[Treatment of chest injuries in intensive care. Critical review of 54 clinical cases].

All 54 chest trauma patients admitted to Resuscitation II of Pavia's S. Matteo Polyclinic in the period 1974-1979 have been reviewed in a critical analysis. Particular attention was paid to mortality in relation to age, associated pathology, treatment and the complications arising during the course of the trauma. Resuscitation Centres are identified as the ideal places for treating these patients. Cranio-encephalic and superimposed abdominal lesions are the main factors pointing to an unfavourable prognosis, and protection against infectious complications is the key to therapy.

[The use of doxapram chlorohydrate (t-doxapril) as an anti-shivering drug].

Following a brief review of the main causes of anaesthetic shock, reference is made to the results obtained with the use of a well-known analeptic: doxapram chlorohydrate, as an anti-shock drug. Using a continuous venous perfusion technique, practically no side-effects were encountered, while the effectiveness of the drug was of undoubted interest (75% of cases). The hypothesis of the anti-shock action remains open.

Tumor necrosis factor alpha levels in serum and cerebrospinal fluid of patients with AIDS.

We used a sensitive enzyme-linked immunosorbent assay technique to measure tumor necrosis factor alpha (TNF alpha) levels in serum and cerebrospinal fluid (CSF) samples from 30 patients infected with human immunodeficiency virus type 1 and from 10 normal controls. We found detectable levels of TNF alpha in 19 of 30 CSF and in 17 of 30 serum samples. The values of TNF alpha ranged between 20-90 pg/ml. All the patients had overt AIDS. More elevated TNF alpha levels in CSF correlate with focal damage within the central nervous system (p less than 0.01). Our results suggest that an intrathecal production of TNF alpha may occur during active inflammation in course of AIDS.

[Experimental EMG-anatomopathological study of rabbits treated with Cialit grafts for the repair of sciatic nerve loss of substance].

The sciatic nerves of 10 rabbits of the New Zealand strain were cut and isolated in the latero-posterior region of the thigh under general anesthesia. Immediate homograft technique was used in animals. The maximal motor conduction velocity of the sciatic nerve was measured in all the animals before the cutting of the nerve. Long-term EMG and anatomo-pathological controls were carried out. The results obtained are discussed. Further research will be continued by applying new techniques and new EMG parameters as well as more sophisticated anatomo-pathological techniques.
