[Ischemic cardiomyopathy: remote complications and their prognostication after isolated myocardial revascularization].

Aim of investigation was to study rate of remote cardiovascular complications after aorto-coronary grafting (ACG) in patients with ischemic cardiomyopathy and revelation of informative pre- and early postoperative markers of left ventricular (LV) function for determination of prognosis of cardiovascular complications and functional state of patients in remote postoperative period. Doppler echocardiographical parameters of left ventricular function were assessed in pre- and early postoperative period in 107 patients in whom cardiovascular events did not recur in late postoperative period (during 1 year) and in 63 patients who had recurrent cardiovascular events. Influence of a sign (x) on probability p(x) of cardiovascular events in postoperative period was estimated on the basis of logistic model. Rate of cardiovascular complications during 1 year after ACG in patients with ischemic cardiomyopathy was 40%. Most significant prognostic markers of cardiovascular complications in remote postoperative period were LV ejection fraction and deceleration time of early filling before surgery and in early postoperative period, as well as LV end systolic diameter in early postoperative period (p<0.05).

[Automatic analysis of cardiological signals for diagnosis of ischemic heart disease].

A new noninvasive method for early detection of ischemic heart disease based on singular value decomposition is introduced. The method automatically explores waveforms of an ECG by making projections of individual ECG leads onto axes formed by the first three components of the singular value decomposition performed for the whole group of ECGs, including cases with and without coronary artery lesions (460 ECGs). Detection of the presence of lesions is performed with 73% sensitivity and 83% specificity. Sensitivity and specificity become lower when specifying which coronary artery contains lesions: 69% and 77%, respectively, for the right coronary artery, 66% and 82%, respectively, for left anterior descending artery. 74% and 71%, respectively, for the left circumflex artery. The method is being developed so that it might be included into the programs of automatic ECG analysis, particularly for population studies.

[Atypical creatine phosphokinase isoenzyme fraction in the diagnosis of acute myocardial infarction].

A study of CPK isoenzyme spectrum in acute myocardial infarction, using electrophoresis and isoelectric focusing techniques, demonstrated the possibility of an earlier detection of myocardial necrosis on the basis of changes in CPK MM subzones, rather than the CPK MB test. This approach does not increase the specificity of isoenzyme CPK assays, but improves the reliability of biochemical assessment of the time of onset and depth of myocardial affection.

[A register of acute myocardial infarction: its incidence and mortality in Kaunas].

A study of acute myocardial infarction morbidity and mortality in Kaunas, using a WHO-recommended method, demonstrated that total mortality rate dropped from 43.9 to 31.7% between 1972 and 1986. An increase in morbidity was recorded among young men, which was due to increasing number of patients with the first myocardial infarction. Mortality changes follow an undulating pattern.

[Changes in the cardiometric parameters in ischemic heart disease].

Study of planimetric and weight indices of individual parts of the heart in 55 males (average age 51.2 +/- 2.7 years) who died of the first or repeated (primarily early) myocardial infarction has revealed the appearance (prior to the first major coronary event) of compensatory myocardial hypertrophy of all parts of the heart. Even in cases of the first infarction it was followed by dilatation of all cardiac chambers. Moreover, myocardial dilatation was exceeded by hypertrophy in all parts of the heart except left ventricle where both processes were found to be of the same extent. In the repeated myocardial infarction group, further increase of hypertrophy and dilatation was noted in the left parts of the heart only. In the ventricle both processes were of a similar degree while hypertrophy was prevalent in the atrium. The hypertrophy and dilatation of individual parts of the heart could be used for more accurate diagnosis--both clinical and postmortem--of early phases of ischemic heart disease and for the assessment of the heart pumping state.

[Transient changes in the ST-T complex of patients with stable stenocardia of effort during 24-hour ECG monitoring].

Changes in ST-T complex were studied using Holter electrocardiographic monitoring in 83 male patients with stable angina of effort. Episodes of transitory ST displacement were detected in 74.8% of patients. Of the recorded 886 ST displacement episodes, ST depression was seen in 35.9% of cases, with accompanying anginal pain in 88.4% of those. ST elevation was seen in 64.1% of cases, but anginal pain only accompanied a more than 2 mm elevation in 3.2% of those, while 96.8% of 1-2 mm elevations were asymptomatic. Total duration of ST depression was found to be an objective criterion for the assessment of the severity of stable angina of effort, amounting to less than 1% of the day in patients referred to functional class (FC) 1, 1-4% in FC 2, 5-10% in FC 3 and more than 10% in FC 4.

[Diagnosis of ischemic heart disease in an organized population].

In a simultaneous cardiological examination of the workers and employees of an electronics plant, a total of 3183 ECGs at the 12 standard leads were recorded with the help of the automatic ECG analysis system HP5600C. Parallelly, the ECGs were coded by the Minnesota Code. A group of high risk for coronary heart disease was identified on the basis of the questionnaire data and ECG findings and comprised 7.8% of the population studied. Pathological ECGs were registered in only 27.1% of the high risk group.

[Acute myocardial infarct register: the intravital and morphological criteria of "definite" and "possible" infarct].

The analysis of frequency and significance of standardized clinical diagnostic criteria of acute myocardial infarction was performed in 258 patients with ischaemic heart disease in the groups of "certain" (227 cases) and "possible" (31 cases) acute myocardial infarction in Kaunas myocardial infarction register. For the analysis of morphological criteria the pathology of coronary arteries and of the myocardium of 85 males, brought in dead, was studied in detail. 26 of these cadavers were classified as "certain" and 59 as "possible" myocardial infarction. The results of investigation showed the accepted criteria to be inadequate for complete identification of acute myocardial infarction. Frequent and more comprehensive electrocardiographic examinations and detailed histological studies may be helpful for reaching this aim.

[Retrospective evaluation of angina pectoris in patients with primary and recurrent myocardial infarct].

Retrospective analysis of information on 283 patients with myocardial infarction (186 males and 97 females) showed that 64.7% of persons who had suffered from the disease for the first time and 88.6% of those with recurrent myocardial infarction had a history of anginose pain. Exacerbation of angina pectoris had occurred in 56.0% of patients with no previous myocardial infarction and in 71,4% of those with recurrent infarction. A preinfarction state was encountered in primary and recurrent myocardial infarction with equal frequency (in 73.0 and 74.7% of cases, respectively). Nitroglycerin efficacy diminishes in the month preceding myocardial infarction. During exacerbation of angina pectoris, 39.8% of patients who had no previous myocardial infarction and 73.8% of those with a history of the disease applied for medical advice.

[Study method for registering cardiovascular morbidity with automatic processing of medical data].

Automation of the registration of cardiological diseases provides the basis for creating an automated system of control in cardiology, which may be used to analyse the disease incidence for any length of time and to obtain, whenever required, extensive operative information on the activity of therapeutic and prophylactic establishments. This information will serve for judging the condition of specialized cardiological service on the whole as well as in different territorial and production-shop areas, and in cardiological offices, and for estimating the need in hospitalization according to the forms of cardiological diseases. This, in turn, makes it possible to improve the planning of medical care measures.

[Method of conducting a staged rehabilitation of patients who have had a myocardial infarct].

Stage rehabilitation with the use of special centres is a combination of physical culture, physiotherapeutic, psychological and medicinal effects. The patients is early passed, contraindications excepted, with an average hospital treatment of 22 days with bicycle ergometry examinations at dismissal. A whole round method of assessment of the patients condition during rehabilitation is being used. Results achieved are reinforced during out-patient examination of all patients who had had myocardial infarction in a cardiological centre.

[Registry data on acute myocardial infarct in Kaunas].

An analysis of the register data on acute myocardial infarction in Kaunas is made covering the period of 1969-1979. The results show the increase in the incidence of acute myocardial infarction over the period studied from 1.0 to 1.67 per 1000 population and the decrease in the general mortality from 46.0 to 30.4%. The decrease in the general mortality is due to the organizational methods and the improvement of the diagnosis and treatment.
