[Endoscopic treatment of early gastric cancer using laser destruction and electrosurgical methods].

For the last 15 years endoscopic treatment of early gastric cancer we used in 52 patients. It involved: electroexcision, laser photocoagulation with argon and YAG-Nd lasers. The recurrences, that occurred in 6 patients were also treated with endoscopic methods. The long-term follow-up ranged from 3 months to 15 years. The combined endoscopic treatment of early gastric cancer is recommended in patients with contraindications for the conventional surgery.

[Differential diagnosis and treatment of benign tumors of the mesentery].

The authors possess experience in surgical treatment of 12 cases. Benign tumor of the mesenterium is among diseases which are recognized with great difficulty in the early and preoperative period. This is mainly due to the vague signs of the disease and the limited possibilities of applying auxiliary methods of examination because of the specificity of the organ in which the neoplasm is situated. To make the correct diagnosis in the early stages of the disease it is necessary in this case to conduct differential diagnosis with other diseases of the abdominal organs. The most typical clinical signs characteristic only of a benign tumor of the mesenterium can be distinguished as a result. When used in a complex with other methods of examination, differential diagnosis makes it possible to increase the number of correct preoperative diagnoses.

[Peritonitis after planned abdominal operations].

The authors have analyzed results of the operative treatment of 1146 patients mainly with malignant tumors of the gastrointestinal tract. The postoperative period was complicated by peritonitis in 4,5% of the patients. The most frequent causes of peritonitis were incompetent sutures of anastomoses, duodenal stumps (51,9%) and so called peritonitis due to "guy sutures" (26,9%). Lethality was 57,7%. The development of postoperative peritonitis may be shown by the phenomenon of worse state of the patient after a HBO session performed in the postoperative period. Early relaparotomy is of main significance in the complex treatment of postoperative peritonitis.

[Diagnostic and therapeutic endoscopy in foreign bodies of the digestive tract].

The authors observed 45 patients with foreign bodies in different parts of the digestive tract. In all the patients the foreign bodies were extracted with the help of endoscopes without complications. Wide introduction of diagnostic and curative endoscopy in the clinical practice in cases with foreign bodies in the digestive tract allows to avoid operations in most cases. Besides, it considerably shortens the time of staying of such patients in the hospital.

[Diagnosis and treatment of digestive tract obstruction using fiberscopes].

The authors propose indications and contraindications for endoscopy performed in 216 patients. The endoscopic picture of the most frequent causes of obstruction is given. In 113 patients endoscopy of the digestive tract was fulfilled also with curative aims, in 48 patients the phenomena of acute obstruction were resolved or considerably abated. This permitted performing the necessary operations in the planned order.
