[Contractile function of ureter in its anomalies in children].

A study on the testing unit was made of a contractile function of ureteral fragments removed at reconstructive operations for vesicoureteral reflux (VUR), neuromuscular dysplasia (NMD) and pelvoureteral segment stricture (PUSS). The study parameters (wall rigidity, amplitude of the contractions, potassium depolarization contracture, responses to adrenoreactive drugs) in patients with VUR were close to normal, whereas in those with NMD or PUSS changed significantly, especially in PUSS patients. Ureteral contractility impairment was heterogenic in all the groups, varying from intact to lost and did not correlate either with age or the disease stage. The value of functional studies of the ureter in reconstructive surgery for specifying the disease pathogenesis, mechanisms of functional decline and individual prognosis of functional efficiency of the operative intervention is discussed.

[The role of electropyelo- and electroureterography in assessing operations on the upper urinary tract].

Biopotential recording is currently used in patients with urologic diseases to determine the status of the upper urinary tract, to assess the electric activity of detrusor urinae, as additional objective indications while choosing an organ-preserving or organ-removing surgery, to check the effects of direct electric stimulation, to elucidate the magnitude of lesion and its extension. Electropyelo- and electroureterographies were performed by the routine procedures by employing foreign and Soviet-made devices in 76 patients with various urologic diseases resulting in upper urinary contractility: hydronephrosis, neuromuscular dysplasia, vesicoureteral reflux, as well as with other diseases. Electroureterography of the ureteropelvic segment will help choose a correct tactics of surgical therapy for patients with hydronephrosis; in case of markedly lower electric activity, the terms of nephrostoma removal after ureteropelvic plastic operation may be increased up to 2 months, as evidenced both by electropyelographic and clinical findings. Electropyelography was found to contribute to definition of indications for electrostimulation of the renal pelvis when its tension was decreased in patients with urolithiasis, which enabled the nephrostomic drain to be healed subsequently, as well as to assessment of electric stimulation used in postoperative reflux, as confirmed by electroureterographic findings. The findings led to the conclusion that it was advisable to use electropyelo- and electroureterography to choose a method of surgical treatment, electrostimulation and as criteria for recovered urinary contractility.

[Hydronephrosis in children].

The authors followed up 125 children with hydronephrosis aged 4-15. The diagnosis was based on ultrasound, radiological, x-ray, pelvic electromyography. Vitaiker's test findings. Postoperative follow-up covered 91 patients and lasted 1-5 years. Histological evidence provided data on 3 types of changes in the ureteropelvic segment. This necessitates not only Andersen-Hynes operation, but also graft plastic surgery contraindicated in the small-size segment, in extended stricture of the small-diameter segment, in existence of an accessory vessel, in the segment thin wall. Poor surgical outcomes were often the result of renal functional failure. In patients with renal functional loss up to 25% positive results were achieved in 91.1% of the cases, in the loss 26-50% in 81.8% of the patients, in the loss 51-75% in 79%, in that 75-100% in 61.5% of those surgically treated.

[The choice of the type of operation in hydronephrosis in children].

113 hydronephrosis children underwent Andersen-Hynes operation, 62 Calp de Werd operation. The ureteropelvic segment was examined morphologically. 3 macroscopic variants of ureteral structure and 5 variants of leiomyocyte presence in the muscular lining of the ureter are defined. Long-term follow-up results (1-10 years) are available. Resection gave positive results in 80%, graft plastic repair was effective in 92% of cases.
