Bilateral parotid gland basal cell adenomas. Case report.

We report a 65-year-old female with basal cell adenomas arising in the left and right parotid glands and review the literature. Diagnosis was based on clinical features, ultrasonographic and CT findings as well as histopathological examination. Parotid tumours usually arise on one side and bilateral occurrence is rare, accounting for 1-3% of all parotid tumours. Most bilateral parotid tumours are Warthin's tumours and pleomorphic adenomas, and bilateral basal cell adenomas of the parotid glands are very rare and only 5 cases including ours have been reported. The exact mechanisms contributing to the development of bilateral parotid tumours remain unknown. We speculate on the involvement of environmental and genetic factors since the histological features of both parotid tumours are identical in most cases.

Temporal bone pathology in DiGeorge's syndrome.

DiGeorge's syndrome is characterized by partial or complete absence of the parathyroid and thymus glands and is often associated with other developmental anomalies, particularly of the structures arising from the third and fourth pharyngeal pouches. The temporal bone findings in three cases of DiGeorge's syndrome are presented. Patients with this condition have a high incidence of Mondini dysplasia in both ears, sometimes with other anomalies of the external or middle ears. Hearing may range from normal to profound deafness and may manifest sensorineural, conductive, or mixed losses of varying degrees.

Individual variation and mechanism of kanamycin ototoxicity in rabbits.

In order to elucidate the mechanism of individual variation of kanamycin ototoxicity, the possible relationship between the outer hair cell damage induced by kanamycin and the levels of kanamycin in serum or perilymph and of renal damage was investigated in rabbits. No relationship was found between the extent of the outer hair cell damage and the level of kanamycin in the serum or the renal damage. The extent of the outer hair cell damage was closely correlated to the levels of kanamycin in the perilymph. These findings suggest that the individual variations in the outer hair cell damage induced by kanamycin are more closely correlated to the individual differences in the transferability of kanamycin into inner ear than to those in the vulnerability of the outer hair cells or kidneys.

Apolipoprotein levels in non-insulin-dependent diabetes mellitus with clinical macroangiopathy.

To elucidate the changes of serum apolipoproteins as possible atherogenic factors in non-insulin-dependent diabetes mellitus (NIDDM), comparison was made between 47 NIDDM patients with clinical macroangiopathy (coronary heart disease and/or cerebral infarction) and 47 NIDDM patients without clinical macroangiopathy individually matched for age, sex, body mass index, glycemic control, mode of therapy, and blood pressure. Serum total cholesterol (223 +/- 63 mg/dl, mean +/- SD) and triglyceride (160 +/- 72 mg/dl) levels in NIDDM with clinical macroangiopathy were significantly higher than those (198 +/- 44 and 126 +/- 78 mg/dl) in control NIDDM. Apolipoprotein AII (apo AII) (28.8 +/- 6.9 mg/dl) in NIDDM with clinical macroangiopathy was significantly lower than that (31.4 +/- 5.5 mg/dl) in control NIDDM, and apo B (147 +/- 48 mg/dl) and apo CIII (13.1 +/- 5.9 mg/dl) in NIDDM with clinical macroangiopathy was significantly higher than that (113 +/- 35 and 10.8 +/- 4.7 mg/dl) in control NIDDM, respectively. There were no significant differences in apo AI, CII, and E levels. The ratio of HDL cholesterol to apo AI (0.36 +/- 0.08) and total cholesterol to apo B (1.6 +/- 0.3) in NIDDM with clinical macroangiopathy was significantly lower than that (0.40 +/- 0.09 and 1.8 +/- 0.4) in control NIDDM. These results suggest that abnormality of lipid metabolism is more significant in NIDDM with clinical macroangiopathy than in NIDDM without clinical macroangiopathy, and is more pronounced in women than in men.
