[Plasminogen activators (t-PA and u-PA) and other fibrinolysis parameters in patients with atherosclerosis obliterans and diabetic macroangiopathy].

UNLABELLED: The aim of the study was to evaluate plasminogen activators in compare fibrinolysis parameters in patients with atherosclerosis obliterans (AO) and diabetic macroangiopathy (DM) of lover extremities. The study group consist of 40 patients with AO (mean age 59 years) and 40 with DM (mean age 61 years). All the patients were diagnosed as the II stage in Fontaine scale. Ischemia lover extremities were: aorta-hip, femoral-popliteal and peripheral dependent closure. The control group consist of 30 healthy volunteers (mean age 51 years). Concentration of: tissue plasminogen activator antigen (t-PA:Ag), urokinase plasminogen activator antigen (u-PA:Ag), plasminogen activator inhibitor type 1 antigen (PAI-1:Ag) (ELISA), PAI-1 activity (PAI-1 act.), euglobulin lysis time (ELT) (Kowarzyk-Buluk method), fibrinogen, fibrin degradation products (FDP) (Merskey method) in blood plasma were evaluated. The results shows statistically higher concentrations of: t-PA:Ag, PAI-1:Ag, fibrinogen, and lower concentrations of u-PA:Ag and elongated ELT in blood plasma patients with AO and DM in compare with healthy volunteers. Comparing analysed parameters between patient groups shows higher concentration FDP in patients with AO than patients with DM. CONCLUSIONS: 1) Although elongated ELT in both groups it was shown evidence of the fibrinolysis activation in the shape of elevated FDP concentration. It was probably compensation for hypercoagulability. 2) Elevated t-PA:Ag concentration in patients with AO and DM is an evidence of the higher releasing of the endothelium in AO and DM.

Evaluation of 8-oxodeoxyguanosine, typical oxidative DNA damage, in lymphocytes of ozone-treated arteriosclerotic patients.

In the present study we measured the amount of 8-oxo-2'-deoxyguanosine (8-oxo-dG) in DNA isolated from lymphocytes of arteriosclerotic patients undergoing ozonetherapy. Treatment of the patients with therapeutic concentration of ozone caused a significant increase over the control value in the amount of 8-oxo-dG of DNA isolated from their lymphocytes. However, only three out of six patients examined responded positively to the treatment in terms of the base damage. The increases varied among patients, and were in the range of 100-450%. This interindividual difference may at least be partly explained by recently demonstrated heritable susceptibility to ozone.

[The influence of ozone therapy on endothelial damage markers in patients with atherosclerosis of lower extremities].

The aim of study was to evaluate the influence of the treatment with oxygen-ozone mixture on the blood plasma antigen concentration of tissue plasminogen activator (t-PA) and von Willebrand factor (vWF) in patients suffering from atherosclerotic disease of lower extremities. The study was performed in the group of 28 (M/F 22/6) patients means aged 64.1 years with atherosclerotic diseases of lower extremities, in whom 2 weeks therapy with oxygen-ozone mixture was used. The control group consisted of 30 healthy volunteers in mean age 51.0 years. In the blood plasma obtained from the patients before and after treatment with oxygen-ozone mixture and from the control group determinations of t-PA and vWF antigen using ELISA were done. Both parameters were significantly increased in the patients before the treatment in comparison to the healthy controls. The treatment with oxygen-ozone therapy caused in patients slight statistically not significant raise of t-PA and vWF antigen showing the endothelial stimulation but not the destruction of vascular endothelium.

[Results of kinesis therapy in patients with diabetic macroangiopathy of the lower extremities].

In 30 patients with diabetes type II and macroangiopathy of lower extremities, physical training was applied. This treatment was carried on in our Clinic for average 4 weeks. The following methods were applied: individual exercises of legs once or twice daily for 30-minutes, training in brine swimming pool for 30 minutes every-day, treatment walks with proportioning effort. Value of physical effort was not more than 30-40% max. pulse rate. The examination was performed before and after the programme of treatment. The examination included the following parameters: subjective test on cycloergometer, examination of blood flow velocity by Doppler method and biochemical examination of glycaemia and lipidemia. We found improvement in intermittent claudication distance, significant subjective improvement in velocity of blood flow in lower extremities. In our opinion physical vascular training is a valuable method of mono-operative treatment of vascular disease of legs in diabetic patients.

[Dietary habits of obese patients with type 2 diabetes mellitus].

For assessment of the way of nutrition of obese patients with diabetes mellitus type 2 (insulin-dependent), questionnaire method was used including dietary history data. The study was carried out in 50 patients during weight-losing treatment in our institute. It was found that the mean energy value of daily food ration of the patients was relatively high, exceeding both the value of predicted energy expenditure and the standard values. This was not true, however, of all patients. Among the studied patients, 32% of persons were not eating excessively, and some of them ate even less than the standards; only 24% of obese patients could have been regarded as hyperfagous. Significant dietary faults concerned also the food composition, that is mainly excessive fat content which accounted even for 46% of calorie value of daily food ration. Among other dietary faults the following should be mentioned: low vegetable content, additional eating between meals, and irregular taking of meals. The weight-losing treatment with simultaneously conducted health education contributed to the change of dietary habits of the observed patients.

[Treatment results of diabetes type 2 with obesity in the elderly].

The aim of these investigations were the estimation of methods and results of treatment obese diabetics over 65 years and presentation of the most common mistakes of treatment. Two groups of elderly obese diabetics type 2 were compared. The first group involved the patients who kept closely to a low caloric diet and they achieved a decrease of their body mass. The second group consisted of the patients who failed to keep to a low caloric diet and their body mass after treatment either remained the same or increased. In almost all the cases of the first group a significant improvement was observed. The improvement manifested itself by the decrease of glycemia and glycosuria as well as by a lower demand for hypoglycemic drugs. In the second group the decrease of hypoglycemic drugs was not possible and in many cases the increase of hypoglycemic drug doses was necessary. The body mass reduction in obese diabetics also those of advanced years is a significant factor in the process of treatment and it is equally important to pharmacotherapy.

[Blood levels of insulin and growth hormone during exercise test in obese patients with normal glucose tolerance and with diabetes mellitus after food intake and fasting].

Ten obese patients with normal glucose tolerance, 11 obese with type II diabetes and 15 normal non-obese controls were studied. The submaximal exercise test was made before fasting in all the mentioned 3 groups of patients and in the obese patients after a 10 days fast. The exercise on normal diet did not cause any changes in serum insulin in all groups. The growth hormone secretion, however, rose in all the groups studied. Fasting did not modify the insulin secretion following the exercise in both groups of the obese. The growth hormone secretion, exercise stimulated, was elevated by fasting as well in the obese with normal and abnormal glucose tolerance. We believe that a marked energetic deficit due to exercise on fasting may supplement the deficit of the growth hormone which usually occurs in obesity. This might be therapeutically usefull in treating obesity in selected cases.

[Physiotherapy of cervical migraine in the light of personal experience].

The study aimed at developing optimal technique of the physical treatment of headache in the course of cervical migraine. The study involved 65 patients with headache accompanying degenerative lesions to the cervical spine. The patients were divided into two groups. Routine balneokinesitherapy was employed in one group of 25 patients whereas diadynamic current of specially adjusted parameters were additionally used in the second group of 40 patients. Markedly better results were achieved in this group as headache was nearly completely relieved in 77.5% and significantly reduced in 22.5% of the patients. In the first group of patients headache disappeared in 12% of the treated patients, was reduced in 52%, and did not change in 36% of the treated patients. Multifactorial etiopathogenesis of cervical migraine advocates the use of complex therapeutical means. Discussed therapy involving balneokinesitherapy and diadynamic currents makes a complex of factors acting on various pathogenetic elements of the disease, leading to favourable effects. Employed techniques are simple, available and free of adverse reactions.
