[Post-traumatic tunnel syndromes (clinical picture and treatment)].

The authors consider one of the key questions of modern neurotraumatology, namely, pathogenetic mechanisms of damage to peripheral nerves. On the basis of examining 66 patients the authors suggest that along with direct damage to nerves tunnel mechanisms play a major role in the pathogenesis of their damage. Different tunnel syndromes are described. The treatment is largely based on intratunnel novocain-hydrocortisone blocking in combination with other methods such as electrophoresis with lydasa and potassium iodide, barotherapy and drug therapy.

[Neurovascular syndromes of the neck, shoulder girdle and extremities].

The author summarizes the data of his long-term studies concerning one of the areas of neuroangiology: the problem of neurovascular syndromes. The latter ones can be identified as symptom-complexes of combined affections of the nervous and vascular systems associated with the compression and reflex mechanisms. Involved in their genesis are vertebral pathologies and anomalies, atherosclerosis of the major vessels, tonic and dystrophic changes of the muscles, etc. The report presents the first attempt at a classification of neurovascular syndromes and their detailed semiologic characteristics. The principles of conservative and surgical treatments are demonstrated.

[Clinico-electromyographic studies in compression and reflex syndromes of spinal osteochondrosis].

The authors carried out electromyographic (EMG) examinations of 115 patients with the compression and reflectory syndromes of vertebral osteochondritis using superficial and needle electrodes. The use of muscular innervation schemes developed by Soviet authors has made it possible to reveal a monoradicular pathology in 32 out of 54 patients examined. It was also found that the electromyographic phenomena characteristic for the pathology of the 1st sacral radicle were more marked than those characterizing the pathology of the Vth lumbar radicle. In patients with cervical osteochondrosis, prevalent were reflectory syndromes to which diffuse EMG changes corresponded. The EMG examinations with the use of needle electrodes revealed a low-amplitude spontaneous activity, a slight deceleration of the rhythm, and a lowered amplitude of the oscillations in the affected muscle. However especially demonstrative was a prolongation of the potential of the motor units on the side of the affection. The clinical, electromyographic, and roentgenological findings taken together contribute to a more precise diagnosing of the compression and reflectory syndromes of vertebral osteochondritis.

[Modeling the reflex syndrome of the piriform muscle].

In order to model the piriform muscle syndrome the author utilized clinical and informational-statistical methods. For the same purpose an algorhythm was formed from 2 programmes: the method of hemogeneity in selected medium and correlational analysis. As a result it was possible to create a clinico-statistical model of the main syndromes of a lumbar osteochondrosis, including the model of the piriform syndrome. On the basis of the achieved data the author proposes different differential diagnostical tables of syndromes of lumbar osteochondrosis which can be used for computer and noncomputer diagnosis.

[Clinico-electromyographic characteristics of piriform muscle syndromes].

The report is concerned with the screening methods of lesions of the piriformis muscle in the piriformis syndrome which is more frequently connected with sacral osteochondrosis. However, this syndrome may sometimes appear in an ankylosis of the hip joint. All the diagnostical methods may be divided into 3 groups: indirect clinical method, palpation and EMG. The first method does not require special experience. A palpation of the piriformis muscle is conducted bimanually by a method proposed by the authors. EMG studies with the aid of needle electrodes permit to distinguish spasms and more crude changes in the piriform muscle.
