[Devices for saliva collection from the major salivary glands. Results in normal subjects].

BACKGROUND: Collection of saliva produced by the major salivary glands may be accomplished either by cannulation of the glandular ducts or by the application of specific collecting devices to the emergence area of the glandular ducts. Those procedures are complex, slow, invasive and require skilled personnel. AIM: To report the design and application of a device to collect parotid saliva (snail collector) and another device to collect saliva from the submandibular/sublingual complex. MATERIAL AND METHODS: The saliva collection devices were tested in 40 healthy volunteers (20 male) aged 18 to 22 years old. Saliva was collected using conventional conditions, during 5 to 15 min. RESULTS: An average of 1 to 1.5 ml of saliva was collected in the 10-15 min period from both parotid and submandibular/sublingual glands. Flow rates from parotid glands were 80 microliters/min and 180 microliters/min from submandibular/sublingual glands. Parotid saliva had a protein and organic material concentration twice as high than saliva from submandibular/sublingual glands. The presence of human alpha-amylase duplet (Mr 55 kD and 58 kD) predominated in parotid saliva, whereas saliva from submandibular/sublingual glands had other molecular markers such as the lysozyme duplet (Mr 18.5 kD and 17 kD). CONCLUSIONS: The tested devices were easily applicable, comfortable and allowed the collection of both parotid saliva and submandibular/sublingual saliva from various subjects at once, under the supervision of a single professional.

[Polydactyly: a genetic epidemiological study in Santiago, Chile].

The aim of this work was to study the prevalence at birth and family aggregation of polydactyly in Chile. We studied 125,652 newborns between 1969 and 1991. The prevalence was 1,329 for each 1,000 live newborns and higher in males than in females. Familial recurrence was 22.5% and gene penetrance was estimated as 0.5 for the postaxial-A type and 0.3 for the postaxial-B type. Gene penetrance for postaxial-B type was higher in males. Estimation of gene frequencies and mutation rates gave the highest values for postaxial-B polydactyly.

[The effect of environmental factors in inflammatory bowel disease].

In an attempt to elicit risk factors in inflammatory bowel disease in Spain, we have carried out a case-control study in which we conducted personal interviews asking marital status, place of residence, economic status, use of tobacco and contraceptives, and the method of lactation in infancy. IBD was more common in patients with a low economic level; UC was predominantly found in rural population. No differences were found in the remaining categories. Our results differ from those reported from North and Central Europe.

[Tropical sprue].

We report the case of a 60 year old female who presented with diarrhoea and malabsorption syndrome, confirmed by the laboratory data and the radiologic examination, which developed after a journey to Egypt. The mainly finding was flattening of the villi in the jejunal biopsy; the syndrome recovered after treatment with antibiotics and folic acid. The case fulfills the diagnostic criteria of tropical sprue although this country is not considered as an endemic area.

[Epidemiologic study of factors associated with hypospadias].

From August 60 to December 90, 103 male newborns with hypospadias were diagnosed among 124 588 consecutive newborns examined (8.3 per 10,000). Mortality among them was 1.94%. The annual incidence rate increased significantly over the study period (p < 0.001. Hypospadias was an isolated finding in 92% of cases, and its was associated to other non genital malformation in 8.7%. Location of hypospadias was distal in 73% and proximal in 15.5%. There was no seasonal variation in the incidence rate. Weight of affected individuals did not differ from that of controls. Older age of parents among affected individuals was not statistically significant.

Measurement of parathyrin in blood from thyroid veins: two radioimmunoassays compared in patients with primary hyperparathyroidism.

We measured parathyrin (PTH) in peripheral venous blood samples and in thyroid veins (both homolateral and contralateral to the lesion) in 13 patients with surgically confirmed parathyroid adenomas. Two different RIAs were used, one specific to the mid-region of the molecule (44-68, M-PTH), the other specific to the carboxy-terminal region (65-84, C-PTH). With the M-PTH assay we established a statistically significant multiple correlation (P less than 0.05) between the PTH concentrations in blood from the peripheral and thyroid veins; no significant correlation was found when we used the C-PTH assay. Our results confirm the superiority of the M-PTH RIA over the C-PTH RIA for study of hormonal secretion in primary hyperparathyroidism.

[Cysts of the parathyroid glands].

Cysts of epithelial bodies are rare causes of hyperparathyroidism. Reported in this paper are 2 patients with clinical symptoms of hyperparathyroidism, high blood calcium levels and established hormones in their epithelial bodies. Cysts of epithelial bodies were diagnosed by means of angiography, computed tomography, and sonography. Various aetiological factors behind cyst formation in epithelial bodies are discussed.

Adenoma

Study of 5-hydroxytryptamine (serotonin) in pericardial fluid in different causes of death (II.). Experimental study of 5-HT levels in two types of shocks (hemorrhagic and septic) in dogs.

In the present work we studied the levels of 5-HT in the pericardial fluid of 160 cadavers according to cause of death; such as myocardial infarction, violent asphyxia, pulmonary embolism, infections, bronchopulmonary diseases, traumatic and hemorrhagic diseases in the CNS, and multiple traumatism. We did not find significant differences in the various causes of death. A complementary study of 20 dead dogs having suffered induced shock, either hemorrhagic or septic, was made. In both series we studied the serum levels of 5-HT and the following parameters: systolic and diastolic arterial pressures, central venous pressure, hematocrit value, pH value, total proteins, albumin, PO2, and PCO2. We found the significant correlation (r = 0.828, P less than 0.01) only between 5-HT serum levels and the systolic arterial pressure in the hemorrhagic shock.
