Normal lung perfusion scan with extensive thromboembolic disease.

A 50-year-old woman with unexplained pulmonary hypertension had normal findings on six-view lung perfusion scan prior to open lung biopsy. The biopsy demonstrated old organized thrombi. The patient expired and autopsy disclosed extensive thromboembolic disease, both old and new. A six-view lung scan may not distinguish primary pulmonary hypertension from thromboembolic hypertension when diffuse symmetrical thrombi are present. Under such conditions, when invasive diagnostic procedures have an unacceptable morbidity, empiric anticoagulation should be considered.

The value of the negative radionuclide scrotal scan in the management of the acutely inflamed scrotum in children.

Torsion of the testis remains a clinical diagnosis and confirmatory testing is not required when the classic signs and symptoms are present. Conflicting laboratory data or adjunctive diagnostic studies should not dissuade the surgeon from exploration when the clinical index of suspicion is high. In those patients having equivocal clinical findings when the diagnosis of torsion is not clear-cut radionuclide scrotal imaging may be useful. In this series a negative scrotal scan at the time of acute scrotal pain did predict reliably the diagnosis of non-torsion, suggesting that in the appropriate clinical setting scrotal imaging is useful in avoiding surgical exploration.

Limits of sensitivity of radiopertechnetate flow studies in the detection of asymmetrical renal perfusion.

A canine model has been devised using electromagnetic flowmeter probes to study the sensitivity of radiopertechnetate renal angiography. Highly significant correlations were obtained when the measured flow rates were compared with normalized values of the peak activity and uptake slopes for each kidney. Analysis of the data suggests the unilateral renal disease associated with less then 20% difference in flow between the kidneys may not be diagnosed reliably from the pertechnetate perfusion test.
