[The course of behaviour changes in children with attention deficit and hyperactivity after drug treatment].

INTRODUCTION: Several studies have reported differences in behaviour in patients with various subtypes of attention deficit with hyperactivity and the benefits of psychostimulant medication in the treatment of behaviour problems of patients with the attention deficit hyperactivity syndrome (TDAH). OBJECTIVES: 1. To determine possible differences in behaviour between patients with subtypes of the attention deficit hyperactivity disorder; 2. To analyze the efficacy of methylphenidate on behaviour in three subtypes of TDAH, evaluated on the opinions of patients and teachers. PATIENTS AND METHODS: A total of 90 children were studied: 39 children with TDAH-C; 36 with TDAH-1 and 15 with TDAH-H/I. All patients were given 0.5 mg/kg of methylphenidate for a period of three months, once in the morning and once in the afternoon. RESULTS: The results show that the children with the three subtypes of TDAH have significant differences in all the behaviour variables studied except for the variable timidity anxiety, in which no differences were observed between the subtypes of TDAH. Statistical analysis also showed that children with all three subtypes of TDAH improved significantly with regard to most of the aspects of behaviour studied after psychostimulant medication, in the opinions of parents and teachers. CONCLUSIONS: Our findings are similar to those of other studies and give fresh data on the behaviour and advantages of methylphenidate in the new subtype included in the DSM-1V, the predominantly hyperactive impulsive type.

[Histologic study of the effect of the CO2 laser and the cicatrization of the tongue in the rat].

In the present paper we analyze the effect of CO2 laser in the mouse tongue, in relation with the intensity and exposition time. We study the histology of the lesions and their healing. We show that with 15 w of intensity the effect is bigger tan supposed affecting nearly to the hole tongue. This experience make us to conclude that there is the possibility of harming the perichondrium and cartilage when using the laser in the laryngeal mucosa.

[Alternating hemiplegia in young babies].

INTRODUCTION. This paper review the state of the art of alternating hemiplegia of childhood. This entity is a rare neurologic disorder of infancy characterize by transient hemiplegic spells shifting from one side to another, and occasionally affecting to both hemispheres at the same time. Usually start before 18 months, many cases exhibit neonatal symptoms related with the disorder. Early symptomatology include abnormal ocular movements, mainly nystagmus, and tonic or dystonic attacks generally beginning before 6 months of age. These symptoms are frequently misdiagnosis as epilepsy. Typical hemiplegic fits which disappeared when infant felt asleep appeared by 12 months of age. Diagnosis is on clinical basis after excluding any other causes of fluctuating and transitory hemiplegic attacks. Complementary investigations, such as electroencephalograms, TAC, MRI, angiographic MRI, CFS are strictly normal. Cerebral SPECT show controversial abnormalities in some studies. We perform SPECT study in a alert 12 months old girl during an interictal period resulting in a non significant asymmetry, with lesser perfusion in left frontal basal and anterior temporal gyrus. Nevertheless, we observed an striking hyperperfusion in middle occipital area. Treatment is symptomatic with flunarizine. Our patient experience a dramatic decrease in frequency, duration and severity of hemiplegic attacks.

[The effects of psychostimulants on the cognitive results and behaviour of children with the combined subtype of attention deficit with hyperactivity].

INTRODUCTION: Many studies have shown the efficacy of methylphenidate in treating children with attention deficit and hyperactivity to reduce the main symptoms of the disorder and problems of behaviour: behaviour which is disruptive, antisocial, negative and aggressive. OBJECTIVES: 1. Analyze the therapeutic effects of methylphenidate on the cognitive function of children with TDAH, combined subtype, evaluated by means of neuropsychological tests of attention and inhibitory control. 2. Determine the efficacy of methylphenidate in improving the basic symptoms of TDAH C according to the DSM 1V for parents and teachers, and their behaviour, evaluated by their teachers, in the combined subtype. PATIENTS AND METHODS: The sample was made up of 48 children with TDAH C. 24 of these had been referred by paediatricians and received drug treatment with methylphenidate, 0.5 mg/kg once in the morning and once after lunch (TDAH C/CTF). The other group of 24 children with TDAH C were referred from their teachers to a school psychologist so that they followed the usual guidelines for orientation but not a systematically developed treatment (TDAH C/C or control group). RESULTS. In relation to the first objective the results were good for the TDAH C/CTF compared with the TDAH C/C group regarding the proportion of children who improved their results on neuropsychological testing of planning inhibitory control and attention. The differences between the two groups were statistically significant in the latent period of the reflexive impulsive test (MFF), arithmetic, cancellation of rhomboids and cancellation of numbers. Regarding the second objective, the improvement in the TDAH C/CTF group according to the teachers opinions were statistically significant with respect to attention, hyperactivity and impulsiveness, as compared with the control group. However, according to the parents assessment, although improvement was greater in both dimensions, the differences between the two groups did not reach statistical significance. Finally, the teachers observed greater improvement in the medicated group as compared to the control group regarding three variables: learning problems, antisocial conduct and failure to adapt to school. CONCLUSIONS: Overall, our results coincide with those of other excellent reviews, which have shown the effectiveness of methylphenidate in the treatment of cognitive and behaviour disorders in this group.

Attention Deficit Disorder with Hyperactivity↗

[Attention deficit hyperactivity disorder and epilepsy].

INTRODUCTION: Attention deficit hyperactivity disorder (ADHD) is currently one of the morbid clinical pictures in childhood and adolescence that is most frequently diagnosed in the Neuropaediatric clinic. DEVELOPMENT: ADHD is sometimes associated to other important neurological problems, such as behavioural disorders, tics or headaches, and may occasionally be related to epilepsy, although in this particular case the epileptic syndromes often involve disorders affecting behaviour and attention. Due to the frequent association of these disorders (epilepsy and ADHD) it is wise to perform an electroencephalographic study in patients who visit because of symptoms of disattention. Likewise, the neuropsychological assessment of epileptic children must not be neglected. We analyse the therapeutic options regarding the use of stimulants when both disorders coexist and we endorse the association of antiepileptic and psychostimulant drugs when necessary.

[Auditory evoked potentials of the brain stem in the study of tinnitus].

A brain stem evoked response study is carried out, over a group of tinnitus patient. The purpose of this work is to evidence the electrophysiologic indices indicating tinnitus presence. Morphology tracing alterations, variability in the wave latency, also important decreasing in the amplitude of the wave V were found.

[Oropharyngeal functional evaluation in patients subjected to total laryngectomy with phonatory prosthesis. Manometric and isotopic study].

Our goal in this paper is to evaluate the motor function of the oropharinx using esophageal manometry and isotopic study, in patients that underwent total laryngectomy (TL) with phonatory prosthesis (PP), and with or without cricopharyngeal myotomy (CM). 50 por 100 of the laryngectomized patients showed oropharyngeal dysphagia, more frequent and stronger in patients without CM. Voice quality was considered good or medium in 12/20. With manometry we achieve that after TL there is an intense pressure reduction of the UES, higher in patients with CM, there aren't differences for other manometrics parameters. The oropharyngeal isotopic clearance is clearly disturbed in all patients, there aren't differences due to the type of surgery, neither the degree of dysphagia nor the CM. The association of a CM to PL plus PP is a surgical procedure that improves markedly the manometric and clinic results in these patients.
