Morphofunctional changes in the placenta after ozone therapy.

Macro- and microscopic aspects in the morphology of placenta after ozone therapy were studied in pregnant women with placental insufficiency and the results were compared with the control group. In the main group, the placentas were smaller and weighed less. After ozone therapy enlargement of terminal villi with true syncytiocapillary membranes was noted. A stimulating effect of ozone therapy on the growth and differentiation of terminal villi was established.

[The immunological aspects of maternal-fetal interrelationships].

The paper deals with different aspects of the relationships occurring between the pregnant woman and the fetus having paternal antigens. Analysis of a great deal of recent information clearly indicates that local uterine immunity plays a great role during pregnancy. The specific features of the gene expression of major histocompatibility by an invasive trophoblast and the immunosuppressive function of placental fibrinoid are discussed. A detailed account of the immunocompetent cells, including T lymphocytes, giant granular lymphocytes, and macrophages, that are involved in immunological responsiveness within the decidua is given.

[Pathomorphology of spiral uterine arteries, umbilical vessels and placenta in disorders of maternal-fetal circulation].

The comparison of the spiral uterine arteries structure, umbilical vessels and placenta is performed in 16 women with normal haemodynamics and in 31 women with disturbances of mother-foetus circulation, and the data obtained were compared with results of dopplerometry made before the delivery. Three clinical groups are distinguished according to the predominant localization of the resistance to the blood flow: the first--with the domination of the spiral arteries obliteration (lack of gestational changes, hyperplastic arteriosclerosis) with a characteristic ultrastructural pathology of the syncytiotrophoblast microvilli, rheological disturbances in the intervillous space and large necroses of villi; the second--with a predominance of early immaturity of the stroma and capillaries or sclerosis and compression of foetal villous capillaries, i. e. with a reduction of a capillary bed and increase of the resistance to the blood flow; the third--with a predominance of the umbilical arteries narrowing and supporting villi and signs of a blood stasis in the foetal veins. Morphologic heterogeneity of the mother--foetus circulation disturbances should be taken into consideration when choosing obstetric tactics.

[Pathology of the spiral arteries of the uterus and its significance in the pathogenesis of disorders of the uteroplacental circulation].

The literature survey concerning the morphological aspects in the uteroplacental blood flow is presented. The methodological approaches applied in the investigation of spiral arteries are classified. Changes in uterine structures in normal pregnancy are characterized. A great emphasis is laid on gestational alterations in spiral arteries. The types of pathological processes occurring in the spiral arteries are described. The data available in the literature are given on the incidence of various types of pathology and its association with nephropathy, intrauterine fetal hypertrophy, and other abnormalities.

[Comparative study of dopplerometry and morphological study of placenta and uterine arteries in the evaluation of hemodynamic disorders in the "mother-placenta-fetus" system].

The paper gives the comparative results of Doppler examination of blood flow in the mother-placenta-fetus system and morphological examination of the vessels of the funis, placenta, and placental bed. The latter examination allowed all the cases of placentomaternal and placentofetal circulatory diseases to be divided into 3 groups. There was a clear-cut correlation between the Doppler metric and morphological findings.

[Isolated symplasts and stromal syncytial buds of the normal human placenta at 6 to 14 weeks of pregnancy].

Formation of isolated symplasts (IS) ("deported trophoblast", according to English nomenclature) and stromal syncytial buds (SSB) has been studied in 10 normal patients at pregnancy of 6-14 weeks by means of the indirect immunoperoxidase method. Monoclonal antibodies against beta-subunit of the human chorionic gonadotropin (beta-HCG), serial paraffin slices, stained with hematoxylin-eosin, semithin slices, stained with toluidin blue-pyronine and scanning electron microscopy have been used. A great amount of syncytiotrophoblastic processes and IS, analogous by their structure to syncytiotrophoblast are revealed. Presence of beta-HCG in the syncytiotrophoblast and its absence in the cytotrophoblast are proved. Concentration of beta-HCG in the IS is higher than in the syncytiotrophoblast of the villi. Maximal concentration of beta-HCG in the SSB is described for the first time. The functional role of the IS and SSB is discussed in the system mother-placenta-fetus.

[Morphology of organs of the immune system in perinatally dying babies with autosomal trisomies].

Morphology of the immune system organs was studied in 13 babies with autosomal trisomies (Down's, Edwards', and Patau's syndromes) dying in the perinatal period and having no morphological signs of infectious diseases. The control group included 5 babies without congenital malformations and 12 babies with unclassified multiple malformations. Hypoplasia of the lymphoid tissue was demonstrated in autosomal trisomies (the relative weight of the thymus decreased 2-fold; the spleen, lymph nodes and the intestinal tract deficient in lymphocytes). In Down's disease hypoplasia was most marked; besides, a 3-fold increase in the average area of Hassall's bodies as compared with that in Patau's and Edwards' syndromes and in the controls was observed. Babies with congenital malformations of non-chromosomal origin had no significant differences in the condition of the immune system organs as compared with babies without malformations which indicates the influence of chromosomal pathology on the formation of the immune system in the intrauterine period.

[Principles of pathological diagnosis in children with congenital abnormalities].

The features of establishment of pathological diagnosis in children with congenital malformations (CM) are discussed. An analysis of 1058 autopsy records of the centralized department of pathology of the Children's Clinical Hospital No. 1, Moscow, was carried out. CM were found in 236 cases (22.3% of the total number of autopsies). In 117 cases (49.6% of all CM observations) CM was the main disease, in 82 (34.7%) observations CM were combined with other diseases. In the establishment of pathological diagnosis in cases of combined diseases, the epidemiological situation, the importance of one or another process for statistical generalizations should be taken into consideration. In our opinion, preference should be given to cases of intrauterine, highly contagious, and nosocomial infections. In 37 (15.7%) observations CM exerted no effect on the course of the disease and was classified as the background condition.

[Pathological changes in spiral arteries in systemic lupus erythematosus].

The uterus removed at 20-week pregnancy in a female with grave lupus erythematosus complicated with an antenatal fetus death was studied. Signs of arthrosis typical for late pregnancy were found in the spiral arteries of basal decidua in the zone of contact between mother tissue and trophoblast. Fetal death was due to massive thrombosis.

[Morphological characteristics of chronic endometritis].

The immunohistochemical examination of 46 endometrial biopsies from women with chronic endometritis (CE) suffering from habitual abortions has found disturbance of correlations between various types of immunocompetent cells with domination of T-suppressors, increased accumulation of IgG on the surface of glandular epithelium. In CE, thickening of spiral arteries occurs not because of sclerosis but due to muscular hypertrophy, endometrial stromal-epithelial relations are impaired. Alterations in the distribution of the extracellular matrix components indicate defective endometrium maturation which may lead to inadequate oocyte implantation.

[Changes in the components of the extracellular matrix and its regulators in the endometrium of women with habitual abortion].

Biopsies of the endometrium from 93 patients with habitual abortions (HA) were studied immunohistochemically. Chronic endometritis (CE) was diagnosed in 26 patients. Decreased tenascin expression in the endometrial stroma with merozin accumulation in the glandular basal membranes, covering epithelium and vessels were found in HA combined with CE. Abnormal distribution of collagen type I and III with predominance of type I collagen and appearance of merozin. This is indicative of alteration in the endometrium maturation which may result in inadequate ovum implantation and pregnancy interruption. Stromal alterations in HA with or without chronic endometritis are under multifactorial control of various mediators. High level of MMP-9 combined with low level of TFP-beta 1 may serve a prognostic criterion of successful completion of pregnancy regardless of CE. Low level of MMP-9 may increase the risk of pregnancy interruption.

[Morphological characteristics of chorion in spontaneous abortions of chromosomic etiology].

Morphological investigation of spontaneous abortions (SA) with normal and abnormal karyotype (220 specimens of the first trimester SA) was carried out. It included a histological pattern, apoptosis and proliferative activity. Estimation of apoptotic index (AI) was carried out in 7 cases of SA with normal karyotype and 7 cases of SA with pathological karyotype using the ApopTag Peroxidase Kits (Biotech, USA). The material of 4 therapeutic abortions was taken as the control group. The proliferative activity (PA) was determined as the rate of Ki-67-positive cytotrophoblast cells. PA was examined in SA groups with normal (15 cases) and pathological (15 cases) karyotypes and 20 cases of control group. Identical morphological signs of the development retardation of the villi were diagnosed among abortuses with normal karyotype and with aneuploidies. The diameter of the villi was bigger in aneuploid SA than in SA with normal karyotype (p > 0.05). The partial moles were diagnosed in triploidy, as it was expected. High level of AI and PA in the control group was revealed. In SA with pathological karyotype a high rate of AI accompanied a low level of PA. The AI and PA were low in SA with normal karyotype. However, the largest uncorrelation between AI and PA in extravillous trophoblast cells of SA with chromosomal anomalies were observed: the level of AI was higher than the level of PA. We suppose that the last one leads to the abruption of the ovum.

[Intrauterine mycoplasmosis combined with bilateral adrenal ganglioneuroblastoma].

The paper reports lethal intrauterine mycoplasmosis in a newborn girl. Her mother suffered from genital mycoplasmosis and had blood antibodies to Myc. Hominis and Ureapl. urealyticum. Most pronounced changes are found in the liver: hepatocyte degeneration and necrosis with PAS- and azur-positive granules in the cytoplasm of hepatocytes and macrophages, lymphohistiocytic infiltration of portal tracts and parenchyma, biliary duct proliferation, cholestases. The lungs: interstitial pneumonia with minimal alteration of the bronchial epithelium. The above spectrum of alterations suggests transplacental contamination. Ganglioneuroblastoma consisting of embryonal neuroblasts found in the adrenals is considered as an independent disease. The first child of this woman died 14-days old and had morphological signs of the intrauterine infection and pronounced liver dysplasia; possible oncogenicity of mycoplasma is discussed.
