dysfunctions such as hypospermia, oligospermia, and asthenospermia. It should also be noted at this point that, for whatever purpose (semen analysis or artificial insemination by husband), the collected specimen should as closely as possible resemble the ejaculate delivered during intercourse. The complete coitus method, as applied in this study, showed that completion of the ejaculatory process during intercourse as compared with the coitus interruptus method, may assist in the improvement of the collected specimen and should closely resemble the ejaculate obtained during intercourse without the use of Silastic condoms. Furthermore, on the basis of the results generated in this study, the complete coitus method should always be the method of choice for male infertility patients with ejaculatory and spermatogenic dysfunctions as well as for scientists and clinicians who deal in the field of infertility diagnosis and treatment.

Confocal scanning laser microscopy of morphometric human sperm parameters: correlation with acrosin profiles and fertilizing capacity.

OBJECTIVES: To develop quantitative criteria for assessing sperm morphology and to determine the correlation between the percentage of morphologically normal spermatozoa and the outcome of the sperm hypo-osmotic swelling test, sperm acrosin profile, and sperm capacity for fertilization. DESIGN: The maximal length and width of the sperm head, the length of the midpiece and principal piece of the sperm tail, and the ratio of the surface of the acrosomal region to the total surface of the head were determined in specimens obtained from a group of infertile men and a group of fertile men using a confocal scanning laser microscope. Group A consisted of 53 infertile men who were participating in an IVF program, and group B consisted of 98 fertile men. The mean +/- 2 SD of the morphometric parameters in group B was established as representing the lowest and highest normal values in both groups. A normal spermatozoon was defined as one with morphometric parameters within normal levels. The lowest percentage of morphologically normal spermatozoa, hypo-osmotic swelling test result, and acrosin activity in group B were also taken as the lowest normal values in group A. SETTING: In vitro fertilization program at the Tottori University School of Medicine, Yonago, Japan. MAIN OUTCOME MEASURES: Sperm morphometric parameters, percentage of morphologically normal spermatozoa, hypo-osmotic swelling test, and acrosin activity. RESULTS: The length of the midpiece, ratio (x 100) of the surface of the acrosomal region to the total surface of the sperm head, percentage of morphologically normal spermatozoa, outcome of hypo-osmotic swelling test, and acrosin activity were significantly higher in group B than in group A. The maximal width of the head was significantly lower in group B than in group A. Strongly positive correlations were observed between percentage of morphologically normal spermatozoa or length of midpiece and the proportion of fertilized oocytes in group A and between ratio (x 100) of the surface of the acrosomal region to the total surface of the head and acrosin activity in groups A and B. Sperm morphology showed high positive and negative predictive values for acrosin activity (normal/abnormal) and fertility potential (present/absent). CONCLUSIONS: Using quantitative strict criteria, we found that sperm morphology was an important predictor of sperm fertilizing capacity. The confocal scanning laser microscope provided useful information about the sperm cytoskeleton and its importance in fertilization.

Evaluation of systemic chemotherapy with methotrexate, vinblastine, adriamycin, and cisplatin for advanced bladder cancer. The Japanese Urological Cancer Research Group for Adriamycin.

In a cooperative study of the Japanese Urological Cancer Research Group for Adriamycin, the usefulness of chemotherapy with methotrexate, vinblastine, Adriamycin, and cisplatin (M-VAC therapy) in treating advanced or recurrent bladder cancer was examined. Evaluation of the clinical responses obtained in 86 evaluable patients revealed 13 complete responses, 29 partial responses, 4 minor responses, 19 cases of no change, and 21 cases of progressive disease. The overall response rate was 48.8% (42/86). The rate of response to M-VAC therapy at each disease site was as low as 21.4% (3/14) in bone lesions but exceeded 40% in the primary lesion, the lymph nodes, the lung, the liver, and other lesions. The clinical response to M-VAC therapy was not significantly influenced by the performance status of the patients, the dose intensity, or previous therapy. The median duration of response for the 42 responders was 22.7 weeks (range, 8.1-134.1 weeks), and the median duration of survival for the 86 evaluable patients was 9.8 months. Side effects were frequently encountered; the patients experienced anorexia, nausea, vomiting, malaise, alopecia, and leukopenia, but all of these symptoms were tolerable.

The alterations of norepinephrine and acetylcholine concentrations in immature rat urinary bladder caused by streptozotocin-induced diabetes.

The concentrations of norepinephrine (NE) and acetylcholine (ACh) in the body and base of the bladder were investigated two, four and eight weeks after the administration of streptozotocin to four-week-old rats. At four and eight weeks after following dosage, NE concentrations in the base of the bladder of the diabetic rats were significantly higher than those of control rats. At the two-week diabetic condition, ACh concentrations in both the body and base of the bladder diabetic rats were about two-fold higher than in the respective control levels and remained constant for eight weeks. However, the control rats showed dramatic increase in ACh concentration from two to four weeks after dosage. There was no significant difference in the response of bladder muscle strips to phenylephrine, isoproterenol and ACh in the diabetic vs. control rats throughout the experiment. These observations suggest that the alteration of autonomic nervous system may occur at an early stage of diabetes mellitus in immature rats and that the high concentration of NE in the diabetic bladder base may reflect an adaptive overfunction of the adrenergic system probably to maintain continence. Furthermore, the lack of change in ACh concentration with age in diabetic rats may suggest an impaired development of the cholinergic system.

The role of the seminal vesicles and coagulating glands in fertilization in the rat.

To investigate the role of the seminal vesicles (SV) and coagulating glands (CG) in fertilization in the rat, partial resections of the SV (25%--group B), 50%--group C, and 75%--group D) or bilateral resection of the SV alone (100%--group E), or together with the CG (group F) were undertaken. In other groups, bilateral resection of the CG only (group G), or bilateral resection of the CG with ipsilateral SV resection (group H) were performed. A group of sham-operated rats served as controls (group A). There were no significant differences among groups A, B, C and D in the number of fertile rats post-operatively but significant differences were evident in groups E, F, G, H when compared with the control group (P less than 0.01 for all groups). Only the fertile rats were able to form copulatory plugs (CP) post-operatively. Bilateral ligation of the SV ducts was performed in ten fertile rats. Post-operatively, the proportion of fertile rats was decreased significantly (P less than 0.01). Removal of the ligatures resulted in a significant increase (P less than 0.01) in fertility. It was also demonstrated that the addition of rat seminal vesicular secretions (SVS) to epididymal sperm suspensions resulted in a significant decrease (P less than 0.001) in sperm motility. These results suggest that the SV and CG are necessary for fertility in rats, and that one role of SVS is the formation of a CP and not the maintenance of sperm motility.

Surgical repair versus medical treatment of varicocele in the rat: pharmacological manipulation of the varicocelized testicle.

Human chorionic gonadotropin, kallikrein, indomethacin, and hydralazine were administered to different groups of varicocelized rats, while surgical repair of the varicocele was performed in another group of rats. The effects of conservative and surgical treatment on epididymal sperm content and motility, the weights of the testes, epididymis, and male accessory genital glands, and fertility were compared between each group and a sham-treated group of rats. Surgical repair significantly improved all the evaluated parameters and all the conservative regimens, except hydralazine, resulted in a significant improvement in most parameters. Our results indicate that stimulation of the Leydig or/and Sertoli cells of a varicocelized testicle can counteract some of the detrimental consequences of the varicocele itself.

Acrosin profiles of human spermatozoa recovered from the new Sperm Prep II filtration column.

Twenty semen samples were collected and two aliquots of 1.5 ml were prepared from each sample. One sample of each pair (fraction-one) was used for evaluation of semen parameters and total acrosin activity of spermatozoa. The other sample of each pair (fraction-two) was mixed with an equal volume of Ham's F-10 medium and filtered through the Sperm Prep II. At the end of the filtration, the filtrate was pooled, sperm parameters were evaluated and total acrosin activity was assessed. Percentage of normal spermatozoa, sperm motility, and total acrosin activity of spermatozoa were significantly higher in the postfiltered fraction-two than in the fresh fraction-one. Considering the great importance of acrosin for fertilization, it is suggested that Sperm Prep II filtration method may be beneficial in preparing spermatozoa from infertile men with low acrosin profiles for assisted reproduction programs. Further studies will be necessary, however, in order to confirm this, since the present study concerned men with known fertility.

[Prostate cancer after subcapsular prostatectomy diagnosed as benign prostate hypertrophy--clinico-pathological analysis].

Of 160 newly diagnosed cases of prostate cancer during last 11 years, six (3.75%) had a prior subcapsular prostatectomy. Digital rectal examination in these six cases revealed a significant prostatic abnormality and multiple bone metastases were showed. Histological examination by step-section technique was done retrospectively using surgical materials from subcapsular prostatectomy. Two cases of incidental carcinoma were detected histologically. One showed stage A1 and another stage A2. Continuous observation should be performed after prostatectomy, even if the surgical specimens revealed no carcinoma.

Inhibition by a new bisphosphonate (AHBuBP) of bone resorption induced by the MBT-2 tumor of mice.

A new bisphosphonate, 4-amino-1-hydroxybuthylidene-1,1-bisphosphonate (AHBuBP), was compared with 3-amino-1-hydroxypropylidene-1,1-bisphosphonate (AHPrBP) and 1-hydroxyethylidene-1,1-bisphosphonate (HEBP) in terms of its effect on tumor-induced osteolysis using a bladder tumor in mice (MBT-2). Tumor cells were inoculated subcutaneously (SC) over the calvaria in mice, resulting in a local tumor causing fragmentation of the bone. The tumor-induced osteolysis associated with osteoclasts proliferation was accompanied with reactive new bone formation. This osteolysis was evaluated by measuring the increased area of bone resorption in reduced opacity to radiograph and histologic study. The results showed the following sequence of potency: AHBuBP greater than AHPrBP = HEBP. This inhibition was obtained with no apparent effect on the growth of the MBT-2 tumor. The authors conclude that AHBuBP appears to be an interesting new bisphosphonate with possible clinical application.
