Factors associated with back pain in nursing staff: a survey in Athens, Greece.

Factors associated with self-reported back pain were investigated using questionnaire data on 407 female nurses from a large hospital in Athens, Greece. Factor analysis was used to construct indices of pain and its impact on normal life, and also to summarize work load descriptions. Pain factors were examined in relation to work load and personal characteristics by logistic regression. Statistically significant items, in relation both to pain and impact, were the existence of previous back injury, self-reported headaches and the 'carrying and lifting' factor of work load which principally included moving equipment. Age, height and weight were not associated with back pain. Compared to a similar study by Harber and colleagues in California, United States of America, the factors associated with back pain were quite similar in this study even though the prevalence of back pain was much higher among Greek nurses and their work load was physically much more demanding.

Prevention of postischaemic lipid peroxidation and liver cell injury by iron chelation.

Liver ischaemia was induced by cross clamping the hilar pedicle for 30 minutes in groups of rats with or without treatment with the iron chelating agent desferrioxamine (deferoxamine, DFR). The groups included eight animals each and were divided into the following treatment categories: control; ischaemia alone; ischaemia with subsequent reperfusion; ischaemia preceded by DFR, 60 mg/kg body weight; and reperfusion preceded by 20, 40, or 60 mg/kg DFR. The drug was given intravenously five minutes before either ischaemia or reperfusion. Malondialdehyde (MDA), a product of lipid peroxidation, and histopathological changes of liver tissue samples were used as indicators of hepatocellular injury. Lipid peroxidation (MDA concentration in mumol/kg liver tissue) was highest (4.76 (1.19] after ischaemia without reperfusion and less pronounced (2.87 (0.34] after reperfusion. Both concentrations, however, were significantly (p less than 0.05) higher than basal (control) values (1.78 (0.27]. At 60 mg/kg body weight, DFR treatment reduced MDA to basal or even lower concentrations in both situations (1.98 (0.08) and 1.26 (0.06), respectively) with a corresponding improvement in liver histopathology. Lower DFR doses were less protective. The data suggest that liver ischaemia is associated with free radical initiated, and apparently iron catalysed lipid peroxidation, which can be significantly decreased by iron chelation.

[Terminalized lateral colostomy].

Complete diversion of the digestive transit requires intestinal section and terminal rather than lateral colostomy. This can now be achieved by using a mechanical stapler to obturate temporarily the distal end of the colonic segment bearing a conventional lateral colostomy, then performing an extra-mucosal anastomosis to re-establish continuity. This technique can be applied to protect low colonic anastomoses or to treat a minor anastomotic disruption. It can also be extended to ileostomy.

The removal of sewing needles from two children's hearts.

Penetrating foreign bodies in the heart comprise a class of serious injuries. The successful removal of sewing needles from the hearts of a 6-week-old baby and a child aged 2 years are presented. The needle had penetrated the left anterior chest wall and had been thrust into the heart in both cases. Determination of the exact location of the foreign body is of utmost importance to the surgeon. A complication attributable to the foreign body may demand immediate surgical intervention. In general, delay in the timing of the operation should be avoided because the foreign body might migrate from the pericardium to the myocardium and cardiac cavities. The surgical approach depends on the location of the foreign body: the median sternotomy being the most frequently employed. With the advent and refinement of the techniques of cardiopulmonary bypass and myocardial preservation this approach renders the removal of even small fragments a straight forward procedure.
