Determination of a new calcium antagonist and its main metabolite in plasma by thermospray liquid chromatography-mass spectrometry.

A highly sensitive thermospray liquid chromatographic-mass spectrometric method has been developed for the simultaneous determination of FRC-8653 (I), a new calcium antagonist, and its main metabolite (M-4) in plasma. A deuterated analogue of I was added to the plasma as the internal standard. After the purification and concentration of the plasma sample on bonded-phase disposable columns, the extract was injected into the thermospray liquid chromatograph and analysed by selected-ion monitoring mass spectrometry. The calibration curves obtained were linear over the concentration range 0.5-100 ng/ml. The limits of quantification are 0.5 ng/ml for I and 1 ng/ml for M-4 in plasma, which are sufficient to evaluate plasma concentrations after oral administration to rats.

A method with three triangular flaps as a secondary operative procedure after reconstruction of the lower lip by Estlander's method.

We have devised a new method for secondary commissuroplasty after reconstruction of the lower lip using Estlander's method with both aesthetically and functionally satisfactory results. This method consists of forming two equilaterally triangular mucosal flaps on the vermilion and a small triangular skin flap in the new position of the commissure and transposing these three flaps to reconstruct the commissure. In the present paper, we reported the procedure and the results. This method produces extremely good results, obtaining favorable commissure form and reconstruction of the mucosa of both upper and lower lips without leaving an unnatural-looking color change in the mucosa or a step deformity in the vermilion. It can be expected that our method will improve the results of Estlander's operations after tumor resection in the lower lip.

A study on bone induction in hydroxyapatite combined with bone morphogenetic protein.

Our present study consisted of an implantation of artificially made hydroxyapatite (HAP) ceramic pellets under the periosteum of the rabbit skull with subsequent inspection of further progress of bone formation and also of an evaluation of the effects of bone morphogenetic protein (BMP). The results revealed that the alkali phosphatase (AL-P) activity of the pellets was elevated only in those of the bone morphogenetic protein group. The results of determination of bone mineral density at the site of the pellets revealed that the increase in bone mineral density was the most remarkable in the bone morphogenetic protein group rather than the control group. The results of the histopathologic examinations revealed that marginal bone formation was found in the pores on the surface between the pellets and the skull in the control group and in the collagen group, whereas in the bone morphogenetic protein group very active bone formation was found not only on the interface in contact with the skull but also surrounding the whole pellet. It also was noted in the animals in the bone morphogenetic protein group that the pellets were corrupted from the peripheries and then absorbed into the newly formed bone. From these results, the efficacy of the hydroxyapatite-collagen-bone morphogenetic protein complex was made clear, and applications in clinical practice are expected in the near future.

N-myc gene amplification and neuron specific enolase production in immature teratomas.

The primary tumour tissues from 13 teratomas were investigated, 3 cases of immature teratoma (1 of pure type and 2 of mixed type), 5 cases of dermoid cyst, and 5 cases of mature solid teratoma, with special reference to N-myc gene amplification, productivity of neuron specific enolase (NSE), and the presence of double minutes (DMs). The possible relationship between these variables and malignancy of the tumours was also examined. N-myc gene amplification and NSE production were recognized in the primary tumour tissues and the first and the third passage cultured cells of all of the 3 cases of immature teratoma containing immature neural tissues. In 2 cases DMs were recognized. In dermoid cysts and mature teratoma, neither N-myc gene amplification nor NSE production were recognized in either the primary tumour tissues or cultured cells. The chromosomes were normal. Malignancy of teratoma is generally decided by the clinical stage and histological grade, but a more securely based decision is necessary where an immature teratoma contains immature neural tissues. The presence of N-myc gene amplification, NSE productivity and the presence of DMs may be valuable.

Translocation t(8;16)(p11;p13) in neonatal acute monocytic leukaemia.

A recent report demonstrated that t(8;16) (p11;p13) may be linked to acute monocytic leukaemia (AMoL) of differentiated subtype (M5b) with active haemophagocytosis by leukaemic cells. Only two cases of neonatal AMoL with t(8;16) (p11;p13) have been reported; M5b with haemophogocytosis and M5a. We report a case of neonatal AMoL (M5b) with t(8;16)(p11;p13), but haemophagocytosis by the leukaemic cells was not detected.

Effects of enriched lactate solution (ELS) for resuscitation after burn injury.

Full skin thickness burns covering 35 per cent of the total body surface of rabbits were followed by measurements of Na-K ATPase and arterial blood gas analyses, before and after the burn injury. Studies of the effects of intravenous fluids with different compositions showed that the active transport of the cell membrane was depressed in vivo immediately after a burn injury, mainly due to acidosis. This phenomenon was not completely corrected by the Baxter formula which uses conventional lactated Ringer's solution. However, an improvement was observed in those groups given the 'Enriched Lactate Solution' (ELS) containing large quantities of lactate as the base source. These results suggest that ELS, which positively corrected acidosis in accordance with its concentration, is very effective and more appropriate than the conventional lactated Ringer's solution for early burn resuscitation.

Carcinoembryonic proteins produced by Wilms' tumor cells in vitro and in vivo.

A Wilms' tumor cell line (HFWT) was established after tissue culture of the Wilms' tumor which had developed in the left kidney of a five-month-old boy. The HFWT line has the following cyto-biological characteristics: 1. The cells have a spindle, round or polygonal shape with neoplastic and pleomorphic features that grew in multilayers without contact inhibition. 2. The cells show a stable proliferation, giving 95 passages within 4 years. 3. The chromosomes show a wide aneuploidy distribution, and the modal number was found in diploid range. The stem cells have a normal karyotyping, 46, XY. 4. Heterotransplantation into nude mice can be easily made, and anaplastic Wilms' tumor resembling the original tumor forms. 5. The principal tumor markers produced by the cells in large amounts in the conditioned culture media were carbohydrate antigen 125 (CA 125) and tissue polypeptide antigen (TPA). 6. A good correlation was found between enlargement of the tumor formed by heterotransplantation into nude mice and the levels of CA 125 and TPA in the serum of the nude mouse. No report on the study of tumor markers widely used in clinical treatment of Wilms' tumor has been available to us, but CA 125 and TPA are considered to be useful in the diagnosis and treatment of Wilms' tumor.

Presence of human papillomavirus genome in human tumor cell lines and cultured cells.

A series of 47 human carcinoma cell lines and their cultured cells were examined for human papillomavirus (HPV) genomes with the use of an HPV detection kit (DNA-RNA hybridization, mixed HPV DNA probe of types 6, 11, 16, 18, 31, 33 and 35). Four of 8 cases of mild dysplasia, 3 of 9 cases of severe dysplasia, 3 of 7 cases of carcinoma in situ, 3 of 15 cases of uterine carcinoma and 5 of 6 cases of condyloma acuminatum were shown to contain the HPV DNA genome in primary cultured cells, while HPV was not detected in the third-passage cells except for the three cases of large cell, nonkeratinizing squamous cell carcinoma. HPV was also not detected in such normal tissues as uterine cervical squamous epithelium, uterine cervical columnar epithelium and endometrium. The presence of HPV DNA genomes was detected consistently in the passages of three lines (SKG-II, HKMUS and HKTUS; large cell nonkeratinizing squamous cell carcinomas of the uterine cervix) with the use of the Southern Blot method (DNA-DNA hybridization, mixed HPV probe of types 6, 11, 16 and 18). HPV type 16 DNA was detected in HKTUS, and HPV type 18 DNA was found in SKG-II and HKMUS. The other 44 cell lines, including ovarian carcinoma, endometrial carcinoma, sarcoma, gastric cancer, pancreatic cancer and rectal cancer, were negative for the HPV-6, HPV-11, HPV-16, HPV-18, HPV-31, HPV-33 and HPV-35 genomes under stringent hybridization conditions.

The cell-components and cytokines in the subcorneal microabscess of psoriasis.

Munro's microabscess of psoriasis has been generally considered to be composed of polymorphonuclear neutrophils (PMNs). However, the cell-components of the microabscess has not been fully clarified. To identify the presence of mononuclear cells (MNCs) in Munro's microabscess and the subcorneal pustule of pustular psoriasis, we observed psoriatic lesions histologically and immunohistologically. Morphologically a few MNCs were found among PMNs and they seemed to include Langerhans cells (CD1a+), helper/inducer T cells (CD4+, CD5+), and macrophages (CD14+) which expressed HLA-DR. Some of them seemed to produce gamma-interferon (IFN-gamma), interleukin 6 (IL-6) and IL-8. The keratinocytes surrounding the microabscess also had IL-8 and some epidermal cells of the proliferated epidermal papillae also expressed IL-6 and IL-8 in psoriatic lesions. The production of IL-6 and IL-8 is considered to effect both chemotaxis and the proliferation of epidermal cells. The presence of these cytokines also suggests that MNCs exist in Munro's microabscess and that they might contribute to the microabscess formation of psoriatic lesions.

[An evaluation of excisional biopsy for tongue carcinoma].

An evaluation of excisional biopsy for tongue carcinoma (T1, T2) as a surgical treatment was performed. Forty-seven patients who received excisional biopsy among 394 patients with tongue carcinoma (T1, T2) treated in National Cancer Center Hospital during 24 years were examined. These patients were divided into following three groups: group a) patients treated with excisional biopsy only, group b) those treated with excisional biopsy and cryosurgery, group c) those treated with excisional biopsy and radiotherapy. We examined local control rate of these groups, degree of differentiation in histopathological examination, and median cancer free surgical margin. From these we obtained following results: 1) There was no significance in local control rate among these three groups (group a 77%, group b 84%, group c 45%) 2) There were no local recurrences in the patients with cancer free margin of 5 mm or more except when the histopathology showed perineural invasion, lymphatic vessel invasion, and/or deep invasion to the muscle. 3) Even if we had cancer positive surgical margin with excisional biopsy, we could obtain high local control rate (75%) with additional cryosurgery. We conclude that excisional biopsy for tongue carcinoma (T1, T2) is a good method for primary therapy.

Treatment for synchronous and metachronous carcinomas of the head and neck and esophagus.

In the past 27 years, 75 patients were found to have separate primary carcinomas of the head and neck and the esophagus. Head and neck tumor was discovered first in 41 patients, synchronously with esophageal tumors in 17 patients, and esophageal tumors were recognized first in 17 patients. Reconstruction by graft after resection of head and neck cancer was necessary in 18 patients and esophagectomy was performed in 62 patients. In eight patients, pharyngeal and esophageal reconstruction was necessary after resection of synchronous and metachronous pharyngeal and esophageal carcinoma. The combined procedure with free tissue transfer was useful for surgical treatment for synchronous or metachronous carcinomas of the head and neck and the esophagus.

Characterization of Krukenberg tumor cell line, especially the biological relationship between cancer and stromal cells.

The signet-ring cell adenocarcinoma cell line (HSKT-C) and the fibroblast cell strain (HSKT-F) were established from a Krukenberg tumor. The HSKT-C cells are small, roundish or spindle-like in shape and form monolayer sheets of epithelial pavement cells and produce carcinoembryonic proteins such as carbohydrate antigen 19-9 (CA 19-9), carcinoembryonic antigen (CEA), tissue polypeptide antigen (TPA), etc. They show a stable proliferation after successful passages of 45 times within 13 months. The chromosome number varied widely and showed aneuploidy. The HSKT-C cells were transplanted to hamster cheek pouches and produced a tumor (signet-ring cell adenocarcinoma). On the other hand, HSKT-F cells are fibroblast-like. Their chromosome number is 46, and no karyological abnormality was observed. They could not be transplanted in the nude mouse or the hamster and did not produce carcinoembryonic proteins. It should be noted that they produce estrogens (estrone and 17 beta-estradiol). Sarcomatous morphological change of the stromal cells in Krukenberg tumor is considered to be a reactive change against invasion of the signet-ring cell adenocarcinoma into stromal tissues.

Aneuploidy

Multiple primary cancers in patients with initial laryngeal cancer.

The risk of a person developing a second primary cancer was evaluated in 1,215 patients with laryngeal cancer at the National Cancer Center Hospital. Overall, 92 (8.2%) of the male patients and 5 (5.7%) of the female patients developed a second cancer, compared with 83.0 and 3.7, respectively, expected on the basis of general population rates, resulting observed: expected values (O/E) to be 1.1 and 1.3. The numbers of second cancers of the lung (O/E = 1.9), oropharynx (O/E = 8.8) and esophagus (O/E = 2.8) were significantly in excess of those expected, while the number of second stomach cancers (O/E = 0.5) was far below expectation. Synchronous second cancers were significantly higher than expected (O/E = 4.6). Smoking, especially heavy smoking, was related to second lung cancers, but alcohol drinking featured less. Histories of benign respiratory tract and digestive organ diseases were related to second oropharyngeal cancers. Alcohol drinking was related to second stomach cancers. Radiation therapy for the initial laryngeal cancer was related to second oropharyngeal cancers, while hazardous occupations related to noxious agents for respiratory systems featured more prevalently in cases of second lung cancer. Further analytical studies should clarify the roles of smoking, drinking, occupation and various forms of therapy on the risk of developing a different second cancer following laryngeal cancer.

A study on the effectiveness of a thromboxane synthetase inhibitor (OKY-046) in increasing the survival length of skin flaps.

In an experimental study to test the thromboxane (TX) synthetase inhibitor OKY-046, two random-pattern skin flaps, each measuring 15.5 x 2 cm, and caudally based, were elevated on the backs of rabbits, and the effect of the test drug on their survival length was evaluated. The results indicated that the survival length of the skin flaps was 4.5 +/- 0.2 cm in the control group and 6.8 +/- 0.3 cm in the OKY-046-treated group, hence exceeding the control value by more than 50 percent, which was statistically significant. A laser speckle flow-meter showed that the OKY-046-treated flaps had significantly greater blood flow as compared with the control group both at 1 and 48 hours after operation. Whereas the blood flow values were significantly lower at 48 hours than at 1 hour after operation in the control group, no such reduction was noted in the OKY-046-treated group. On the other hand, while plasma TXB2 was found elevated at 1 hour postoperatively in the control group, such a response to the surgical intervention was blocked and the plasma TXB2/6-keto prostaglandin (PG) F1a ratio was decreased in the OKY-046-treated group. These results clearly indicated that OKY-046 suppressed a plasma thromboxane elevation induced by surgery, it augmented the flap blood flow, and it thereby increased flap survival length, suggesting that the drug might be helpful clinically and that further investigation must be carried out concerning its application.
