The ulcer disease: an updated classification system.

The study included 736 probands with ulcer disease and 110 healthy subjects as well as all their relatives up to a third-degree of relation (n = 21889). All subjects underwent a thorough genealogical analysis, blood group, PTC and dermatoglyphic studies were performed on the whole contingent. Using extensive genealogical, genetico-mathematical and multifactorial analysis we studied the manifestations of anticipation, the influence of hereditary predisposition on gastric secretion and acidity under conditions of maximum pentagastrin stimulation as well as the disease inheritance pattern. The results of our comprehensive studies suggest that type I gastric ulcer according to H. Johnson, unlike type II and type III, is not genetically determined. On the other hand, type II and type III gastric ulcer do not differ substantially from duodenal ulcer and should therefore be categorised as belonging to this form of the disease. On this basis we propose a new classification system which accepts the existence of two ulcer diseases: gastric ulcer disease, and ulcer disease of the pyloroduodenal region.

Influence of the type of family predisposition on the age of ulcer disease onset.

736 probands with endoscopically and/or roentgenologically proven ulcer disease of the stomach and duodenum were studied. Of these 429 were with hereditary predisposition to the disease (including in the study all their third-degree relatives). Our findings suggest that the age of disease onset is markedly dependent on the type of familial predisposition. It occurs the earliest and relatively the most frequently in probands with two affected parents (familial predisposition type III). The peak frequency of the disease for non-predisposed subjects is between 30 and 39 years of age while in subjects with familial predisposition of the first type (two healthy but genetically predisposed parents) the II-A type (a sick father) and the II-B type (a sick mother) it occurs a decade earlier. Type III familial predisposition causes the disease to affect the probands even earlier by another decade so half of all the probands of this type of familial predisposition are affected by the disease by the age of 19. The different types of familial predisposition to gastric ulcer (types II and III) and duodenal ulcer reflect the hereditary ulcerogenic load received by the offspring. All other conditions being equal, the greater it is the earlier the disease occurs.

Genetico-mathematical analysis of the inheritance pattern of peptic ulcer disease. II. Method of simple registration. Assessment of the direct methods of Weinberg.

The inheritance pattern of ulcer disease was analyzed by means of the method of simple registration. We also studied comparatively and assessed all direct methods of Weinberg using the coefficients of genetic proportion calculated by them as a criterion at complete inclusion of the investigated groups. 351 probands' families were studied: 57 with type II and 27 with type III gastric ulcer disease and 267 with duodenal ulcer disease. Gastric ulcer types were determined according to the classification of H. Johnson. The coefficient of genetic proportion in the cases of duodenal ulcer disease was additionally calculated according to the type of patients' familial pre-disposition. The coefficient of genetic proportion was found to acquire identical values for all groups studied without exception when calculated on the basis of one proband in a family (y = 1) both by the method of probands and the method of simple registration. The segregation coefficient values obtained using the direct methods of Weinberg do not support the hypothesis of monogenic transmission pattern either of the ulcer disease as a whole or of its different varieties.

Genetico-mathematical analysis of the inheritance pattern of peptic ulcer disease. Direct methods of Weinberg. I. Method of probands and method of siblings.

Inheritance of ulcer disease was analyzed genetico-mathematically using two methods: the method of probands and the method of siblings. Families of 351 probands were enrolled in the study: 57 had type II and 27 type III gastric ulcer disease. 267 were with duodenal ulcer disease. The types of gastric ulcer were determined according to H. Johnson. A further 188 families of type I gastric ulcer probands were also studied. However, data from these are not included in the present study because our previous investigation has shown no genetic determination of this type of ulcer disease. The detailed segregation analysis employed in the study does not support the assumption of a monogenic type of disease inheritance. The values obtained for the coefficient of genetic proportion do not substantiate either autosomal-recessive or autosomal-dominant pattern of transmission. This refers both to ulcer disease in general and to the calculation of the coefficient depending on the type of disease and the type of familial predisposition of probands.

[Gastric secretion and acidity in clinically healthy persons in old age].

Gastric secretion and acidity under maximum pentagastrin stimulation were studied in 36 phenotypical healthy subjects of advanced age and compared with the same parameters in 36 young healthy subjects. Both the volume of gastric secretion and the acidity were significantly lower in the clinically healthy subjects of advanced age and normal weight than in the young adults remaining, however, within the normalcy range. The data suggest that the organism of elderly persons seems to be adequately supplied with gastric secretion and acid content.

[Effect of complications on the rate of gastric secretion and acidity in duodenal ulcer].

The effect of the complications of duodenal ulcer on the rate of gastric secretion and acidity was studied under the conditions of maximum stimulation with pentagastrin. A total of 440 patients were studied. It has been established that the volume of gastric secretion and acid secretion are significantly higher in complications than in the absence of complications. The elevated values of gastric secretion and acid production in the patients with complicated duodenal ulcer are determined by their predominant elevation in case of past perforations and in case of stenosis of pylorus in particular. As compared with the patients without complications, the patients with the two indicated complications have 1.11 to 1.42 and 1.15 to 1.92 times higher indices respectively. The elevation of the parameters in past histories of gastrointestinal hemorrhages is rather small. The chronic reactive hepatitis, originating as a complication of duodenal ulcer, has lower values of gastric secretion and acidity as compared with the non-complicated form of the disease. The mean values, however, of none of the indices drop under the norm but range within the upper physiological limits.

[Duodenal peptic ulcer--hereditary and gastric secretion].

The effect of hereditary predisposition on gastric secretion and acidity in duodenal ulcer was studied. A total of 231 patients (146 males and 85 females) were studied under the conditions of maximum stimulation with pentagastrin. The results reveal that the hereditary tainted probands have higher values of the single indices, with 1,30 to 2,03 times on the average for gastric secretion nd acidity than the patients, not hereditary tainted. The comprehensive analysis of the data obtained show that the sex differences in the degree of the indices studied are, most likely, secondary and seemed to be determined by the higher incidence of the disease among the males, hereditary tainted. That presumption is confirmed also by the fact that no differences in gastric secretion and acidity were established between the males and females with duodenal ulcer non-hereditary tainted.

[Anticipation of duodenal peptic ulcer. I. Prevention in subjects at risk of inheriting it].

A total of 267 patients with duodenal ulcer with a familial history of the same disease were studied, whose patients, one of them or both, had suffered from the same disease. It was established that in the children of the affected parents, the disease occurred at a considerably earlier age. The premature occurrence for the separate groups is with 12.5 to 16.7 years on the average and is established in 96,6 per cent of the probands, whereas a retardation was observed in only 1,5%. It could, therefore, be admitted that the anticipation is a regular phenomenon for the duodenal ulcer. Its effect, however, has some specific characteristics in the duodenal ulcer disease, and cannot be explained only from the position of the hypothesis of multigen heredity. The anticipation of ulcerogenesis in the children of the affected parents is admitted to be determined, primarily, by the progressively enhanced action of polygens in the generations. The environmental factors, very likely, play an additional role in that process.

[Blood group affiliation of patients with duodenal ulcer].

The blood groups of 766 patients with duodenal ulcer were studied. A significant association of the disease was established with blood group O (I), particularly emphasized in hereditary loaded patients. The carriedship of the indicated blood group increases (under the same other conditions) the risk of origination of duodenal ulcer with about 50 per cent. The belonging to blood group A (II), on the other hand, decreases that risk with approximately the same percentage. The rest blood groups from the system ABO (H) appear not to have an essential effect on ulcerogenesis in peptic ulcer of duodenum.

[Genealogic analysis of peptic ulcer of the duodenum].

The results are presented from a meticulous genealogical study, to III stage of family descent, of 348 patients with duodenal ulcer and 101 healthy subjects. The index of hereditary defects, with the strictly collected data, revealed that in 3/4 of the patients degree greater than families some other representatives with ulcer disease were found. With broadening the circle and including more relatives, that index regularly grows. The incidence of ulcer diseases among the relatives of the propositus with duodenal ulcer is 5 to 10 times more frequent as compared with the corresponding stages of kinship among the healthy controls. The duodenal ulcer, in the hereditary defective patients, is manifested with an average of 8--10 years earlier. The same localization of the ulcer niche was found in 91.12 per cent among the blood relatives of the propositi. The males suffer 2.5 times more often from duodenal ulcer.
